ABSTRACT: Epicardial connections provided the anatomical substrate for the biatrial reentry circuit. The connections between the right atrium and right pulmonary vein were called "intercaval bundle," and there are few reports of atrial flutter related to this bundle. We present a case of a biatrial tachycardia, involving the intercaval bundle.

Project description:A 78-year-old woman presented 2 years after mitral valve replacement for rheumatic mitral stenosis with cardioversion-resistant atrial tachycardia (AT). Dual-loop AT was identified by activation mapping with the Rhythmia™ system (Boston Scientific, Marlborough, MA, USA) and confirmed by entrainment-mapping; one circuit with localized re-entry turned around the scar on the posterior left atrium and the other circuit, which was macro re-entrant, turned around the left superior pulmonary vein (LSPV) using the PV-carina, the ridge beween the left atrial appendage and the LSPV, and the roof. The two wavefronts fused on the posterior wall close to the LSPV. Radiofrequency ablation of an area of slow conduction on the posterior wall changed the tachycardia to roof-dependent AT which was then terminated by completion of a roof line. <Learning objective: In this case report, we demonstrate the importance of using both conventional entrainment mapping methods and a novel ultra-high density mapping system to precisely understand the mechanism and appropriately terminate the complex atrial tachycardia in patients with prior atrial fibrillation ablation.>.

Utility of coherent and ripple mapping for post-mitral valve plasty biatrial macro-re-entrant tachycardia: a case report.

Project description:BackgroundBiatrial tachycardia (BiAT) is a rare form of atrial macro-re-entrant tachycardia. Precise identification of interatrial connections and circuits of the BiAT is difficult. And incomplete understanding of the re-entrant circuit may lead to unnecessary ablation, thus increasing the risk of complications.Case summaryA 69-year-old man with a history of mitral valve plasty for mitral regurgitation due to mitral valve prolapse was admitted for persistent atrial tachycardia. Electrophysiological examination using the CARTO mapping system was performed. A coherent map revealed an atrial tachycardia with a cycle length of 304 ms and a re-entrant circuit involving the left atrial septum and right atrial septum, while a ripple map suggested an epicardial interatrial connection between the right atrium and left atrium. Radiofrequency ablation on the epicardial connection successfully terminated BiAT without complications.DiscussionIn BiAT using both atrial septum as a re-entrant circuit, an interatrial connection or an atrial septum can be the target site for ablation. However, septal ablation can be challenging because of the risk of atrioventricular block or interatrial conduction delay, and minimal line or point ablation is needed. Coherent and ripple mapping can accurately determine the re-entrant circuit and interatrial connection of BiAT and reduce complication risks by terminating the atrial tachycardia with minimal ablation.

Atrioventricular nodal reentry tachycardia in pregnancy: 'I have ice for you'.

Project description:BackgroundPregnancy is a known trigger of novel and pre-existing supraventricular tachyarrhythmias. We present a case of a stable pregnant patient presenting with atrioventricular nodal reentry tachycardia (AVNRT) and application of the 'facial ice immersion technique'.Case summaryA 37-year-old pregnant woman presented with recurrent AVNRT. Due to unsuccessful attempts of conventional vagal manoeuvres (VMs) and refusal of pharmacological agents, we successfully performed a non-conventional VM with the 'facial ice immersion technique'. This technique was applied successfully at repeated clinical presentation.DiscussionThe role of non-pharmacological interventions remains pivotal and may lead to desired therapeutical effects without the use of any costly pharmacological agents with possible adverse events. However, non-conventional VMs such as the 'facial ice immersion technique' are less commonly known but appear to be easy and a safe option for both mother and foetus in the management of AVNRT during pregnancy. Clinical awareness and understanding of treatment options are imperative in contemporary patient care.

Pulsed-field ablation for the treatment of left atrial reentry tachycardia.

Project description:BackgroundWe describe our initial experience using a multipolar pulsed-field ablation catheter for the treatment of left atrial (LA) reentry tachycardia.MethodsWe included all patients with LA reentry tachycardia treated with PFA at our institution between September 2021 and March 2022. The tachycardia mechanism was identified using 3D electro-anatomical mapping (3D-EAM). Subsequently, a roof line, anterior line, or mitral isthmus line was ablated as appropriate. Roof line ablation was always combined with LA posterior wall (LAPW) ablation. Positioning of the PFA catheter was guided by a 3D-EAM system and by fluoroscopy. Bidirectional block across lines was verified using standard criteria. Additional radiofrequency ablation (RFA) was used to achieve bidirectional block as necessary.ResultsAmong 22 patients (median age 70 (59-75) years; 9 females), we identified 27 LA reentry tachycardia: seven roof dependent macro-reentries, one posterior-wall micro-reentry, twelve peri-mitral macro-reentries, and seven anterior-wall micro-reentries. We ablated a total of 20 roof lines, 13 anterior lines, and 6 mitral isthmus lines. Additional RFA was necessary for two anterior lines (15%) and three mitral isthmus lines (50%). Bidirectional block was achieved across all roof lines, 92% of anterior lines, and 83% of mitral isthmus lines. We observed no acute procedural complications.ConclusionAblation of a roof line and of the LAPW is feasible, effective, and safe using this multipolar PFA catheter. However, the catheter is less suited for ablation of the mitral isthmus and the anterior line. A focal pulsed-field ablation catheter may be more effective for ablation of these lines. This study shows the feasibility to ablate linear lesions with a multipolar pulsed-field ablation catheter. 27 left atrial reentry tachycardia were treated in 22 patients.

Micro-reentry right atrial tachycardia originating from fossa ovalis: a case report of high-density mapping by PentaRay catheter.

Project description:BackgroundMicro-reentry tachycardia usually emerges in scar tissues related to post-atrial fibrillation ablation and cardiomyopathy. It is difficult to identify the micro-reentry circuit accurately by conventional mapping method.Case summaryA 74-year-old man presented with paroxysmal atrial tachycardia (AT) presenting as palpitations. He was evaluated by an electrophysiological examination using a high-density CARTO mapping system. The mapping results showed the AT with a cycle length of 184 ms was focused on his right atrial fossa ovalis (FO). In this small area, the high-density mapping demonstrated a significant micro-reentrant tachycardia. Radiofrequency ablation at the centre of the micro-reentrant circuit successfully terminated the AT. No recurrences were observed during a 12-month follow-up.DiscussionThis case demonstrated a micro-reentrant AT originates from the FO without cardiomyopathy or previous ablation with specific loops. This is an unusual location for AT though and can cause difficulty for operators if it terminates or is non-sustained. High-density mapping using a PentaRay catheter can effectively characterize micro-reentrant circuits and determine the real target for ablation therapy.

Project description:Biatrial tachycardia (BiAT) is a rare arrhythmia, and identification of the re-entry circuit is often complicated. By creating an activation map of the right atrium, left atrium, and coronary sinus as a single chamber, the LUMIPOINT module of the Rhythmia mapping system (Boston Scientific, Marlborough, MA, USA) can be used in real time to make an accurate diagnosis. Ablation of the Bachmann bundle is a feasible way to terminate BiAT, but might cause interatrial conduction delay and electrical isolation of the left atrial appendage. Chemical ablation into the vein of Marshall might be the more beneficial treatment, avoiding any potential interatrial conduction delay.
