Postless Distraction Technique With No Additional Equipment in Hip Arthroscopy.

Project description:Hip arthroscopy has become a predominant treatment for hip disorders such as femoroacetabular impingement syndrome and labral injury, and appropriate distraction for the hip joint is necessary for successful surgery. The traditional distraction method uses a perineal post but may cause complications such as perineum injury and nerve damage. For this reason, some surgeons have proposed postless distraction techniques, but they usually require additional equipment purchase and cost, which is not conducive to application. Therefore, we developed a post-free distraction technique without additional equipment. This method uses only surgical draw sheets, safety straps, a hip fracture table, and a hip distractor that are routinely provided in the operating room, and postless hip distraction can be achieved by using the Trendelenburg position, which is reliable, simple, and reproducible to be used in hip arthroscopy.

Patient Positioning for Postless Hip Arthroscopy.

Project description:Hip arthroscopy has been on the rise since its inception. Initial descriptions of the procedure required skeletal traction of the operative extremity with countertraction provided by a perineal post. Perineal posts are associated with complications such as perineal nerve palsy, genital laceration, and hematoma formation. Postless traction has been developed to avoid complications related to use of perineal posts. A patient is positioned supine on a postless traction table (Guardian; Stryker, Greenwood Village, CO) with a disposable traction pad. The anterior superior iliac spine is positioned at the distal aspect of the semicircle cutout in the traction pad. The patient is placed in approximately 3° of Trendelenburg positioning. Both lower extremities are secured into traction boots. The operative extremity is prepared and draped in the standard sterile fashion. Traction is applied to the operative extremity with countertraction being applied manually to the pelvis during hip dislocation. Many pathologies around the hip including femoral acetabular impingement can be effectively managed with hip arthroscopy. Postless hip arthroscopy is an effective method of obtaining sufficient traction for hip dislocation. Significant complications related to the perineal post can be avoided with postless traction. We expect postless traction use to increase in hip arthroscopy.

The Physics of Postless Hip Arthroscopy.

Project description:Hip arthroscopy is commonly performed to treat femoroacetabular impingement syndrome. A post-assisted arthroscopic hip preservation surgery approach provides joint distraction for central-compartment access. Owing to the location, compression of the post in the perineum may cause injuries to the pudendal nerve, perineal soft tissue, or genitourinary system. A postless technique significantly reduces the risk of these complications. Postless arthroscopy uses friction between the patient's torso and the table surface to permit distraction without the post. An air arthrogram, general anesthesia with muscle paralysis, and variable degrees of Trendelenburg positioning reduce the force needed for joint distraction. Early postless literature suggested Trendelenburg angles of approximately 15°, which may be disorienting to surgeons and compromise the precision and accuracy of the surgical procedure. With the described technique, hip arthroscopy can be effectively performed with a Trendelenburg angle of less than 5° in nearly every case. Understanding the physics of postless hip arthroscopy using free-body diagrams of inclined planes with friction permits surgeons to understand the required Trendelenburg angle of the bed, the force of traction for any patient given his or her body habitus, and the coefficient of static friction of the table surface to achieve a minimum amount of joint distraction.

The Yoga Mat Technique in Postless Hip Arthroscopy.

Project description:Hip arthroscopy for the treatment of femoroacetabular impingement syndrome with anti-sliding techniques and without the use of a perineal post to achieve hip distraction has increased greatly in the past 5 to 10 years. To access the hip joint, distraction is mandatory to treat intra-articular disorders such as labral tears, acetabular cartilage defects, loose bodies, ligamentum teres tears, and avascular necrosis of the hip. In hip distraction, counter-distraction is needed, and this is achieved with a bulky and cushioned perineal post. Most of the described techniques in hip arthroscopy worldwide use a perineal post, but iatrogenic pudendal nerve, genital lacerations, hematomas, and groin complications have been reported to occur. In Latin American countries, disposable hip pad devices are expensive, and not all the arthroscopic companies provide them. Our yoga mat technique provides enough countertraction to achieve adequate hip distraction. Labral repair, labral reconstruction, and decompression of femoroacetabular impingement syndrome have been achieved properly, reliably, and reproducibly, and no Trendelenburg position is needed. Postless hip arthroscopy is made simple, and positioning the patient is not difficult. Hip surgeons can adapt this technique to a fracture table, a hip distractor, and a standard operating room table.

