ABSTRACT: Introduction
An educational mismatch is defined as the situation where the education qualifications of an employee do not match the qualifications required for the job they do. A mismatch can be vertical where the level of the employee's qualification is not the one required by the job. This study contributes to the literature on work-related social determinants of health, by carrying out the first assessment of the relationship between educational mismatch and health in Italy.Methods
Data come from PLUS, a national survey of labor supply. The risk of suffering from bad or very bad health associated with educational mismatch is investigated through a logistic regression model accounting for the socioeconomic context and occupation.Results
Our findings show women are at greater risk of suffering from bad or very bad health than men, especially if under-educated.Discussion
Our results show the need to address more research on work-related social determinants of health, which can represent a barrier to achieving health equity.

Project description:BackgroundPeru is a Latin American country with a significant burden of hypertension that presents worrying rates of disparities in socioeconomic determinants. However, there is a lack of studies exploring the association between those determinants, hypertension and prehypertension in Peruvian population.ObjectiveWe aimed to assess the association betwgeen socioeconomic determinants, hypertension and prehypertension using a nationally representative survey of Peruvians.MethodsWe performed a cross-sectional analysis of the Peruvian Demographic and Health Survey (2018), which is a two-staged regional-level representative survey. We used data from 33,336 people aged 15 and older. The dependent variable was blood pressure classification (normal, prehypertension and hypertension) following the Seventh Report of the Joint National Committee (JNC-7) on hypertension management. Independent variables were socioeconomic: age, sex, marital status, wealth index, health insurance, education, region and area of residence. Due to the nature of the dependent variable (more than two categories), we opted to use the multinomial regression model, adjusting the effect of the multistage sample using the svy command. We tested interactions with the adjusted Wald test.ResultsThe prevalence of prehypertension and hypertension was 33.68% and 19.77%, respectively. Awareness was higher in urban than in rural areas (9.61% vs. 8.31%, p = 0.008). Factors associated with a higher prevalence ratio of both prehypertension and hypertension were age (ratios rose with each age group), male sex (prehypertension aRPR 5.15, 95%CI 4.63-5.73; hypertension aRPR 3.85, 95% CI 3.37-4.40) and abdominal obesity (prehypertension aRPR 2.11, 95%CI 1.92-2.31; hypertension aRPR 3.04, 95% CI 2.69-3.43). Factors with a lower prevalence ratio of both diseases were secondary education (prehypertension aRPR 0.76, 95%CI 0.60-0.95; hypertension aRPR 0.75, 95% CI 0.58-0.97), higher education (prehypertension aRPR 0.78, 95%CI 0.61-0.99; hypertension aRPR 0.62, 95% CI 0.46-0.82), being married/cohabiting (prehypertension aRPR 0.87, 95%CI 0.79-0.95; hypertension aRPR 0.77, 95% CI 0.68-0.87), richest wealth index (only prehypertension aRPR 0.76, 95%CI 0.63-0.92) and living in cities different to Lima (rest of the Coastline, Highlands and Jungle). Having health insurance (only hypertension aRPR 1.26, 95%CI 1.03-1.53) and current drinking (only prehypertension aRPR 1.15, 95%CI 1.01-1.32) became significant factors in rural areas.ConclusionsWe evidenced socioeconomic disparities among people with hypertension and prehypertension. Better health policies on reducing the burden of risk factors are needed, besides, policy decision makers should focus on hypertension preventive strategies in Peru.

Project description:ObjectivesTo quantify how poor health and inhospitable working conditions each contribute to educational disparities in work disability in midlife and old age.MethodsWe used the Health and Retirement Study (2006-2016) to examine educational disparities in reporting "any impairment or health problem that limits the kind or amount of paid work" in ages 51-80.ResultsWe found disparities to be profound and persistent over time. Blinder-Oaxaca threefold decomposition revealed that distributions of income and employer insurance made the largest contribution to explaining different rates of work limitations among respondents with versus without high school degrees, followed by work characteristics (physical job demands, insufficient hours) and health conditions (diabetes, lung disease). Comparing respondents with high school versus college degrees, distributions of health conditions mattered most (high blood pressure, lung disease, heart disease, stroke), followed by health behaviors (smoking, drinking). Health-induced work limitations are often used as a measure of health, but we found that work characteristics explained 57% of the disadvantage of those without a high school degree and 44% of the disadvantage of high school compared to college graduates.DiscussionWork environments appear to play an important role in educational disparities in mid- to late-life disability.

