Gender Differences among Nurses in Managing Arterial Puncture-Related Pain: A Multicenter Cross-Sectional Study.

ABSTRACT: There is evidence that healthcare can be executed differentially depending on the gender of patients, researchers, and clinicians. The aim was to analyze the possible existence of nursing gender differences in pain management produced by arterial puncture for blood gas analysis. A cross-sectional, multicenter study designed was conducted in Castilla-la Mancha (Spain). Variables of interest were collected from nurses in the public health system of a European region through a questionnaire. Data were collected for four months; the primary outcome was the use of any intervention to reduce pain and the explanatory variable was the nurse's gender. Bivariate analysis was carried out to assess associations between gender and pain-reducing interventions and a multivariate model was created with those factors that were relevant using logistic regression. A significantly higher proportion of men reported using some form of intervention (45% vs. 30%) and had more specific training (45.9% vs. 32.4%). The adjusted probability of using pain-reducing interventions by men was 71% higher than women. Thus, we found gender differences in the management of pain caused by arterial punctures performed by nurses as the main healthcare providers.

Project description:Aims(1) To examine registered nurses' knowledge and confidence in recognizing and managing to patients with sepsis and (2) identify nurse and workplace factors that influence their knowledge on sepsis.DesignA multi-site, cross-sectional survey.MethodsAn online survey was developed and content validated. Data was collected from registered nurses working in the inpatient wards and emergency departments of three hospitals of a single healthcare cluster in Singapore during August 2021. Statistical analyses of closed-ended responses and content analysis of open-ended responses were undertaken.ResultsA total of 709 nurses completed the survey. Nurses possessed moderate levels of knowledge about sepsis (mean score = 10.56/15; SD = 2.01) and confidence in recognizing and responding to patients with sepsis (mean score = 18.46/25; SD = 2.79). However, only 369 (52.0%) could correctly define sepsis. Nurses' job grade, nursing education level and clinical work area were significant predictors of nurses' sepsis knowledge. Specifically, nurses with higher job grade, higher nursing education level or those working in acute care areas (i.e. emergency department, high dependency units or intensive care units) were more likely to obtain higher total sepsis knowledge scores. A weak positive correlation was observed between sepsis knowledge test scores and self-confidence (r = .184). Open comments revealed that participants desired for more sepsis education and training opportunities and the implementation of sepsis screening tool and sepsis care protocol.ConclusionA stronger foundation in sepsis education and training programs and the implementation of sepsis screening tools and care bundles are needed to enhance nurses' knowledge and confidence in recognizing and managing patients with sepsis.ImpactThe findings of this study are beneficial to administrators, educators and researchers in designing interventions to support nurses in their role in recognizing and responding to sepsis.

Project description:BackgroundIn 2017, the hashtags #MeToo and #MeTooMedicine went viral and raised awareness of sexual harassment (SH) and sexual violence affecting all ages, genders, races, sexualities, and careers. Most studies investigating SH have found that women report higher rates of SH experiences compared to men, with documented incidences reaching as high as 81%. Notably, 47% of these incidents go unreported. A study from the Association of Anesthesiologists found that female victims of sexual assault often do not speak up due to the myth that harassment is rare, which further silences women and stigmatizes this issue, leading them to be discouraged from participating in academic anesthesia. Due to the outdated data examining the incidence of SH in the field of anesthesiology, an anonymous 20 item survey was administered to physician attendings and trainees to assess the incidence of SH perceiving SH within the field.MethodsAn anonymous questionnaire-based cross-sectional study, adapted from a validated survey tool on SH and burnout, was administered to a sample of registered members of the American Society of Anesthesiologists (ASA) to evaluate physicians' perceptions related to SH and burnout in their workplace.ResultsThe email survey was sent to 30,765 registered ASA members; a total of 2,830 (9.2%) members responded to the 20-item survey for this quantitative analysis. 53.4% (n = 1,511) and 44.2% (n = 1,251) identified themselves as men and women, respectively. Among all the respondents, 57.6% perceived some form of SH (75.5% women and 44.2% men, p-value:<0.0001). Around 40.1% of respondents indicated that they did not react to or report SH events, with a significantly higher incidence among women when compared to men (46.3% vs. 35.5%; p-value:<0.0001). Respondents who perceived SH were more likely to agree with the 5-point Likert-scale questions related to experiencing burnout, facing declining job opportunities or leaving a job, and becoming more emotionally detached from others.ConclusionSH has been reported by both male and female anesthesiologists, with female physicians perceiving significantly higher rates of SH compared to their male counterparts. Most respondents who experienced SH in their workplaces expressed agreement with statements related to "burnout feelings" and/or "declining a job opportunity or leaving the workplace." This study contributes to the current literature that SH is prevalent within the field of anesthesiology. Furthermore, our study demonstrates that SH has a positive correlation to feelings of burnout. This study demonstrates the critical importance of instituting policies regarding reporting SH events. Additionally, implementing bystander training can empower individuals to report witnessed SH events. Lastly, safeguards should be implemented to protect those who report witnessing or experiencing SH events.

Project description:BackgroundMusculoskeletal pain is a major cause of work disability. Many patients with musculoskeletal pain seek care from health care providers other than their general practitioners, including a range of musculoskeletal practitioners. Therefore, these musculoskeletal practitioners may play a key role by engaging in sickness absence management and work disability prevention. This study aimed to determine the prevalence of musculoskeletal practitioners' practice behaviours, and their perceptions and beliefs about sickness absence management by using Scandinavian chiropractors as an example, as well as to examine the association between these characteristics and two different practice behaviours.MethodsAs part of a mixed-methods study, we surveyed members of the national chiropractic associations in Denmark, Norway, and Sweden in 2016. Descriptive statistics were used to describe prevalence. Multilevel logistic regression with backwards stepping was used to estimate odds ratios with 95% confidence intervals between each of the two practice behaviours and the characteristics.ResultsOut of the 802 respondents (response rate 56%), 372 were Danish, 349 Norwegian, and 81 Swedish. In Denmark and Norway, 38.7 and 37.8% always/often considered if sick leave was appropriate for their patient compared to 21.0% in Sweden (p = 0.007); and 86.5% of the Norwegian chiropractors always/often recommended to return-to-work versus 64.5 and 66.7% in Denmark and Sweden respectively (p < 0.001). In the final models, factors associated with the two practice behaviours were age, level of clinical experience, working as a teacher, the tendency to be updated on current legislations and policies using social services, contact with general practitioners, relevance of engagement in SAM, consideration of workplace factors, SAM as part of the clinical tool box, patient out-of-pocket fee, and recommending fast return-to-work.ConclusionsWhilst not always engaged in sickness absence management with regards to musculoskeletal pain, chiropractors favour a 'return-to-work' rather than a 'stay-at-home' approach. Several practice behaviours and perceptions and beliefs are associated with these outcomes; however, system or organisational barriers are linked to clinician non-engagement.
