Right trace wrong place: a normal capnography trace despite the tip of the tracheal tube existing outside the airway.

ABSTRACT: Head and neck trauma can result in difficult airway management. A 25-year-old male required emergency tracheal intubation on arrival to the emergency department following a motorbike accident. Despite the presence of a normal capnography a computed tomography scan demonstrated a tracheal opening, an extra-tracheal position of the distal end of the tracheal tube, and extensive subcutaneous emphysema. The tube was re-directed into the trachea and the tracheal injury was surgically repaired. This case highlights that the presence of a normal capnograph does not necessarily mean that the distal end of the tracheal tube resides within the airway.

The Wrong Place at the Wrong Time? Territorial Autonomy and Conflict During Regime Transitions.

Project description:This article evaluates how territorial autonomy affects ethnic mobilization and conflict during regime transitions. Previous research has highlighted its conflict-inducing role during prominent transition contexts. Alternatively, it has shown its pacifying role in the "average" case, without distinguishing transition periods from stable contexts. Addressing these gaps, we argue that the de-escalatory consequences of autonomy depend on critical stabilizing factors which are themselves "muted" during transitions. We test our expectations in a cross-national analysis, covering all regime transitions between 1946 and 2017. We also revisit the 1989 transition from Communism, focusing on the role of "inherited" autonomy in the post-communist successor states. This enables us to address concerns whereby autonomy is offered to ward off transitions or whereby transitions are themselves induced by mobilization. Our findings indicate that during transitions, territorial autonomy increases the likelihood of ethnic mobilization, government concessions in response, and violent escalation where these are not forthcoming.

Digital tracheal intubation and finger palpation to confirm endotracheal tube tip position in neonates: A systematic review and meta-analysis.

Project description:BackgroundTo evaluate digital tracheal intubation (DTI) when compared to laryngoscope-assisted TI; finger palpation of endotracheal tube (ETT) tip position when compared to any standard method.DesignA systematic review of Medline, Embase, CENTRAL, and CINAHL with synthesis of data using meta-analysis was performed.Main outcome measureThe proportion of successful TI and correct ETT tip positioning were the main outcome measures.ResultsFive studies (one observational study and four RCTs) enrolling 310 neonates were included. 94% (81%-98%) of the DTI were successful on the first attempt (certainty of evidence [CoE]: low). The proportion of successful intubation on the first attempt was higher with DTI when compared to laryngoscope-assisted TI (RR 95% CI: 1.81 [1.18; 2.76]) (CoE: very low). Time to successful TI with DTI was 7.4 (95% CI: 6.3, 8.5) s (CoE: low). Time to successful TI was significantly shorter with DTI when compared to laryngoscope assisted TI (MD [95% CI]: -4.9 [-7.3, -2.4] s) (CoE: very low). There was a trend towards a higher proportion of correct ETT tip positions with finger palpation when compared to weight-based formulae alone (RR 95% CI: 1.12 [0.96; 1.31]) (CoE: very low).ConclusionsDTI and finger palpation to ascertain ETT tip position in neonates are promising strategies. Future studies with emphasis on their learning trajectory and generalizability are needed.

Trans-inactivation: Repression in a wrong place.

Project description:Trans-inactivation is the repression of genes on a normal chromosome under the influence of a rearranged homologous chromosome demonstrating the position effect variegation (PEV). This phenomenon was studied in detail on the example of brownDominant allele causing the repression of wild-type brown gene on the opposite chromosome. We have investigated another trans-inactivation-inducing chromosome rearrangement, In(2)A4 inversion. In both cases, brownDominant and In(2)A4, the repression seems to be the result of dragging of the euchromatic region of the normal chromosome into the heterochromatic environment. It was found that cis-inactivation (classical PEV) and trans-inactivation show different patterns of distribution along the chromosome and respond differently to PEV modifying genes. It appears that the causative mechanism of trans-inactivation is de novo heterochromatin assembly on euchromatic sequences dragged into the heterochromatic nuclear compartment. Trans-inactivation turns out to be the result of a combination of heterochromatin-induced position effect and the somatic interphase chromosome pairing that is widespread in Diptera.

