Case report: Rare case of multinodular and vacuolar neuronal tumors in the cerebellum.

ABSTRACT: Multinodular and vacuolar neuronal tumor (MVNT) is a rare and benign neuroepithelial tumor. Most reports describe tumors located in the cerebral hemisphere. A literature review found that 15 cases were located in the posterior cranial fossa, but all lacked pathological evidence. In this case, a patient sought medical attention due to insomnia and irritability. Neuroepithelial tumors were found in the imaging, and the patient underwent radiation therapy. Three years later, malignant tumors were found upon imaging examination. After surgical resection and pathological testing, MVNT occurring in the cerebellum was diagnosed. MVNT is rare in the cerebellum, and direct imaging diagnosis becomes difficult after treatment. Therefore, our report of this case helps to further accurate understanding of the imaging, pathological, and molecular genetic changes occurring before and after MVNT treatment, and will improve the accuracy of pre-treatment diagnosis and reduce the likelihood of overtreatment.

Project description:Renal leiomyosarcomas (LMS) are extremely rare neoplasms with aggressive behaviour and poor survival prognosis. The most frequent somatic events in leiomyosarcomas are mutations in TP53, RB1, ATRX and PTEN genes, chromosomal instability and chromoanagenesis. By using chromosomal microarray analysis we identified monosomy of chromosomes 3 and 11, gain of Xp (ATRX) arm and three chromoanasynthesis regions (6q21-q27, 7p22.3-p12.1 and 12q13.11-q21.2), with MDM2 and CDK4 oncogenes copy number gains, whereas no CNVs or tumor specific SNVs in TP53, RB1 and PTEN genes were observed.

Multinodular and vacuolating neuronal tumors in epilepsy: dysplasia or neoplasia?

Project description:Multinodular and vacuolating neuronal tumor (MVNT) is a new pattern of neuronal tumour included in the recently revised WHO 2016 classification of tumors of the CNS. There are 15 reports in the literature to date. They are typically associated with late onset epilepsy and a neoplastic vs. malformative biology has been questioned. We present a series of ten cases and compare their pathological and genetic features to better characterized epilepsy-associated malformations including focal cortical dysplasia type II (FCDII) and low-grade epilepsy-associated tumors (LEAT). Clinical and neuroradiology data were reviewed and a broad immunohistochemistry panel was applied to explore neuronal and glial differentiation, interneuronal populations, mTOR pathway activation and neurodegenerative changes. Next generation sequencing was performed for targeted multi-gene analysis to identify mutations common to epilepsy lesions including FCDII and LEAT. All of the surgical cases in this series presented with seizures, and were located in the temporal lobe. There was a lack of any progressive changes on serial pre-operative MRI and a mean age at surgery of 45 years. The vacuolated cells of the lesion expressed mature neuronal markers (neurofilament/SMI32, MAP2, synaptophysin). Prominent labelling of the lesional cells for developmentally regulated proteins (OTX1, TBR1, SOX2, MAP1b, CD34, GFAPδ) and oligodendroglial lineage markers (OLIG2, SMI94) was observed. No mutations were detected in the mTOR pathway genes, BRAF, FGFR1 or MYB. Clinical, pathological and genetic data could indicate that MVNT aligns more with a malformative lesion than a true neoplasm with origin from a progenitor neuro-glial cell type showing aberrant maturation.

Bilateral sporadic carotid body tumors-A rare case report.

Project description:Carotid body tumor presents as palpable mass in anterolateral aspect of the neck. However, bilateral sporadic carotid body tumor is a rare anomaly, reported to occur in 5% of population. We are going to report the case of a 60-year-old patient who presented to our department with painless, palpable mass bilaterally in anterolateral aspects of the neck. He did not have any family history of neck masses. This paper summarizes the topic of carotid body tumor and discusses the radiological and clinical implications of this condition.

A rare coexistence of papillary carcinoma and anaplastic carcinoma of thyroid in multinodular goitre: Case report and literature review.

Project description:IntroductionMultinodular goiter is defined as multiple discrete nodules in the thyroid gland. The incidence of Papillary carcinoma thyroid was found to be highest out of total Multinodular Goiter cases while that of Anaplastic carcinoma was the least. We report a rare coexistence of Papillary carcinoma and Anaplastic carcinoma in adult patient with a long-standing Multinodular Goiter.Case presentationHere we present a case of 54 years male with huge anterior neck swelling for 20 years with a gradual increase in size. Computerized tomography of neck revealed solidocystic mass lesion without any significant lymphadenopathy, features suggesting Multinodular goiter with differential diagnosis of Carcinoma Thyroid. Cytological examination showed Papillary thyroid Carcinoma. He underwent total thyroidectomy with central neck dissection. Postoperative period was uneventful. Histopathological report revealed features suggestive of mixed tumor of Papillary thyroid Carcinoma and Anaplastic Carcinoma thyroid TNM Staging T3 N0 M0, Stage IVA. After the final reports patient was sent for adjuvant therapy three weeks later where he received megavoltage external beam radiation and he was followed up till 12th week. He was assessed radiologically which showed no signs of physical progression of the disease. However, he was lost to follow up after that visit.DiscussionLong-standing benign conditions of thyroid can transform into malignant forms in the undefined duration of time.ConclusionRegular follow up and early management of multinodular goiter at the right time can save a patient from undue stress and complication like the conversion into malignancy.

