What do nurses experience in communication when assisting in robotic surgery: an integrative literature review.

ABSTRACT:

Background

Communication in surgery is integral to the fundamentals of perioperative nursing practice and patient safety. Research exploring team communication in robotic-assisted surgery (RAS) is evident in the literature but little attention has been focused on how the experiences of operating room nurses' communication affect safety, practice and patient care outcomes.

Objective

To synthesise current evidence regarding communication during robotic-assisted surgery as experienced by registered nurses.

Design

An integrative literature review informed by Whittemore and Knafl's (2005) methodology was used to conduct a rigorous analysis and synthesis of evidence.

Methods

A comprehensive database search was conducted using PRISMA guidelines. CINAHL, Pubmed, PsychINFO and British Nursing Web of Science databases were searched using a Boolean strategy.

Results

Twenty-five relevant papers were included in this literature review. Thematic analysis revealed two main themes with four related subthemes. The two main themes are: 'Adaptive operating room nursing in RAS' and 'RAS alters team dynamics'. The four subthemes are: 'Navigating disruptions in RAS', 'RAS heightens interdependence on team working', 'Augmented communicative workflow in RAS', and 'Professional empowerment to speak up'.

Conclusions

This integrative review identifies how current research largely focuses on communication in the wider OR team. However, current evidence lacks the input of nurses. Therefore, further evidence is needed to explore nurses' experiences to highlight their perspectives.

Clinical relevance

Robotics significantly benefit patients, and this review identifies different challenges that robotic-assisted surgery nurses encounter. A better understanding of the communication from the perspective of nurses is needed to guide future research, practice education, policy development and leadership/management.

Project description:AimTo synthesize evidence on factors influencing negative outcomes following patient safety incidents.BackgroundPatient safety incidents affect not only patients and families but also healthcare workers (second victims) and institutions (third victims). Nurses are at risk due to stressful environments and direct patient care, leading to defensive practices, job turnover, and errors. Identifying these factors and developing support strategies are crucial.MethodsThis integrative review followed Whittemore and Knafl's method, guided by the middle-range theory of nurses' psychological trauma. Seven databases (PubMed, CINAHL, PsycINFO, Cochrane Library, KISS, KMbase, and RISS) were searched for English and Korean articles from 2014 to 2023. Study quality was evaluated using Joanna Briggs Institute tools, and reporting followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses checklist.ResultsA review of 24 cross-sectional studies from the United States and China revealed factors such as genetic/biological (e.g., gender and age), environmental (e.g., work experience and organizational support), and psychological (e.g., distress and coping). Nonpunitive cultures reduced negative emotions, while high harm and preventable incidents increased negative impacts, mitigated by spirituality. These findings underscore the need for targeted interventions and supportive policies.ConclusionMultidimensional support strategies are necessary to improve nurses' working environments and patient care quality. Further research should explore additional mitigating factors and third victim studies.Implications for nursing policyThis review highlights the need for global mental health strategies and supportive organizational cultures for nurses. Addressing punitive environments and systemic factors through policy changes can improve nurse well-being and patient care quality. Creating safer environments not only benefits nurses but also enhances the quality and safety of patient care, ultimately driving the provision of better healthcare services.

Project description:PurposeRobotic surgery has been progressively implemented for colorectal procedures but is still limited for multiquadrant abdominal resections. The present study aims to describe our experience in robotic multiquadrant colorectal surgeries and provide a systematic review and meta-analysis of the literature investigating the outcomes of robotic total proctocolectomy (TPC), total colectomy (TC), subtotal colectomy (STC), or completion proctectomy (CP) compared to laparoscopy.MethodsAt our institution 16 consecutive patients underwent a 2- or 3-stage totally robotic total proctocolectomy (TPC) with ileal pouch-anal anastomosis. A systematic review of the literature was performed to select studies on robotic and laparoscopic multiquadrant colorectal procedures. Meta-analyses were used to compare the two approaches.ResultsIn our case series, 14/16 patients underwent a 2-stage robotic TPC for ulcerative colitis with a mean operative time of 271.42 (SD:37.95) minutes. No conversion occurred. Two patients developed postoperative complications. The mean hospital stay was 8.28 (SD:1.47) days with no readmissions. Mortality was nil. All patients underwent loop-ileostomy closure, and functional outcomes were satisfactory. The literature appraisal was based on 23 retrospective studies, including 736 robotic and 9,904 laparoscopic multiquadrant surgeries. In the robotic group, 36 patients underwent STC, 371 TC, 166 TPC, and 163 CP. Pooled data analysis showed that robotic TC and STC had a lower conversion rate (OR = 0.17;95% CI, 0.04-0.82; p = 0.03) than laparoscopic TC and STC. The robotic approach was associated with longer operative time for TC and STC (MD = 104.64;95% CI, 18.42-190.87; p = 0.02) and TPC and CP (MD = 38.8;95% CI, 18.7-59.06; p = 0.0002), with no differences for postoperative complications and hospital stay. Reports on urological outcomes, sexual dysfunction, and quality of life were missing.ConclusionsOur experience and the literature suggest that robotic multiquadrant colorectal surgery is safe and effective, with low morbidity and mortality rates. Project description:BackgroundRobotic bariatric surgery serves as an alternative to laparoscopy. The technology provides the surgeon with an accurate three-dimensional view, allowing complex maneuvers while maintaining full control of the operating room.HypothesisWe report our experience with this innovative surgery compared with laparoscopy during Roux-en-Y gastric bypass to demonstrate its safety and feasibility. The aim of this study is to evaluate potential differences between the robotic and laparoscopic techniques.Materials and methodsOur study retrospectively identified 153 consecutive obese patients who underwent either laparoscopic or robotic gastric bypass (RGB) procedures over a 2-year period at the Department of Medical and Surgical Sciences, University of Foggia. Data on demographics, operative time, conversion rate, length of hospital stay, and mortality were collected and compared between two groups of patients: 82 patients who underwent laparoscopic procedures and 71 who underwent robotic procedures.ResultsWe analyzed 153 patients who underwent gastric bypass with a mean age of 42.58 years, of whom 74 were female; 71 were treated with a robotic approach and 82 with a laparoscopic approach. The mean operative time was 224.75 ± 10.4 min for RGB (including docking time) and 101.22 min for laparoscopic gastric bypass (LGB) (p < 0.05), which is statistically significant. The median length of stay was 4.1 days for the RGB group and 3.9 days for the LGB group (p = 0.89). There is only one conversion to laparoscopy in the RGB group. We observed only one case of postoperative complications, specifically one episode of endoluminal bleeding in the laparoscopic group, which was successfully managed with medical treatment. No mortality was observed in either group.ConclusionThe statistical analysis shows to support the robotic approach that had a lower incidence of complications but a longer operative duration. Based on our experience, the laparoscopic approach remains a technique with more haptic feedback than the robotic approach, making surgeons feel more confident.This study has been registered on ClinicalTrial.gov Protocol Registration and Results System with this ID: NCT05746936 for the Organization UFoggia (https://clinicaltrials.gov/ct2/show/NCT05746936).