Why pictures look right when viewed from the wrong place.

Project description:A picture viewed from its center of projection generates the same retinal image as the original scene, so the viewer perceives the scene correctly. When a picture is viewed from other locations, the retinal image specifies a different scene, but we normally do not notice the changes. We investigated the mechanism underlying this perceptual invariance by studying the perceived shapes of pictured objects viewed from various locations. We also manipulated information about the orientation of the picture surface. When binocular information for surface orientation was available, perceived shape was nearly invariant across a wide range of viewing angles. By varying the projection angle and the position of a stimulus in the picture, we found that invariance is achieved through an estimate of local surface orientation, not from geometric information in the picture. We present a model that explains invariance and other phenomena (such as perceived distortions in wide-angle pictures).

Opioids and cancer survival: are we looking in the wrong place?

Project description:There is a controversial narrative in the anaesthetic literature suggesting that anaesthetic technique (including opioids) may be detrimental to survival after tumour resection. The initial observations were retrospective. Several prospective studies are ongoing; one in breast cancer has reported no adverse outcome. The evidence for an effect of opioids stems from three pieces of information: (1) opioids depress the immune system, (2) opioids potentially promote angiogenesis, and (3) opioids potentially support tumour growth. Although the evidence for (2)/(3) is unclear, combinations of these effects are beneficial to tumours and potentially promote metastatic reseeding. Accepted wisdom suggests that opioid effects are driven by opioid receptor activation but the presence of opioid receptors on immune cells for example is unlikely. Immune cells, vascular endothelium and a range of tumour cells express Toll-like receptor 4 (TLR4) receptors (for Gram-negative bacterial wall components), and there is growing evidence for opioids interacting with this alternative receptor; and for some there is paradoxical naloxone sensitivity. Is the focus on opioid receptors and cancer the wrong target? TLR4 receptor activation produces immune activation, stimulates angiogenesis, and supports tumour survival. We know that some opioids are more immune suppressive than others (there is no such comparative information for angiogenesis and tumour survival); this may correlate with TLR4 activation. If there are clusters of opioids that have more opioid than TLR4 profiles and vice versa, this may influence outcome. If this is the case, then evidence-based advice could be given for perioperative use in the oncology–anaesthesia setting.

Effect of tracheal suctioning on aspiration past the tracheal tube cuff in mechanically ventilated patients.

Project description:UnlabelledBackgroundThis clinical study evaluated the effect of a suctioning maneuver on aspiration past the cuff during mechanical ventilation.MethodsPatients intubated for less than 48 hours with a PVC-cuffed tracheal tube, under mechanical ventilation with a PEEP ≥5 cm H2O and under continuous sedation, were included in the study. At baseline the cuff pressure was set at 30 cm H2O. Then 0.5ml of blue dye diluted with 3 ml of saline was instilled into the subglottic space just above the cuff. Tracheal suctioning was performed using a 16-French suction catheter with a suction pressure of - 400 mbar. A fiberoptic bronchoscopy was performed before and after the suctioning maneuver, looking for the presence of blue dye in the folds within the cuff wall or in the trachea under the cuff. The sealing of the cuff was defined by the absence of leakage of blue dye either in the cuff wall or in the trachea under the cuff.ResultsTwenty-five patients were included. The size of the tracheal tube was 7-mm ID for 5 patients, 7.5-mm ID for 16 patients, and 8-mm ID for four patients. Blue dye was never seen in the trachea under the cuff before suctioning and only in one patient (4%) after the suctioning maneuver. Blue dye was observed in the folds within the cuff wall in 6 of 25 patients before suctioning and 11 of 25 after (p = 0.063). Overall, the incidence of sealing of the cuff was 76% before suctioning and 56% after (p = 0.073).ConclusionsIn patients intubated with a PVC-cuffed tracheal tube and under mechanical ventilation with PEEP ≥5 cm H2O and a cuff pressure set at 30 cm H2O, a single tracheal suctioning maneuver did not increase the risk of aspiration in the trachea under the cuff.Trial registrationClinicalTrials.gov, number NCT01170156.