Project description:IntroductionMaternal health during pregnancy plays a part in child health, and several conditions have been associated with adverse child outcomes.ObjectivesTo determine the socioeconomic determinants and maternal health factors associated with dental caries in young children.MethodsThis cross-sectional study is part of a register-based cohort study including all children who were born from 2000 to 2003 and were residing in Stockholm County, Sweden, at age 3 y (n = 73,658). The study followed the cohort until individuals were 7 y old. The final study cohort comprised all children examined at 3 and 7 y (n = 65,259). Data on socioeconomic conditions, maternal health, and maternal health behavior were extracted from Swedish national registries. The multivariate analyses used 2 outcomes: caries experience at age 3 and 7 y (deft > 0 [decayed, extracted, and filled teeth]).ResultsThe results of this study show that socioeconomic and maternal health behaviors during pregnancy are important determinants of oral health in their preschool offspring. When all significant risk factors were present, the cumulative probability of being diagnosed with dental caries at age 7 y was 75%.ConclusionThis study also showed that maternal obesity and smoking during pregnancy were predictors of dental caries in preschool children. Strategies must be developed for increasing maternal motivation and self-efficacy and providing mothers with knowledge and caries-preventive tools.Knowledge transfer statementThe results of this study inform clinicians about the importance of including a more detailed history regarding maternal health and maternal health behaviors during pregnancy to assess caries risk in preschool children. Education, income, and other socioeconomic factors are difficult to modify in the short term. Therefore, strategies must be developed to increase parental motivation and self-efficacy to give parents the determination, knowledge, and tools for prevention.

Project description:BackgroundSocioeconomic factors play a critical role in influencing mental health outcomes, particularly during periods of crisis such as the COVID-19 pandemic. In Hawai'i, working adults face unique challenges related to employment, food security, and trust in community safety measures, which may exacerbate risks for depression, low self-esteem, and suicidal ideation. Understanding the interplay of these factors is crucial to addressing mental health disparities and informing targeted policy interventions.MethodsThis study analyzed data from 2,270 adults aged 18 to 65 residing in Hawai'i, collected in 2022. Using probit regression models and conditional inference decision trees, the study assessed the impact of 15 socioeconomic and demographic factors on mental health outcomes, specifically symptoms of depression, low self-esteem, and suicidal ideation. Key variables of interest included food security status, employment, marital status, pre-existing health conditions, and perceptions of COVID-19-related community safety.ResultsThe findings revealed significant mental health challenges among the participants, with 39.6% reporting symptoms of depression, 14.7% experiencing low self-esteem, and 4.2% expressing suicidal ideation. Food insecurity emerged as the most significant predictor of poor mental health, particularly for depression and suicidal ideation. Within the food-insecure group, individuals with pre-existing health conditions faced worsened mental health outcomes, while marital status served as a protective factor. Employment reduced the likelihood of depression by 2.8%, and perceptions of community safety during the COVID-19 pandemic were associated with a 9.9% reduction in depression risk.ConclusionFood insecurity, particularly when coupled with pre-existing health vulnerabilities, is a critical risk factor for adverse mental health outcomes among working adults in Hawai'i. Employment and positive perceptions of community safety were identified as key protective factors. These findings highlight the urgent need for targeted interventions to improve food security and foster community trust and safety.

Project description:This study aimed to analyze the prevalence of work-related musculoskeletal disorders (WMSD) and their association with individual and contextual factors in the Brazilian population. This quantitative cross-sectional study used secondary data from the Brazilian National Health Survey from August 2013 to February 2014. The dependent variable included WMSD, and independent variables were analyzed as individual and contextual factors. WMSD was mostly prevalent in females, individuals aged 43 to 59 years, with chronic physical or mental disorders, reporting frequent sleep disorders, and performing integrative and complementary health practices, physical exercise or sports, and heavy physical activity or housework. Regarding contextual factors, high social classes and proportion of individuals with formal work were associated with a high prevalence of WMSD, whereas a high Gini index was associated with a low prevalence. Thus, a high prevalence of WMSD in the Brazilian population was associated with individual and contextual factors, which should be the target of health professionals for actions of promotion, prevention, and intervention at individual or collective care levels.

Project description:Schedule control and supervisor support for family and personal life are work resources that may help employees manage the work-family interface. However, existing data and designs have made it difficult to conclusively identify the effects of these work resources. This analysis utilizes a group-randomized trial in which some units in an information technology workplace were randomly assigned to participate in an initiative, called STAR, that targeted work practices, interactions, and expectations by (a) training supervisors on the value of demonstrating support for employees' personal lives and (b) prompting employees to reconsider when and where they work. We find statistically significant, though modest, improvements in employees' work-family conflict and family time adequacy and larger changes in schedule control and supervisor support for family and personal life. We find no evidence that this intervention increased work hours or perceived job demands, as might have happened with increased permeability of work across time and space. Subgroup analyses suggest the intervention brings greater benefits to employees more vulnerable to work-family conflict. This study advances our understanding of the impact of social structures on individual lives by investigating deliberate organizational changes and their effects on work resources and the work-family interface with a rigorous design.

