Natural Multifunctional Silk Microcarriers for Noise-Induced Hearing Loss Therapy.

ABSTRACT: Noise-induced hearing loss (NIHL) is a common outcome of excessive reactive oxygen species in the cochlea, and the targeted delivery of antioxidants to the inner ear is a potential therapeutic strategy. In this paper, a novel natural biomaterials-derived multifunctional delivery system using silk fibroin-polydopamine (PDA)-composited inverse opal microcarriers (PDA@SFMCs) is presented for inner ear drug delivery and NIHL therapy. Due to their large specific surface area and interpenetrating nanochannels, PDA@SFMCs can rapidly load active biomolecules making them a convenient medium for the storage and delivery of such molecules. In addition, surface modification of PDA enables the microcarriers to remain in the round window niche, thus facilitating the precise local and directed delivery of loaded drugs. Based on these features, it is demonstrated here that n-acetylcysteine-loaded silk microcarriers have satisfactory antioxidant properties on cells and can successfully prevent NIHL in guinea pigs. These results indicate that the natural multifunctional silk microcarriers are promising agents for local inner ear drug delivery in the clinic.

Project description:Noise-induced hearing loss (NIHL) is a prevalent form of adult hearing impairment, characterized by oxidative damage to auditory sensory hair cells. Although certain dihydropyridines, the L-type calcium channel blockers, exhibit protective properties against such damage, the ability of third-generation dihydropryidines like lercanidipine to mitigate NIHL remains unclear.We utilized glucose oxidase (GO)-treated OC1 cell lines and cochlear explants to evaluate the protective influence of lercanidipine on hair cells. To further investigate its effectiveness, we exposed noise-stimulated mice in vivo and analyzed their hearing thresholds. Additionally, we assessed the antioxidative capabilities of lercanidipine by examining oxidation-related enzyme expression and levels of oxidative stress markers, including 3-nitrotyrosine (3NT) and 4-hydroxynonenal (4HNE). Our findings demonstrate that lercanidipine significantly reduces the adverse impacts of GO on both OC-1 cell viability (0.3 to 2.5 µM) and outer hair cell (OHC) survival in basal turn cochlear explants (7 µM). These results are associated with increased mRNA expression of antioxidant enzyme genes (HO-1, SOD1/2, and Txnrd1), along with decreased expression of oxidase genes (COX-2, iNOS). Crucially, lercanidipine administration prior to, and following, noise exposure effectively ameliorates NIHL, as evidenced by lowered hearing thresholds and preserved OHC populations in the basal turn, 14 days post-noise stimulation at 110 dB SPL. Moreover, our observations indicate that lercanidipine's antioxidative action persists even three days after simultaneous drug and noise treatments, based on 3-nitrotyrosine and 4-hydroxynonenal immunostaining in the basal turn. Based on these findings, we propose that lercanidipine has the capacity to alleviate NIHL and safeguard OHC survival in the basal turn, potentially via its antioxidative mechanism. These results suggest that lercanidipine holds promise as a clinically viable option for preventing NIHL in affected individuals.

Project description:ObjectiveFarmworkers who have prolonged exposure to loud noise are at risk for disabling hearing loss. The objectives of this scoping review are to (1) identify and summarize the evidence on noise-induced hearing loss in farmworkers, (2) describe instruments used to evaluate hearing loss outcomes, (3) describe testing approaches and limitations, and (4) provide recommendations for future studies that seek to quantify hearing loss in this population.MethodsWe performed a systematic search of three electronic databases, PubMed, CINAHL, and Scopus, to identify articles related to noise-related hearing loss in farmworkers. Our search was guided by Arksey and O'Malley's methodological framework and PRISMA-ScR guidelines.ResultsA total of 57 articles met inclusion criteria. The majority of studies were undertaken in North America (n = 32, 56%), and most were in the midwestern United States. Farmworkers tended to be white, male, and work in crop agriculture. A total of 47 studies (82%) used audiometry to measure farmworkers' hearing sensitivity, among which testing was carried out at various locations, including hospitals, clinics, farmworkers' homes, and worksites. The criteria for defining hearing loss varied across studies making it difficult to summarize results. Among 14 studies that used a cutoff point greater than 25 decibels, the prevalence of hearing loss ranged between 46 and 98%. Subjective outcomes (used in 14 studies) were typically assessed using a variety of researcher-developed questions. The prevalence of hearing difficulties in this category was as high as 87%.ConclusionHearing loss is prevalent across studies and does not appear to decrease over the years. Our findings call for more research among diverse farmworker populations. Further, given the high prevalence of hearing loss in many of the studies reviewed, there is clearly a need to develop strategies to protect farmworkers from noise exposure and noise-induced hearing loss.
