Comparative efficacy and safety of therapeutic strategies for EGFRvIII positive recurrent glioblastoma.

ABSTRACT: Glioblastoma multiforme (GBM) is the most aggressive primary brain tumor, and EGFRvIII mutation has been associated with treatment resistance and poor prognosis, highlighting the need for more effective therapeutic strategies. We conducted a random-effects Bayesian network meta-analysis to compare the efficacy and safety of treatments for EGFRvIII-positive recurrent GBM (rGBM), evaluating overall survival (OS), progression-free survival (PFS), and objective response rate (ORR). Seven clinical trials were included (n = 716). Rindopepimut (Rind) + Bevacizumab (Beva) emerged as the most promising regimen, supported by its OS advantage in randomized trials, consistently favorable OS and PFS trends, top rankings in all three endpoints, and recommendation by clinical guidelines.