Project description:The purpose of this study is to identify inequities in the distribution of socioeconomic determinants of health within Poland and their impact on the health status of Poles, as measured by mortality rate. We hypothesised that (1) there are inequities in the socioeconomic characteristics within geographically defined population groups and (2) some socioeconomic determinants of health have a particularly strong impact on the health status of Poles. Poland is administratively divided into three levels: voivodeships, powiats and gminas. We used a dataset covering all 380 powiats in Poland for the year 2018. We employed a two-stage nested Theil index and Herfindahl-Hirschman Index. In order to identify which of these determinants has the strongest impact on health, we conducted a regression analysis. The study revealed some inequities in the distribution of socioeconomic determinants of health. The mortality rate can be partly understood from variations within voivodeships in the distribution of health determinants. Important national inequalities were found in the case of two determinants, which simultaneously proved their significant impact on Poles' health in the regression analysis. Thus, type of employment and access to modern infrastructure should be of particular concern for public authorities.

Project description:The World Health Organization (WHO) states that examining medical financial systems is the most important process in evaluating universal health coverage (UHC). This study used the service coverage index (SCI) as a proxy of the progress toward UHC in eleven Asian countries. We employed a fixed-effects regression model to analyze panel data from 2015 to 2017, to explain the interrelationship between the SCI and major socioeconomic indicators. We also conducted a performance analysis (ratio of achieved SCI level to gross domestic product (GDP) or health expenditure displacement) to examine the balance between the degree of achievements related to UHC and a country's economic level. The results showed that GDP and health expenditure were significantly positively correlated with the SCI (p < 0.01). The panel data analysis results showed that GDP per capita was a factor that greatly influenced the SCI as well as poverty (partial regression coefficient: 0.0017, 95% CI: 0.0013-0.0021). The results of the performance analysis showed that the Philippines had the highest scores (GDP: 1.84 SCI score/USD per capita, health expenditure: 1.04 SCI score/USD per capita) and South Korea the lowest. We conclude that socioeconomic factors, such as GDP, health expenditure, unemployment, poverty, and population influence the progress of UHC, regardless of system maturity or geographic characteristics.

Project description:The natural stone sector is an important driver of the Spanish and Italian economies, which underwent internationalization after the financial crisis of 2008 as part of a survival and development strategy. This article aims to study the financial and economic profitability of this sector in the two leading European production countries, as well as its determinants. For this purpose, the economic-financial data of a sample composed of 453 companies (203 Spanish and 250 Italian) from 2015-2019 were analyzed using the multiple linear regression methodology. To address the problems of possible endogeneity and omission of variables in the model, the dependent variable was used as a regressor with one and two lags, and panel data with fixed effects were considered after performing the Hausman test. The results show significant differences between the two countries, with higher profitability in Italy. Company size, company growth (measured as the change in assets), and the variation in the country's GDP all positively affected profitability. At the same time, the level of indebtedness showed a negative relationship. The country's inflation rate and gender diversity in top management were shown to be non-relevant variables. The research conducted indicates that, to increase profitability, Spanish and Italian companies in the natural stone sector should undergo mergers in order to grow in size, increase efficiency in the use of assets, reduce their dependence on external financing, and promote equity capital. In addition, Italian companies should reduce the average period of payment to suppliers to lower deferral costs, and boost exports to become less dependent on the country's domestic economy.

Project description:BackgroundThis study aimed to examine the association between educational level and unable to work due to ill-health (UWdIH) among 30- to 79-year-old South Koreans.MethodsA cross-sectional nationwide survey of the 2010-2016 Korea National Health and Nutritional Evaluation Survey was used for analyses. A total of 29,930 participants aged ≥30 and < 80 years, who do not have any disability in their daily life because of health problems, were included. Educational level and reason for nonworking are self-reported with multiple choices. Multivariate logistic regression was used to examine an association between education level and UWdIH by setting those who graduated college as their final education (n = 6,997) as a baseline while controlling for potential confounding factors.ResultsIn the age-stratified result, the ratio of UWdIH was increased as age increases in 3 educational groups (p < .0001). There was a tendency that low-level educated [International Standard Classification of Education (0-2)] participants showed higher ratio of UWdIH than high-level educated [International Standard Classification of Education (5≤)] participants in both sexes (odds ratio: 2.54, 95% confidence interval: 2.12-3.05).ConclusionThere is a clear link between educational level and UWdIH; the less the educated, the more likely to be UWdIH. Policy priority should be given to plans that can help this vulnerable social group to work and enjoy healthy lives.
