Mucosal Melanoma Clinical Management and Prognostic Implications: A Retrospective Cohort Study.

ABSTRACT: Mucosal melanoma (MM) is an uncommon melanoma subtype affecting mucosal surfaces of the head and neck, anorectal region, and vulvovaginal area. We aimed to present our experience at a tertiary-level hospital regarding MM diagnosis, management, monitoring of progression, mutations, and outcome predictors. We performed a registry-based cohort study including MM cases diagnosed from 2012 to 2022 and retrospectively characterized somatic mutations on BRAF, NRAS and c-KIT. We employed Kaplan-Meier curves, log-rank tests, and Cox regression analysis to explore prognostic factors and survival outcomes in a cohort of 35 patients, mainly women (63%) with a median age of 70 years. Predominantly, MM occurred in the vulvovaginal region (48.6%). At diagnosis, 28.6% had lymph node involvement, and 31.4% also had distant metastasis. Mutations in BRAF and c-KIT were identified in 3/35 (9%) and 2/6 patients (33%), respectively. Surgery was performed in 71.4% of patients, and most received systemic treatment (65.7%). Lower disease stage, thinner Breslow depth, and surgical resection were associated with improved overall survival. Notably, age, sex, radiotherapy, and BRAF mutant status did not affect survival. Standard management typically involves immunotherapy. Cases with BRAF or c-KIT mutations may be considered for targeted therapies. Unfortunately, MM prognosis remains unfavorable, with a less than 50% survival rate at 2 years.

Project description:Background: Colorectal liver metastases (CRLM) are the leading cause of colorectal cancer (CRC)-related mortality. Transfer RNA-derived fragments (tRFs), a novel class of small non-coding RNAs (sncRNA), regulate gene expression, stress response, and immune functions in cancer. While tRFs are increasingly implicated in CRC progression, their prognostic significance in CRLM remains unknown. This study investigates the abundance and prognostic value of genomic (ge) and mitochondrial (mt) tRFs in CRLM. Methods: This single-center retrospective cohort study included all CRLM patients who underwent curative liver resection between January 2012 and December 2015. Small RNA sequencing (sRNA-seq) quantified ge- and mt-tRF expression in tumor samples. Event-free survival (EFS) was the primary outcome, and the impact of tRFs on EFS was assessed using Cox regression, spline modeling of the effects of tRFs on EFS, and network analysis. Results: Among 588 screened samples, 40 met eligibility criteria (18 females [45%], median age 64 [42-79]). A total of 432 tRFs were identified, with ge-tRFs (67%) being more abundant than mt-tRFs (33%). Using spline regressions, tRFs were classified into 10 ten prognostic groups. High ge-tRF abundance correlated predominantly with unfavorable EFS (FDR<0.2; 94%), whereas mt-tRFs were significantly (p<0.001; χ2 test) more often associated with favorable EFS (FDR<0.2; 26%). Network analysis of tRF abundance correlations revealed a significantly higher intra-mitochondrial network density compared to the intra-genomic tRF network. However, no significant differences in network structure were observed between prognostically significant vs. non-significant or favorable vs. unfavorable tRFs. Finally, key tRF candidates, such as tRHalve3-His-CAU (mt-tRF) or tRNAleader-Gln-UUG (mt-tRF) paired with tRFmisc-Tyr-GTA (ge-tRF), remained independent prognostic markers after adjustment for clinical covariates. Conclusion: This study provides the first comprehensive characterization of tRF expression in CRLM, revealing distinct prognostic roles for ge- and mt-tRFs. While ge-tRFs were predominantly associated with unfavorable prognosis, a subset of mt-tRFs demonstrated a favorable impact on EFS. These findings highlight the potential of tRFs as novel prognostic biomarkers and therapeutic targets in CRLM, warranting validation in prospective studies.

Project description:BackgroundThe mainstay of endometrial cancer treatment is surgical resection of tumors and postoperative adjuvant treatment is recommended if necessary. However, there is no consensus on the management of unresectable metastatic endometrial cancer. This study aimed to assess the feasibility and effectiveness of neoadjuvant chemotherapy followed by interval debulking surgery (NAC-IDS) in unresectable, metastatic endometrial cancer.MethodsFrom the endometrial cancer cohorts of four institutions in Korea, we identified patients with International Federation of Gynecology and Obstetrics stages IIIC-IVB endometrial cancer who received NAC-IDS between January 2008 and December 2020. Through a medical record review, we collected patients' clinicopathological data. Progression-free survival (PFS), overall survival (OS), and the factors affecting survival outcomes were analyzed.ResultsOverall, 32 patients were included with endometrioid (n = 18), serous (n = 5), carcinosarcoma (n = 6), and other histological types (n = 3). Among them, 28 (87.5%) patients had stage IVB disease. The most common neoadjuvant chemotherapy (NAC) regimen was paclitaxel-carboplatin (n = 25, 78.1%), which was administered for a median of six cycles. While 26 (81.3%) patients showed an objective response, two (6.3%) progressed despite NAC. At the time of interval debulking surgery (IDS), 23 (71.9%) patients achieved complete cytoreduction. During 31.0 months of the median follow-up, there were 23 recurrences and 11 deaths, corresponding to a median PFS of 19.7 months and a 3-year OS rate of 69.7%. In multivariate analyses, non-endometrioid histology and residual tumor after IDS were identified as independent poor prognostic factors for PFS (adjusted hazard ratio [HR], 7.322; P < 0.001 and 5.934; P = 0.001, respectively). Multivariate analysis for OS could not be conducted because of the small number of events, although non-endometrioid histology was the only factor associated with worse OS in univariate analysis (adjusted HR, 4.523; P = 0.032).ConclusionsNAC-IDS may be a treatment option for unresectable metastatic endometrial cancer. Tumor histology and the possibility of complete cytoreduction are the primary considerations for NAC-IDS.
