The impact of Covid-19 in Chhattisgarh, India: A descriptive study.

ABSTRACT:

Background

India is among the most severely affected countries in the ongoing Covid-19 pandemic, second to the USA. Until July 2022, there were 43 million confirmed Covid-19 cases, with 0.53 million Covid-19 deaths in India. Chhattisgarh, the twelfth-highest affected state by Covid-19 disease, has had an enormous mortality toll, with 14,046 deaths by 15^th July 2022. This study is a descriptive analysis of the Covid-19 deaths across Chhattisgarh from September 2021 to February 2022 (the third pandemic wave).

Materials and methods

All Covid-19 deaths that occurred between September 2021 to February 2022 were line-listed, and family members of all who died in the hospital were interviewed.

Results

356 Covid-19 deaths were included in the present study. The top three districts which observed maximum deaths (63.48%) were Raipur (27.53%), Durg (15.45%), and Bilaspur (7.87%). Around 50% of the deceased were of age 65 years or above. The male deaths were twice as compared to the female deaths. The RT-PCR was the most used testing method, followed by RAT. Most of the admissions (58%) were in a government facility. Among the deceased, 60% had one or two associated comorbidities. Hypertension and Diabetes were the most common comorbidity identified. Almost 90% of patients were symptomatic. Breathlessness was the most common symptom. Half of the deceased were not vaccinated. The median expenditure on Covid-19 management was ₹ 50,000 (€ 565.13).

Conclusion

The deaths in the study were urban-centric. Higher Covid-19 mortality was observed in unvaccinated older men (≥65 years) with one or more comorbidity.

Project description:BackgroundThe COVID-19 pandemic has disrupted health-care systems, leading to concerns about its subsequent impact on non-COVID disease conditions. The diagnosis and management of cancer is time sensitive and is likely to be substantially affected by these disruptions. We aimed to assess the impact of the COVID-19 pandemic on cancer care in India.MethodsWe did an ambidirectional cohort study at 41 cancer centres across India that were members of the National Cancer Grid of India to compare provision of oncology services between March 1 and May 31, 2020, with the same time period in 2019. We collected data on new patient registrations, number of patients visiting outpatient clinics, hospital admissions, day care admissions for chemotherapy, minor and major surgeries, patients accessing radiotherapy, diagnostic tests done (pathology reports, CT scans, MRI scans), and palliative care referrals. We also obtained estimates from participating centres on cancer screening, research, and educational activities (teaching of postgraduate students and trainees). We calculated proportional reductions in the provision of oncology services in 2020, compared with 2019.FindingsBetween March 1 and May 31, 2020, the number of new patients registered decreased from 112 270 to 51 760 (54% reduction), patients who had follow-up visits decreased from 634 745 to 340 984 (46% reduction), hospital admissions decreased from 88 801 to 56 885 (36% reduction), outpatient chemotherapy decreased from 173634 to 109 107 (37% reduction), the number of major surgeries decreased from 17 120 to 8677 (49% reduction), minor surgeries from 18 004 to 8630 (52% reduction), patients accessing radiotherapy from 51 142 to 39 365 (23% reduction), pathological diagnostic tests from 398 373 to 246 616 (38% reduction), number of radiological diagnostic tests from 93 449 to 53 560 (43% reduction), and palliative care referrals from 19 474 to 13 890 (29% reduction). These reductions were even more marked between April and May, 2020. Cancer screening was stopped completely or was functioning at less than 25% of usual capacity at more than 70% of centres during these months. Reductions in the provision of oncology services were higher for centres in tier 1 cities (larger cities) than tier 2 and 3 cities (smaller cities).InterpretationThe COVID-19 pandemic has had considerable impact on the delivery of oncology services in India. Project description:BackgroundIn early 2020, during the COVID-19 pandemic, New Zealand implemented graduated, risk-informed national COVID-19 suppression measures aimed at disease elimination. We investigated their impacts on the epidemiology of the first wave of COVID-19 in the country and response performance measures.MethodsWe did a descriptive epidemiological study of all laboratory-confirmed and probable cases of COVID-19 and all patients tested for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) in New Zealand from Feb 2 to May 13, 2020, after which time community transmission ceased. We extracted data from the national notifiable diseases database and the national SARS-CoV-2 test results repository. Demographic features and disease outcomes, transmission patterns (source of infection, outbreaks, household transmission), time-to-event intervals, and testing coverage were described over five phases of the response, capturing different levels of non-pharmaceutical interventions. Risk factors for severe outcomes (hospitalisation or death) were examined with multivariable logistic regression and time-to-event intervals were analysed by fitting parametric distributions using maximum likelihood estimation.Findings1503 cases were detected over the study period, including 95 (6·3%) hospital admissions and 22 (1·5%) COVID-19 deaths. The estimated case infection rate per million people per day peaked at 8·5 (95% CI 7·6-9·4) during the 10-day period of rapid response escalation, declining to 3·2 (2·8-3·7) in the start of lockdown and progressively thereafter. 1034 (69%) cases were imported or import related, tending to be younger adults, of European ethnicity, and of higher socioeconomic status. 702 (47%) cases were linked to 34 outbreaks. Severe outcomes were associated with locally acquired infection (crude odds ratio [OR] 2·32 [95% CI 1·40-3·82] compared with imported), older age (adjusted OR ranging from 2·72 [1·40-5·30] for 50-64 year olds to 8·25 [2·59-26·31] for people aged ≥80 years compared with 20-34 year olds), aged residential care residency (adjusted OR 3·86 [1·59-9·35]), and Pacific peoples (adjusted OR 2·76 [1·14-6·68]) and Asian (2·15 [1·10-4·20]) ethnicities relative to European or other. Times from illness onset to notification and isolation progressively decreased and testing increased over the study period, with few disparities and increasing coverage of females, Māori, Pacific peoples, and lower socioeconomic groups.InterpretationNew Zealand's response resulted in low relative burden of disease, low levels of population disease disparities, and the initial achievement of COVID-19 elimination.FundingMinistry of Business Innovation and Employment Strategic Scientific Investment Fund, and Ministry of Health, New Zealand.
