Left ventricular chordae tendinae myxoma causing stroke: A rare finding.

Project description:A 52-year-old woman presented with dysarthria and right-sided weakness in her upper and lower extremities prompting thrombolytic therapy with mild resolution of symptoms. Further work-up revealed (the source) a left ventricular myxoma on the chordae tendinae of the posterior medial papillary muscle, confirmed with transesophageal echocardiography and pathology. Herein, we present a rare case of embolic stroke from a myxoma originating on the chordae tendinae. To the best of our knowledge, the literature on the location and presentation of this tumor as seen in our patient is sparse in contemporary findings.

Project description:Rheumatic mitral stenosis is a common rheumatic heart diseases in sub-Saharan Afrleading to increased morbidity and mortality. Percutaneous mitral balloon valvuloplasty is the mainstay of less invasive therapy in those with moderate to severe valve stenosis, being symptomatic despite optimal medical therapy. Though generally save with experienced hand and setup, post procedural complications include development of mitral regurgitation, tear and thrombosis. Mitral leaflet tear and development of infective endocarditis are an uncommon but might lead to increased morbidity, rehospitalization and mortality.

A declining pattern of malaria prevalence in Asendabo Health Center Jimma zone, Southwest Ethiopia.

Project description:ObjectiveTo assess the status of malaria prevalence in one of the malaria endemic areas of Ethiopia.ResultsA 10-year report of malaria cases were obtained from Asendabo Health Center, Jimma zone, Southwest Ethiopia. Following a retrospective study design, data of 68, 421 febrile patients diagnosed and treated in the health center were included in the study. The year with the highest prevalence rate (34.9%) was 2010, whereas the lowest was 2016 (0.62%). The number of diagnosed malaria cases from September to November were significantly higher (P = 0.023, n = 6336, 46.5%) than in other months. Plasmodium falciparum (52.1%, n = 7087) and Plasmodium vivax (44.2%, n = 6009) were the two principal plasmodium species accountable for malaria infections in the study area. The current study is a supportive evidence for the reduction of malaria prevalence in malaria endemic areas of Ethiopia.

Birth outcomes of singleton term breech deliveries in Jimma University Medical Center, Southwest Ethiopia.

Project description:ObjectiveBreech delivery is generally associated with higher perinatal morbidity and mortality than cephalic presentation. Hence describing the outcomes of singleton term breech deliveries in Jimma University Medical Center (JUMC), Southwest Ethiopia addresses in recommendation of improving perinatal outcomes and developing protocols in selecting eligible women.ResultsThe incidence of singleton term breech delivery was 5.3%. Majority, (52.8%) of them had undergone emergency cesarean delivery (C/D), and 38.9% had vaginal breech delivery. There were 14 (13.9%) intrapartum fetal deaths of whom 5.6% were recorded at JUMC. A quarter (25%) of the neonates required admission to the neonatal intensive care unit; 40.7% had perinatal asphyxia, and there were 3 early onset neonatal deaths making up a perinatal mortality rate of 157.4 per 1000 breech births. The incidence of breech delivery was relatively high. Vaginal breech delivery was lower. Significant proportions of adverse perinatal outcomes were recorded. Introduction of a protocol for managing breech deliveries to select eligible women for trial of breech delivery and strengthen training of junior health professionals regularly on how to conduct assisted vaginal breech delivery to improve perinatal outcome is recommended. Further studies to identify determinants of perinatal outcomes is recommended.
