Methods for Assessing Health Outcomes Associated with Food Insecurity in the United States College Student Population: A Narrative Review.

ABSTRACT: In the United States, college students experience disproportionate food insecurity (FI) rates compared to the national prevalence. The experience of acute and chronic FI has been associated with negative physical and mental health outcomes in this population. This narrative review aims to summarize the current methodologies for assessing health outcomes associated with the experience of FI in college students in the United States. To date, assessing the health outcomes of FI has predominately consisted of subjective assessments, such as self-reported measures of dietary intake, perceived health status, stress, depression, anxiety, and sleep behaviors. This review, along with the emergence of FI as an international public health concern, establishes the need for novel, innovative, and objective biomarkers to evaluate the short- and long-term impacts of FI on physical and mental health outcomes in college students. The inclusion of objective biomarkers will further elucidate the relationship between FI and a multitude of health outcomes to better inform strategies for reducing the pervasiveness of FI in the United States college student population.

Project description:BackgroundFood insecurity, lack of consistent access to the food needed for an active, healthy life, harms population health. Although substantial biomedical evidence examines the connections between food insecurity and health, fewer studies examine why food insecurity occurs.MethodsWe propose a conceptual understanding of food insecurity risk based on institutions that distribute income-the factor payment system (income distribution stemming from paid labor and asset ownership), transfers within households, and the government tax-and-transfer system. A key feature of our understanding is 'roles' individuals inhabit in relation to the factor payment system: child, older adult, disabled working-age adult, student, unemployed individual, caregiver, or paid laborer. A second feature is that the roles of others in an individual's household also affect an individual's food insecurity risk. We tested hypotheses implied by this understanding, particularly hypotheses relating to role, household composition, and income support programs, using nationally-representative, longitudinal U.S. Current Population Survey data (2016-2019).ResultsThere were 16,884 participants (year 1 food insecurity prevalence: 10.0%). Inhabiting roles of child (Relative Risk [RR] 1.79, 95% Confidence Interval [95%CI] 1.67 to 1.93), disabled working age-adult (RR 3.74, 95%CI 3.25 to 4.31), or unemployed individual (RR 3.29, 95%CI 2.51 to 4.33) were associated with a greater risk of food insecurity than being a paid laborer. Most food insecure households, 74.8%, had members inhabiting roles of child or disabled working age-adult, and/or contained individuals who experienced job loss. Similar associations held when examining those transitioning from food insecurity to food security in year 2.ConclusionsThe proposed understanding accords with the pattern of food insecurity risk observed in the U.S. An implication is that transfer income programs for individuals inhabiting roles, such as childhood and disability, that limit factor payment system participation may reduce food insecurity risk for both those individuals and those in their household.

Project description:BackgroundCancer patients face a costly trade-off between medical care and basic necessities including food. This study aims to explore whether catastrophic health expenditures lead to food insecurity among older cancer survivors in the US.MethodsLongitudinal study of individuals aged 50 or older who were diagnosed with cancer during 2000-2020 and their follow-up measurements selected from the Health and Retirement Study. Data consists of 2505 cancer survivors and 11,614 person-year observations for an average of 4.6 observations per participant. Catastrophic health expenditures were defined as out-of-pocket costs exceeding 5%, 10%, or 15% of household income. Participants were classified as food insecure if they experienced insufficient access to food due to financial limitations. This study utilized fixed effects ordered logistic regression to implement a within-subject research design.ResultsOf the 2505 cancer survivors, 77 (3.1%) were moderately food insecure and 73 (2.9%) were severely food insecure. In ordered logistic regression, all three measures of catastrophic health expenses were associated with a higher odds of food insecurity. These associations were more pronounced for males, ethnic minorities, survivors without college education, those in fair or poor health, retirees, and survivors with below-median income.ConclusionsThe prevalence of food insecurity among older cancer survivors was relatively low, with 6% of the sample experiencing food insecurity. Multivariate regression analyses revealed that a major predictor of food insecurity among older cancer survivors is catastrophic health costs. Given the health benefits of secure food access, older cancer survivors should consult care providers about their financial capacity to afford recommended cancer treatments while maintaining healthy diets. Policymakers should also consider interventions to reduce out-of-pocket financial burden on older cancer survivors, as improved financial security may enhance treatment outcomes and lower cancer-related mortality.
