Spinal Arachnoid Web.

ABSTRACT:

Case presentation

We describe a case of a 57-year-old male with multiple medical comorbidities who presented to the emergency department with a two-week history of upper back pain with associated numbness. Physical exam demonstrated sensory loss in a bilateral third and fourth thoracic dermatome distribution. The diagnosis of spinal arachnoid web was made based on neurological exam and imaging findings.

Discussion

Spinal arachnoid web is a rare diagnosis, but consideration is important, as early recognition and surgical intervention can resolve symptoms and prevent worsening neurological sequelae.

Endoscopic treatment of spinal arachnoid cysts.

Project description:Background/objectiveSpinal arachnoid cysts (SAC) are intradural lesions, which may provoke a compression of the spinal cord and roots. Endoscopic techniques are increasingly used to minimize the surgical access and the postoperative scar tissue. Shunts may also represent an option. The aim of this paper is to illustrate the technique of endoscopic-assisted fenestration and positioning of a cysto-peritoneal diversion in a thoracic SAC using a flexible endoscope and to perform a systematic literature review on this subject.Material and methodsWe reported our case and we performed a review of the literature, searching for all the adult cases of Type III SACs in English language treated through endoscopic procedures.ResultsWe found 5 articles matching our search criteria and we included 9 adult patients in our analysis. Six patients were females and the most common localization was the thoracic spine. Six patients underwent selective laminectomies followed by endoscopic fenestration without cyst wall resection. Three patients had a percutaneous endoscopic inspection of the cyst and in two cases a cysto-subarachnoid shunt space was performed. Improvement of pre-operative neurological deficit was reported in six patients, no patients experienced clinical deterioration. The mean follow-up was 22 months and no progression or recurrence was reported.ConclusionThe implementation of endoscopy allows a minimally invasive treatments with good visualization of cyst anatomy and precise shunt positioning under real-time guidance. Endoscopy is technically demanding but it can offer similar clinical outcomes when compared to microscopic procedures with a limited rate of post-operative complications.The long-term risk of recurrence should be established by prospective studies.

Spinal extradural arachnoid cyst in cervicothoracic junction.

Project description:Introduction:Spinal extradural arachnoid cysts (SEACs) are rare expansive lesions that account for ~1% of all spinal tumors. They develop as the result of arachnoid membrane herniation through a small dural defect and are mostly reported to be located at lower thoracic region and thoracolumbar junction. Case presentation:We report a rare case of SEAC located in the cervicothoracic junction causing neurologic compression. Discussion:Only 3% of all SEACs are observed in the cervical spine. They are the most common in young men and present with compressive symptoms. SEACs have potential to enlarge. As they are surgically removable lesions, early and definite diagnosis is important in preventing clinical deterioration. MRI is recommended for the diagnosis and preoperative planning.

A novel FOXC2 mutation in spinal extradural arachnoid cyst.

Project description:Spinal extradural arachnoid cyst (SEDAC) is a cyst in the spinal canal, which causes spinal cord compression and subsequent neurological damage. We previously identified two FOXC2 mutations in two SEDAC families. The FOXC2 mutations have been shown to be responsible for lymphedema-distichiasis syndrome (LDS), which includes SEDAC as an occasionally associated phenotype. We encountered a non-familial patient with SEDAC associated with LDS, and identified a novel nonsense mutation in FOXC2, c.349C>T (p.Q117*).

Identification of HOXD4 Mutations in Spinal Extradural Arachnoid Cyst.

Project description:Spinal extradural arachnoid cyst (SEDAC) is a cyst in the spinal canal that protrudes into the epidural space from a defect in the dura mater and leads to neurological disturbances. We previously showed that familial SEDAC is caused by FOXC2 mutation; however, the causal gene of sporadic SEDAC has not been identified. To identify the causal gene of sporadic SEDAC, we performed whole exome sequencing for 12 subjects with sporadic SEDAC and identified heterozygous HOXD4 loss-of-function mutations in three subjects. HOXD4 haplo-insufficiency causes SEDAC and a transcriptional network containing HOXD4 and FOXC2 is involved in the development of the dura mater and the etiology of SEDAC.

Arachnoid webs with spinal cord compression: insights from three cases.

Project description:Spinal arachnoid webs are intradural bands of abnormally formed arachnoid tissue, located within the subarachnoid space and causing compression of the dorsal aspect of the spinal cord. Arachnoid webs are uncommon and can be difficult to treat. We report 3 patients presenting with a spinal arachnoid web within a 6-month period. All of them exhibited signs of thoracic myelopathy and the MRI showed the pathognomonic 'scalpel sign'. Two of the patients underwent surgery for removal of their spinal arachnoid web, whereas the third patient case is currently being managed conservatively. We also present our 2D intraoperative video for arachnoid web removal and spinal cord decompression.

FOXC2 mutations in familial and sporadic spinal extradural arachnoid cyst.

Project description:Spinal extradural arachnoid cyst (SEDAC) is a cyst in the spinal canal that protrudes into the epidural space from a defect in the dura mater. Most cases are sporadic; however, three familial SEDAC cases have been reported, suggesting genetic etiological factors. All familial cases are associated with lymphedema-distichiasis syndrome (LDS), whose causal gene is FOXC2. However, FOXC2 mutation analysis has been performed in only 1 family, and no mutation analysis has been performed on sporadic (non-familial) SEDACs. We recruited 17 SEDAC subjects consisting of 2 familial and 7 sporadic cases and examined FOXC2 mutations by Sanger sequencing and structural abnormalities by TaqMan copy number assay. We identified 2 novel FOXC2 mutations in 2 familial cases. Incomplete LDS penetrance was noted in both families. Four subjects presented with SEDACs only. Thus, SEDAC caused by the heterozygous FOXC2 loss-of-function mutation should be considered a feature of LDS, although it often manifests as the sole symptom. Seven sporadic SEDAC subjects had no FOXC2 mutations, no symptoms of LDS, and showed differing clinical characteristics from those who had FOXC2 mutations, suggesting that other gene(s) besides FOXC2 are likely to be involved in SEDAC.