Challenges in Diagnosing Rare Retroperitoneal Tumors: A Case Report of Extrarenal Giant Angiomyolipoma.

Project description:IntroductionExtrarenal angiomyolipomas are extremely rare tumors, with only isolated reports in the literature. Their diagnosis is challenging as these lesions are often misdiagnosed as more common retroperitoneal tumors. This report presents a case of a retroperitoneal extrarenal angiomyolipoma, emphasizing its clinical, imaging, and histopathological features to facilitate accurate diagnosis and avoid errors.Case presentationA 58-year-old male with a history of benign prostatic hyperplasia presented with progressive abdominal distension and weight gain. Imaging studies revealed a giant retroperitoneal mass displacing the left kidney and abdominal aorta. A core needle biopsy initially suggested a well-differentiated liposarcoma T4N0M0, leading to neoadjuvant chemotherapy with doxorubicin and ifosfamide. Surgical resection of a 20 × 30 × 25-cm mass was performed. Histopathological and immunohistochemical analysis of the specimen confirmed the diagnosis of retroperitoneal extrarenal angiomyolipoma. Chemotherapy was discontinued, and the patient remains stable under follow-up.Discussion and conclusionRetroperitoneal extrarenal angiomyolipomas are rare, presenting significant diagnostic challenges due to their resemblance to other retroperitoneal neoplasms such as liposarcomas. This case highlights the importance of comprehensive imaging, histopathological examination, and immunohistochemical studies for accurate diagnosis. Increased awareness of this entity can help clinicians avoid unnecessary treatments and ensure appropriate management of similar cases.

Very rare metastatic phenomena of biliary tract cancer to the cerebellum: A case report and review of the literature.

Project description:Introduction and importanceThe occurrence of brain metastasis due to cholangiocarcinoma is an exceedingly uncommon phenomenon, documented in only a few numbers of published cases. Recent studies indicated an incidence rate of just 0.15 % for brain metastases in connection with this condition, which was also linked to a reduced survival rate.Case presentationA 58-year-old woman with a history of hepatobiliary cholangiocarcinoma presented with a recent onset of unsteady walking, dizziness, vomiting, and worsening occipital headaches. Her medical history included successful chemotherapy treatment for her cholangiocarcinoma. Neurological examination revealed right-sided cerebellar deficits, and imaging indicated a sizable lesion in the right cerebellar hemisphere with surrounding vasogenic edema. A PET scan revealed a liver lesion but no other significant abnormalities. The recommended approach was surgical excision of the cerebellar lesion to relieve symptoms, halt deterioration, and obtain a tissue sample for analysis. After comprehensive discussions with the patient and her family, they opted for the surgical procedure.Clinical discussionThe major contributors to brain metastases include lung cancers, breast cancers, testicular cancers, melanomas, and renal tumors. In contrast, brain metastases originating from gastrointestinal cancers are less frequent, accounting for fewer than 4 % of cases, with notable impact on 1 % of colorectal cancers, 0.62 % of gastric cancers, and 0.33 % of pancreatic cancers. However, brain metastases are extremely rare.ConclusionThis study underscores the significance of anticipating and identifying brain metastases in biliary tract cancers, even in the face of their low incidence and the limited amount of available literature on the subject.

Apocrine Hidrocystoma: A Rare Case Report.

Project description:Hidrocystoma is a cystic tumor originating from a dilated sweat gland. The tumor is typically small and located predominantly on the face, particularly in the eyelid margins. They are two classical types of hidrocystomas i.e., Apocrine and Eccrine. Apocrine hidrocystomas are cystic lesions that arise from the apocrine secretory coil and Eccrine hidrocystomas represent retention cysts of the eccrine duct. Multiple apocrine hidrocystomas confined to the eyelids have been recognized as a feature of a new variant of ectodermal dysplasia. Here we present a rare case of Apocrine hidrocystoma of supraorbital area for which excisional biopsy of the lesion was performed. Histopathological evaluation proved the lesion was a unilocular cyst lined by a double-layered epithelium of the apocrine type. The purpose of this paper is to report the unique occurrence of a Supra orbital apocrine hidrocystoma which was clinically as a sebaceous cyst and to discuss its differential diagnosis with other more common supraorbital and face lesions.
