How to study biofilms: technological advancements in clinical biofilm research.

ABSTRACT: Biofilm formation is an important survival strategy commonly used by bacteria and fungi, which are embedded in a protective extracellular matrix of organic polymers. They are ubiquitous in nature, including humans and other animals, and they can be surface- and non-surface-associated, making them capable of growing in and on many different parts of the body. Biofilms are also complex, forming polymicrobial communities that are difficult to eradicate due to their unique growth dynamics, and clinical infections associated with biofilms are a huge burden in the healthcare setting, as they are often difficult to diagnose and to treat. Our understanding of biofilm formation and development is a fast-paced and important research focus. This review aims to describe the advancements in clinical biofilm research, including both in vitro and in vivo biofilm models, imaging techniques and techniques to analyse the biological functions of the biofilm.

Project description:Plant production systems such as plant factories and greenhouses can help promote resilience in food production. These systems could be used for plant protection and aid in controlling the micro- and macro- environments needed for optimal plant growth irrespective of natural disasters and changing climate conditions. However, to ensure optimal environmental controls and efficient production, several technologies such as sensors and robots have been developed and are at different stages of implementation. New and improved systems are continuously being investigated and developed with technological advances such as robotics, sensing, and artificial intelligence to mitigate hazards to humans working in these systems from poor ventilation and harsh weather while improving productivity. These technological advances necessitate frequent retrofits considering local contexts such as present and projected labor costs. The type of agricultural products also affects measures to be implemented to maximize returns on investment. Consequently, we formulated the retrofitting problem for plant production systems considering two objectives; minimizing the total cost for retrofitting and maximizing the yearly net profit. Additionally, we considered the following: (a) cost of new technologies; (b) present and projected cost for human labor and robotics; (c) size and service life of the plant production system; (d) productivity before and after retrofit, (e) interest on loans for retrofitting, (f) energy consumption before and after retrofit and, (g) replacement and maintenance cost of systems. We solved this problem using a multi-objective evolutionary algorithm that results in a set of compromised solutions and performed several simulations to demonstrate the applicability and robustness of the method. Results showed up to a 250% increase in annual net profits in an investigated case, indicating that the availability of all the possible retrofitting combinations would improve decision making. A user-friendly system was developed to provide all the feasible retrofitting combinations and total costs with the yearly return on investment in agricultural production systems in a single run.

Project description:Electrochemotherapy, a combination of high voltage electric pulses and of an anticancer drug, has been demonstrated to be highly effective in treatment of cutaneous and subcutaneous tumors. Unique properties of electrochemotherapy (e.g., high specificity for targeting cancer cells, high degree of localization of treatment effect, capacity for preserving the innate immune response and the structure of the extracellular matrix) are facilitating its wide spread in the clinics. Due to high effectiveness of electrochemotherapy in treatment of cutaneous and subcutaneous tumors regardless of histological origin, there are now attempts to extend its use to treatment of internal tumors. To advance the applicability of electrochemotherapy to treatment of internal solid tumors, new technological developments are needed that will enable treatment of these tumors in daily clinical practice. New electrodes through which electric pulses are delivered to target tissue need to be designed with the aim to access target tissue anywhere in the body. To increase the probability of complete tumor eradication, the electrodes have to be accurately positioned, first to provide an adequate extent of electroporation of all tumor cells and second not to damage critical healthy tissue or organs in its vicinity. This can be achieved by image guided insertion of electrodes that will enable accurate positioning of the electrodes in combination with patient-specific numerical treatment planning or using a predefined geometry of electrodes. In order to be able to use electrochemotherapy safely for treatment of internal tumors located in relative proximity of the heart (e.g., in case of liver metastases), the treatment must be performed without interfering with the heart's electrical activity. We describe recent technological advances, which allow treatment of liver and bone metastases, soft tissue sarcomas, brain tumors, and colorectal and esophageal tumors. The first clinical experiences in these novel application areas of electrochemotherapy are also described.

Project description:BackgroundPatient recruitment and data management are laborious, resource-intensive aspects of clinical research that often dictate whether the successful completion of studies is possible. Technological advances present opportunities for streamlining these processes, thus improving completion rates for clinical research studies.ObjectiveThis paper aims to demonstrate how technological adjuncts can enhance clinical research processes via automation and digital integration.MethodsUsing one clinical research study as an example, we highlighted the use of technological adjuncts to automate and streamline research processes across various digital platforms, including a centralized database of electronic medical records (enterprise data warehouse [EDW]); a clinical research data management tool (REDCap [Research Electronic Data Capture]); and a locally managed, Health Insurance Portability and Accountability Act-compliant server. Eligible participants were identified through automated queries in the EDW, after which they received personalized email invitations with digital consent forms. After digital consent, patient data were transferred to a single Health Insurance Portability and Accountability Act-compliant server where each participant was assigned a unique QR code to facilitate data collection and integration. After the research study visit, data obtained were associated with existing electronic medical record data for each participant via a QR code system that collated participant consent, imaging data, and associated clinical data according to a unique examination ID.ResultsOver a 19-month period, automated EDW queries identified 20,988 eligible patients, and 10,582 patients received personalized email invitations. In total, 1000 (9.45%) patients signed consents to participate in the study. Of the consented patients, 549 unique patients completed 779 study visits; some patients consented to the study at more than 1 time period during their pregnancy.ConclusionsTechnological adjuncts in clinical research decrease human labor while increasing participant reach and minimizing disruptions to clinic operations. Automating portions of the clinical research process benefits clinical research efforts by expanding and optimizing participant reach while reducing the limitations of labor and time in completing research studies.