Spinal Arachnoid Diverticula: Outcome in 96 Medically or Surgically Treated Dogs.

Project description:BackgroundLittle is reported about the role of medical management in the treatment of spinal arachnoid diverticula (SAD) in dogs.ObjectivesTo describe the outcome of 96 dogs treated medically or surgically for SAD.AnimalsNinety-six dogs with SAD.MethodsRetrospective case series. Medical records were searched for spinal arachnoid diverticula and all dogs with information on treatment were included. Outcome was assessed with a standardized questionnaire.ResultsFifty dogs were managed medically and 46 dogs were treated surgically. Dogs that underwent surgery were significantly younger than dogs that received medical management. No other variables, related to clinical presentation, were significantly different between both groups of dogs. The median follow-up time was 16 months (1-90 months) in the medically treated and 23 months (1-94 months) in the surgically treated group. Of the 38 dogs treated surgically with available long-term follow-up, 82% (n = 31) improved, 3% (n = 1) remained stable and 16% (n = 6) deteriorated after surgery. Of the 37 dogs treated medically with available long-term follow-up, 30% (n = 11) improved, 30% (n = 11) remained stable, and 40% (n = 15) deteriorated. Surgical treatment was more often associated with clinical improvement compared to medical management (P = .0002).Conclusions and clinical importanceThe results of this study suggest that surgical treatment might be superior to medical treatment in the management of SAD in dogs. Further studies with standardized patient care are warranted.

Spinal arachnoid diverticula in cats: Clinical presentation, diagnostic imaging findings, treatment, and outcome.

Project description:BackgroundSpinal arachnoid diverticulum (SAD) is considered a rare disease in cats. Previous reports mainly classified SAD in cats as acquired.Hypothesis/objectivesThe aim of this study was to describe the signalment, clinical presentation, diagnostic imaging findings, and outcome in a group of cats with SAD.AnimalsTwenty-one client-owned cats.MethodsMulticenter observational retrospective review of the medical records of cats diagnosed with SAD by magnetic resonance imaging.ResultsMost cats were Domestic Short Hair (67%), male (63%), and had a wide range of ages (18 weeks to 13 years old). Neuroanatomical localization was consistent with a T3-L3 myelopathy in 18 cats (86%) and C1-C5 myelopathy in 3 cats (14%). One cat with a C1-C5 myelopathy demonstrated bilateral vestibular clinical signs. One cat (5%) had fecal incontinence. Most cats demonstrated a chronic, progressive, nonlateralized, nonpainful myelopathy. No underlying previous or concurrent spinal condition was found in 48% of the cats. No difference in age, body weight, breed, sex, treatment, or outcome was found between cats with or without a concurrent spinal disorder. One cat was euthanized after diagnosis. Six cats improved, 1 deteriorated and 1 remained static after surgery, whereas 3 cats improved, 5 deteriorated and 4 remained static after medical management on the short-term outcome. Overall, 73% (8/11) of the cats deteriorated on available long-term follow-up information.Conclusions and clinical importanceSpinal arachnoid diverticulum should be considered for cats with chronic, progressive, symmetrical, nonpainful myelopathy, particularly if male and with a history of spinal disease or surgery.

Spinal extradural arachnoid cysts: A novel formation mechanism and dural defect location technology.

Project description:PurposeThe formation mechanism of spinal extradural arachnoid cysts (SEACs) remains unclear. There are several hypotheses for the formation of SEACs, but none of them can fully explain its pathological findings and surgical procedures. In this study, we retrospectively analyzed the cases of SEACs, aiming to clarify the formation mechanism of SEACs. In addition, we summarize a concise method for locating dural defects preoperatively and formulate a putative explanation of this method.MethodsThe clinical data of 14 patients with SEACs underwent surgery in our hospital from January 2017 to December 2021 were retrospectively analyzed.ResultsFourteen patients were identified during the study period. The cysts all spanned the T12/L1 segment, and dural defects were also located at the T12/L1 level (2 cases not recorded) as well as the middle or the upper-middle level of the cysts. Nine cases were treated with total cyst excision, 2 cases were treated with dural defect closure only, and 3 cases were treated with total cyst excision and dural defect closure. Histopathological examination demonstrated that the cyst wall contained both the arachnoid epithelial and compact fibrous connective tissue. The symptoms were relieved in all patients, and no recurrence was observed.ConclusionsAccording to intraoperative and pathological findings, the dural outer layer cyst (DOLC) is a more reasonable hypothesis about SEACs formation. When CT myelography or cinematic MRI cannot determine the location of the dural defect preoperatively, it can be located according to the middle level of the SEACs with high accuracy.

Interhemispheric arachnoid cyst.

Project description:BackgroundInterhemispheric arachnoid cysts are uncommon and typically associated with other midline neurodevelopmental disorders, such as complete or partial agenesis of the corpus callosum.Case descriptionWe report a case of a 27-year-old woman with worsening headache, memory deficit, and radiological progression of an interhemispheric arachnoid cyst. The treatment consisted of craniotomy for interhemispheric cyst fenestration into both the interhemispheric cistern and lateral ventricle. The postoperative course was unremarkable, with considerable clinical improvement and significant reduction in cyst size.ConclusionWe successfully treat a patient with an enlarging arachnoid cyst and associated progressive symptoms with microsurgical fenestration.