Postless Hip Arthroscopy for Labrum Reconstruction and Labrum Augmentation.

Project description:Hip arthroscopy has been shown to be an effective surgical treatment with excellent outcomes and a low percentage of complications; however, there are published data regarding iatrogenic complications with hip distraction. Hip distraction is mandatory to gain access to the central compartment and to perform a reliable labral repair or labral reconstruction. Postless hip arthroscopy is very popular nowadays, and several techniques have been published. To our knowledge, no published techniques have been described that access the hip with a postless technique in reconstructing a labrum in a complete, segmental, or augmented manner. We present a modified technique for hip labral augmentation and reconstruction without a post. The described technique was obtained from the original technique for hip arthroscopic labral reconstruction by Dr. Brian J. White in the "front-to-back" technique.

Hip Spica Stroller: A Technical Note.

Project description:BackgroundCare of the child after hip-spica application is the most challenging issue faced by parents. Reduced mobility and recumbency can cause respiratory problems, skin irritation and discomfort for the child. Parents need to lift the child in a spica to make him upright and to mobilize. To improve the comfort of the family in post spica care, we have designed a 'Hip Spica Stroller' which is low profile, comfortable, easy to construct and relatively inexpensive. This manuscript discusses our early experience with this hip-spica stroller use.MethodsA sturdy, relatively lightweight and portable stroller was designed in collaboration with local mechanical engineering team. The stroller allowed safe upright placement of the child with spica and their easy mobility. From the second post-operative day, children were mobilised in the stroller. We looked for development of any complications related to the stroller such as spica breakage, skin irritation or excoriation development. Parents were also inquired about their feedback and satisfaction with the device upon spica removal.ResultsWe used the spica stroller in nine patients after Developmental Dysplasia Hip (DDH) treatment and in eight patients post shaft femur fracture treatment. None of the patients experienced any stroller-related complications. All the patients maintained the joint reduction and the fractured bone fragment alignment. High satisfaction rates were reported by parents.ConclusionHip Spica Stroller is an innovative and easy-to-make device which would enhance the post spica care. Achieving Post-Free Distraction in Hip Arthroscopy With a Pink Pad Patient Positioning Device Using Standard Hip Distraction Tables.

Project description:Iatrogenic pudendal nerve and groin complications have been reported to occur at rates up to 4.3%. While many of these complications are transient, the hip preservation community is in search of a safe postless distraction technique. Currently available postless distraction tables are expensive, require the surgeon to adopt a different technique, and have a high disposable fee. We present our technique for postless distraction using a standard table and a pink pad positioning device. Using our technique, we have demonstrated safe and cost-effective post-free distraction.

The Pink Pad: A Method of Post-Free Distraction During Hip Arthroscopy.

Project description:Distraction is essential during hip arthroscopy to allow for adequate working space during central and peripheral compartment procedures. Pudendal posts are frequently used with traction boots to achieve distraction, but use of these posts is believed to be associated with the iatrogenic pudendal area and perineal nerve injuries seen with hip arthroscopy. Current post-free distraction tables are costly and cumbersome. The pink pad positioning device allows for post-free distraction through the creation of friction between the patient, the pad, and the bed. This device can be used with a standard hip arthroscopy table, is easy to learn, allows for adequate distraction, and allows for greater access to the femoral neck during osteochondroplasty, as there is less restriction of the patient's range of motion without a post present. The purpose of this Technical Note is to describe a method of post-free distraction using the pink pad positioning device.
