Project description:Early mother-infant coordinated interactions play a critical role in infant development. The present study describes the development of the dyadic coordination of vocalization and gaze behavior between mothers and infants over the first year of life. In addition to describing developmental trajectories of behavior, the study contributes to our understanding of dyadic coordination by investigating how different measures of coordination relate to one another and how individual behaviors relate to dyadic coordination within the vocal and gaze domains. Thirty dyads were recorded playing together with a standard set of toys when infants were 3, 6, 9, and 12 months of age, and mother and infant vocalization and gaze behaviors were coded from these videos on a moment-to-moment basis. Coordination was analyzed using both cross-recurrence quantitative analysis and event-based measures of analysis. Hierarchical linear modeling was used to examine developmental trajectories of vocalization and gaze coordination. Results indicated that dyadic coordination of the timing of vocalization and gaze behaviors is a very early emerging phenomenon that is supported by both mothers and infants. Vocal coordination did not show developmental change over time, whereas mothers and infants became increasingly coordinated in their gaze behaviors as infants got older. Taken together, findings from this study highlight the importance of considering bidirectional and developmental influences on parent and infant behavior. Project description:Background/objectiveGreater parent-infant synchrony is associated with improved child outcomes. Behavioral measures of synchrony are still developing in young infants; thus, researchers need tools to quantify synchrony between parents and their young infants. We examined parent-infant neural synchrony measured using dual EEG hyperscanning and associations between neural synchrony, infant behavioral measures of synchrony, and maternal bondedness and depression.MethodsOur prospective cohort study included mother-infant dyads at 2-4 months of age. We collected time-locked dual EEG recordings of mother and infant and simultaneous video-recordings during a scaffolded interaction where the mother sequentially layered sensory modalities to the interaction. Neural synchrony measured using EEG hyperscanning was analyzed using the circular correlation coefficient (CCorr), infant behavioral synchrony was measured using the validated Welch Emotional Connection Screen (WECS) scores, and maternal bondedness and depression were measured using standardized questionnaires.ResultsOur study included n = 47 dyads. Dyadic CCorr increased across the interaction as the mother added tactile stimulation to visual stimulation. We also found associations between behavioral and neural measures of dyadic synchrony such that infants with higher scores on behavioral measures of emotional connection on the WECS showed greater increases in CCorr indicative of dyadic synchrony with their mother across this interaction. We found no associations between neural synchrony and maternal bondedness or depression.ConclusionThese findings support the construct validity of mother-infant dyadic neural synchrony measured using EEG hyperscanning and analyzed using CCorr. Opportunities for future research on quantification of neural synchrony between parents and young infants abound.
Project description:Attachment security is theorized to shape stress reactivity, but extant work has failed to find consistent links between attachment security to mothers and infant cortisol reactivity. We examined family configurations of infant-mother and infant-father attachment security in relation to infant cortisol reactivity. One-year old infants (N = 180) participated in the Strange Situation with mothers and fathers in two counterbalanced lab visits, one month apart (12 and 13 months). Infants with secure attachments only to their fathers and not their mothers had higher cortisol levels than infants with a secure attachment to mother and also exhibited a blunted cortisol response (high at baseline and then a decrease after stress). Results suggest that a secure attachment to father may not be enough to reduce infant stress reactivity when the infant-mother attachment is insecure, and future research is needed to uncover the family dynamics that underlie different family configurations of attachment security.
Project description:BackgroundTinea capitis is a cutaneous fungal infection common among 3 to 7 year old children but it is rare in the first year of life.Case presentationWe present a case of a 12-month-old infant with erythematous scalp lesions combined with hair loss. He was suspected of dermatophytosis and mycological analysis of all suspected lesions was performed. Clinical features and culture results confirmed tinea capitis caused by Microsporum canis. The infant patient was treated with griseofulvin for 2 months. However, 15 days later at the end of treatment he presented with a single vesicle positive for M. canis. Griseofulvin therapy continued for another month. After 3 months of follow-up, no recurrence was observed.ConclusionsIn infant, sometimes tinea capitis is misdiagnosed and underreported because it is similar to other scalp pathologies. Therefore, if erythematous scalp lesions are present, they must be examined from a mycological point of view to inform the differential diagnosis. Once diagnosed, treatment of tinea capitis can pose a dilemma because different factors may influence the choice between equally effective therapies (i.e. safety, age, formulation, cost). This case report suggests that it is important to establish an accurate diagnosis and treatment for this dermatophytosis to avoid recurrences or therapeutic failures, especially in infants.
Project description:Early functioning of neural networks likely underlies the flexible switching between internal and external orientation and may be key to the infant's ability to effectively engage in social interactions. To test this hypothesis, we examined the association between infants' neural networks at 3 months and infant-mother dyadic flexibility (denoting the structural variability of their interaction dynamics) at 3, 6, and 9 months. Participants included thirty-five infants (37% girls) and their mothers (87% White). At 3 months, infants participated in a resting-state functional magnetic resonance imaging session, and functional connectivity (FC) within the default mode (DMN) and salience (SN) networks, as well as DMN-SN internetwork FC, were derived using a seed-based approach. When infants were 3, 6, and 9 months, infant-mother dyads completed the Still-Face Paradigm where their individual engagement behaviors were observed and used to quantify dyadic flexibility using state space analysis. Results revealed that greater within-DMN FC, within-SN FC, and DMN-SN anticorrelation at 3 months predicted greater dyadic flexibility at 6 months, but not at 3 and 9 months. Findings suggest that early synchronization and interaction between neural networks underlying introspection and salience detection may support infants' flexible social interactions as they become increasingly active and engaged social partners.
