The association between hepatitis B virus and semen quality: a systematic review and meta-analysis.

ABSTRACT:

Background

Some studies have suggested that hepatitis B virus (HBV) infection had a negative association with semen quality, but the conclusions have been inconsistent. The purpose of our study was to systematically assess the association between HBV infection and semen parameters.

Methods

We searched electronic databases for studies published from January 1980 to August 2023. Eleven studies were included in the analysis. Primary outcomes were semen volume, sperm concentration, sperm morphology, sperm motility and sperm progressive motility. We also conducted a subgroup analysis between China and other countries.

Result

Compared with the semen quality of HBV-negative men, HBV infection had a negative association with semen volume (MD: -0.20 mL, 95%CI: -0.32 to - 0.09, P = 0.0004), sperm concentration (MD: -4.46 × 10⁶/mL, 95%CI: -7.09 to - 1.84, P = 0.0009), sperm morphology (MD: -2.49%, 95%CI: -4.35 to - 0.64, P = 0.008), sperm motility (MD: -6.85%, 95%CI: -11.53 to - 2.18, P = 0.004), and sperm progressive motility (MD: -6.63%, 95%CI: -10.24 to - 3.02, P = 0.0003). However, HBV infection had no significant association with total sperm count (MD: -31.50 × 10⁶, 95%CI: -74.11 to 11.10, P = 0.15). The association between HBV and semen quality were inconsistent between the subgroups.

Conclusion

HBV infection had a negative association with sperm concentration, motility, morphology, and semen volume. However, The association between HBV and total sperm count remain unclear. This metaanalysis suggests that we should pay attention to the adverse effect of HBV on sperm quality, and several studies have reported the relevant mechanisms. But due to the significant heterogeneity among studies on some semen parameters, further large and well-designed researches are needed before introducing clinical management recommendations.

Project description:Background: Pyrethroids are natural organic compounds extracted from flowers of pyrethrums and commonly used as domestic and commercial insecticides. Although it is effective in insect and parasitic control, its associated toxicity, including spermotoxicity, remains a challenge globally. Currently, the available reports on the effect of pyrethroids on semen quality are conflicting, hence an evaluation of its detrimental effect is pertinent. This study conducts a detailed systematic review and meta-analysis of the effects of pyrethroids on sperm quality. Materials and methods: The present study was performed according to Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Using a pre-defined strategic protocol, an internet search was done using combined text words. The criteria for eligibility were selected based on Population, Exposure, Comparator, Outcome, and Study Designs (PECO) framework, and relevant data were collected. Appraisal was done using The Office of Health Assessment and Translation (OHAT) tool for the evaluation of the Risk of Bias and the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) Working Group guidelines for the certainty of evidence. A quantitative meta-analysis was conducted with the Review Manager (RevMan). Results: Only 12 out of the 4, 050 studies screened were eligible for inclusion in this study. The eligible studies were from China (4), Japan (3), Poland (3), and United States (2). All the eligible studies were cross-sectional. A total of 2, 050 male subjects were included in the meta-analysis. Pyrethroid exposure significantly reduced sperm motility. Region-stratified subgroup analyses revealed that pyrethroid significantly reduced sperm motility among men in Poland and United States, and decreased sperm count among men in Japan. Pyrethroid exposure also reduced sperm concentration among men in Poland but increased sperm concentration among men in the United States. Conclusion: Although the study revealed inconsistent evidence on the detrimental effect of pyrethroids on semen quality, the findings showed that pyrethroids have deleterious potentials on sperm motility, count, and concentration. Studies focusing on the assessment of semen quality in pyrethroid-exposed men, especially at specific varying levels of exposure, and employing prospective cohort studies or controlled cross-sectional designs are recommended.

Project description:IntroductionIt is hypothesized that air pollutants could be associated with semen parameters. This systematic review and meta-analysis of observational studies was conducted to reach a firm conclusion regarding the possible association between outdoor air pollution and semen parameters among the adult population.MethodsPubMed, Scopus, and ISI Web of Science were systematically searched using the text keywords and MeSH terms, including "air pollution" and "semen parameters,". The population, Intervention, Comparison, and Outcome (PICO) framework was used as follows: P (Adult men), I (Individuals with the highest exposure to the air pollutants), C (Individuals with the lowest exposure to the air pollutants), O [Semen parameters, including semen volume, total sperm count, sperm concentration, total motility, progressive motility, normal morphology rate, and DNA fragmentation index (DFI)]. The overall effect was presented as a weighted mean difference (WMD) and 95% confidence interval (CI) analyzed via a fixed (inverse-variance) or random (DerSimonian-Laird) weighted model. Low, moderate, and high heterogeneity were defined as I2 index < 40, 40-75, and > 75%, respectively.ResultsSeventeen studies covering 24,065 participants were enrolled in this systematic review and meta-analysis. Higher exposure to outdoor air pollution was associated with significant decreases in semen volume (WMD: -0.13 mL; 95% CI, -0.21 to -0.05; P = 0.001; I2 = 32.1%), sperm concentration (WMD: -12.41 × 106/mL; 95% CI, -23.29 to -1.53; P = 0.03; I2 = 98.7%), total motility (WMD: -5.96%; 95% CI, -10.76 to -1.16; P = 0.01; I2 = 96.2%), progressive motility (WMD: -4.89%; 95% CI, -9.23 to -0.55; P = 0.03; I2 = 98.0%), normal morphology rate (WMD: -2.64%; 95% CI, -4.36 to -0.92; P = 0.003; I2 = 94.6%), and significant increases in DNA fragmentation index (WMD: 5.41%; 95% CI, 3.24 to 7.59; P < 0.001; I2 = 70.4%).ConclusionBased on the results, it can be stated that air pollution can impair sperm parameters. Further prospective cohort studies are needed to illuminate this issue and clarify the underlying mechanisms.
