Project description:In skeletally immature patients with recurrent patella dislocation that needs operation, how to increase the medial stability of the patella effectively without disturbance of the epiphysial plate is of concern. Through evaluation of multiple techniques that designed to avoid injury to the epiphysial plate, we found that medial patellofemoral ligament augmentation with high-strength nonabsorbable sutures is feasible and effective. Thus, we would like to describe this technique, whose critical point is the correct location and creation of the 2 femoral tunnels. We consider the introduction of the current technique will provide reasonable choices when operation is needed in this special group of patients.

Project description:Patellar instability is a common problem in the active pediatric population. When nonoperative treatment of the instability fails, growth-respecting surgical stabilization techniques are required. As the incidence of medial patellofemoral ligament (MPFL) reconstruction has increased, techniques have improved to avoid physeal injury to the distal femur. These techniques are technically demanding because of the small size of the distal femoral epiphysis in children, as well as the relatively large socket size (7-8 mm in diameter, >20 mm in length) required for sound fixation with a tenodesis screw as originally described. The size of the femoral tunnel for interference fixation puts the surrounding structures at risk of damage. We present a modification of the epiphyseal socket technique for anatomic growth-sparing MPFL reconstruction using a small soft anchor for femoral graft fixation. This has the proposed advantages of diminishing volumetric bony removal from the epiphysis; increasing the margin of safety with respect to notch, trochlear, and/or physeal damage; and reducing the risk of thermal damage to the physis during socket reaming. This technique is technically simple and can be easily learned by surgeons familiar with adult MPFL reconstruction techniques.

Project description:Conventional adult anterior cruciate ligament reconstruction techniques are controversial in skeletally immature patients due to the risk of iatrogenic physeal damage and potential growth disturbance. The physeal-sparing, combined intra- and extra-articular anterior cruciate ligament reconstruction using an autogenous iliotibial band was developed to mitigate this risk in prepubescent, skeletally immature patients, with excellent functional outcomes and a low revision rate. This article describes the surgical details of this reproducible reconstruction technique.

Project description:A technique for anterior cruciate ligament (ACL) reconstruction in patients with open physis is presented. The patient is positioned supine with the knee flexed 90°. After intraarticular injuries are addressed, an autologous hamstring graft is harvested and prepared using a suspension device attached in its expansion device. All-epiphyseal femoral and tibial tunnels of the same diameter of the graft are created; both of them are drilled in an outside-in direction, sparing the physis under radioscopic control. A second divergent tibial tunnel of the same diameter of the graft, distal to the physis, is created in an outside-in, mediolateral, and craniocaudal direction, leaving a 1-cm bone bridge between the 2 tibial tunnels. The graft is passed through the all-epiphyseal tunnels, from femoral to tibial, and pulled until the suspension device leans on the lateral femoral cortex. The graft is passed through the second divergent tibial tunnel and fixed in it with an interference screw to move the pressure away from the physis.

Project description:Aberrant anatomy, such as patella alta, can be a risk factor for recurrent patellar instability or inferior clinical outcomes after medial patellofemoral ligament reconstruction. In patients with significant patella alta (Caton Deschamps Index >1.2), tibial tubercle distalization may be considered to improve outcomes. However, despite patellar instability commonly affecting pediatric patients, a distalization osteotomy is not feasible in this patient population due to the presence of open physes. Our article presents a technique describing an alternative soft-tissue distalization approach whereby patellar height is decreased by shortening the patellar tendon.

Project description:The subsequent dislocation of a contralateral patellofemoral joint sometimes occurs after a first-time lateral patellar dislocation (LPD). However, the anatomic risk factors for subsequent contralateral LPD remain elusive. This study included 17 patients with contralateral LPD and 34 unilateral patellar dislocators. The anatomic parameters of the contralateral patellofemoral joints were measured using CT images and radiographs that were obtained at the time of the first dislocation. The Wilcoxon rank-sum test was performed, and a binary regression model was established to identify the risk factors. The receiver operating characteristic curves and the area under the curve (AUC) were analyzed. The tibial tubercle-Roman arch (TT-RA) distance was significantly different between patients with and without contralateral LPD (24.1 vs. 19.5 mm, p < 0.001). The hip-knee-ankle (HKA) angle, patellar tilt, congruence angle, and patellar displacement were greater in the study group than in the control group (p < 0.05). The TT-RA distance revealed an OR of 1.35 (95% CI (1.26-1.44]), p < 0.001) and an AUC of 0.727 for predicting contralateral LPD. The HKA angle revealed an OR of 1.74 (95% CI (1.51-2.00), p < 0.001) and an AUC of 0.797. The Patellar tilt, congruence angle, and patellar displacement had AUC values of 0.703, 0.725, and 0.817 for predicting contralateral LPD, respectively. In conclusion, the contralateral patellofemoral anatomic parameters were significantly different between patients with and without subsequent contralateral LPD. Increased TT-RA distance and excessive valgus deformity were risk factors and could serve as predictors for contralateral LPD. At first-time dislocation, the abnormal position of the patella relative to the trochlea may also be an important cause of subsequent LPD.

Project description:Patellar instability remains a ubiquitous and troublesome problem in orthopaedics and represents a challenge in the pediatric population. Reconstruction of the medial patellofemoral ligament (MPFL) has become a mainstay of patellar instability management in recent years. As with any procedure at or around the physes, there is concern among surgeons regarding safe placement of hardware and drilled tunnels. The authors describe a technique for anatomic MPFL reconstruction with the aid of fluoroscopic guidance to maintain a "safe zone" without violating the distal femoral physis. This technique allows for reliable MPFL reconstruction in the skeletally immature population with concomitant lateral lengthening, while requiring only minor deviations from the procedure in a skeletally mature patient.

Project description:Patella alta is a major contributor to recurrent patellar instability, which is commonly seen in young athletes. Distalizing tibial tubercle osteotomy has been used for the correction of patella alta and patellar instability, but this procedure is contraindicated in the skeletally immature patient population, as it could lead to growth arrest of the proximal tibial physes. We propose a patellar tendon imbrication technique as a soft-tissue alternative to tibial tubercle osteotomy. When used with concomitant medial patellofemoral ligament reconstruction, we suggest this procedure may prove beneficial for the treatment of patellar instability in skeletally immature patients.

Project description:The number of anterior cruciate ligament (ACL) reconstructions in skeletally immature patients has been gradually increasing in recent years owing to the poor outcomes of conservative treatment. However, ACL reconstruction in children is a challenging procedure and may lead to severe complications, such as growth disturbance. Although double-bundle (DB) ACL reconstructions are preferable because of their superior clinical outcomes, there are few reports of DB ACL reconstruction in children with open epiphyses. In this Technical Note, an all-epiphyseal sparing DB ACL reconstruction technique, which is considered to help avoid growth disturbance, is described. This procedure does not create a thick tunnel and may facilitate subsequent revision surgery.