Project description:Interoceptive sensibility, which denotes the self-perceived sensitivity to inner-body sensations, has been associated with the emotional experiences and inferences of others' emotional states. Focusing on the role of interoceptive sensibility in the emotional states and psychological well-being of mothers during caregiving, this study explores how physiological arousal and interoceptive sensibility mediate the association between mother-infant interaction and maternal well-being using an experience sampling method. Infant-directed-singing (IDS) with social touch was used to facilitate mother-infant interaction. Pairs of 2-8-month-old infants and their mothers participated. Mothers in an IDS group (N = 25) and a no-IDS group (N = 26) recorded their and the infant's daily feelings and physiological states using a smartphone application for one month. All participants, including the control group (N = 78) who neither performed IDS nor used the application, answered the Multidimensional Assessment of Interoceptive Awareness questionnaire before and after the recording period. Results indicated that IDS improved mother-infant interactions and positive feelings, enhancing maternal physiological arousal. Increased interoceptive sensibility enhanced infants' positive feelings in the IDS group, whereas in the no-IDS group, it weakened mother's positive feelings, suggesting that maternal interoceptive sensibility mediated the effects of IDS on mother and infant well-being.
Project description:The human gut microbiota plays a vital role in health and disease, and microbial colonization is a key process in infant development. Here, we analyze 2684 fecal specimens from 12 infants during their first year of life, providing detailed insights into the human gut colonization process. Maturation of the gut microbial community shows strong temporal structure and specific developmental stages. At 2-4 months of age, there is a period of accelerated convergence concurrent with a bloom of Bifidobacterium, a genus associated with metabolism of oligosaccharides found in breast milk. The end of this period coincides with the introduction of solid food, a reduction in the relative abundance of Bifidobacterium, and an increase in several groups of Firmicutes. Our findings highlight the dynamic nature and individuality of the gut colonization process, and the need for high-frequency sampling over an extended period when designing and interpreting infant microbiome studies.
Project description:Perturbations to the gut microbiome are implicated in altered neurodevelopmental trajectories that may shape life span risk for emotion dysregulation and affective disorders. However, the sensitive periods during which the microbiome may influence neurodevelopment remain understudied. We investigated relationships between gut microbiome composition across infancy and temperament at 12 months of age. In 67 infants, we examined if gut microbiome composition assessed at 1-3 weeks, 2, 6, and 12 months of age was associated with temperament at age 12 months. Stool samples were sequenced using the 16S Illumina MiSeq platform. Temperament was assessed using the Infant Behavior Questionnaire-Revised (IBQ-R). Beta diversity at age 1-3 weeks was associated with surgency/extraversion at age 12 months. Bifidobacterium and Lachnospiraceae abundance at 1-3 weeks of age was positively associated with surgency/extraversion at age 12 months. Klebsiella abundance at 1-3 weeks was negatively associated with surgency/extraversion at 12 months. Concurrent composition was associated with negative affectivity at 12 months, including a positive association with Ruminococcus-1 and a negative association with Lactobacillus. Our findings support a relationship between gut microbiome composition and infant temperament. While exploratory due to the small sample size, these results point to early and late infancy as sensitive periods during which the gut microbiome may exert effects on neurodevelopment.
Project description:Exposure to social adversity has been associated with cortisol dysregulation during pregnancy and in later childhood; less is known about how prenatal exposure to social stressors affects postnatal cortisol of infants. In a secondary analysis of data from a longitudinal study, we tested whether a pregnant woman's reports of social adversity during the third trimester were associated with their infant's resting cortisol at 1, 6, and 12 months postnatal. Our hypothesis was that prenatal exposure to social adversity would be associated with elevation of infants' cortisol. Measures included prenatal survey reports of social stressors and economic hardship, and resting cortisol levels determined from infant saliva samples acquired at each postnatal timepoint. Data were analyzed using linear mixed effects models. The final sample included 189 women and their infants (46.56% assigned female sex at birth). Prenatal economic hardship was significantly associated with infant cortisol at 6 months postnatal; reports of social stressors were not significantly associated with cortisol at any time point. Factors associated with hardship, such as psychological distress or nutritional deficiencies, may alter fetal HPA axis development, resulting in elevated infant cortisol levels. Developmental changes unique to 6 months of age may explain effects at this timepoint. More work is needed to better comprehend the complex pre- and post-natal physiologic and behavioral factors that affect infant HPA axis development and function, and the modifying role of environmental exposures.
Project description:Abstract The Multimodal Maternal Infant Perinatal Outpatient Delivery System (MOMI PODS) was developed to facilitate the pregnancy to postpartum primary care transition, particularly for individuals at risk for severe maternal morbidity, via a unique multidisciplinary model of mother/infant dyadic primary care. Specialized clinical informatics platforms are critical to ensuring the feasibility and scalability of MOMI PODS and a smooth perinatal transition into longitudinal postpartum primary care. In this manuscript, we describe the MOMI PODS transition and management clinical informatics platforms developed to facilitate MOMI PODS referrals, scheduling, evidence-based multidisciplinary care, and program evaluation. We discuss opportunities and lessons learned associated with our applied methods, as advances in clinical informatics have considerable potential to enhance the quality and evaluation of innovative maternal health programs like MOMI PODS.
