Association between sodium-glucose cotransporter 2 inhibitor use and clinical outcomes in patients with type 2 diabetes after urinary tract infection.

ABSTRACT:

Aims

To evaluate whether initiating sodium-glucose cotransporter 2 inhibitors (SGLT2i) within 3 months after a urinary tract infection (UTI) is associated with improved long-term outcomes in patients with type 2 diabetes mellitus (T2DM), compared with dipeptidyl peptidase-4 inhibitors (DPP-4i).

Materials and methods

A retrospective cohort study was conducted using the TriNetX US Collaborative Network (2016-2023). Adults with T2DM who initiated SGLT2i or DPP-4i within 90 days after a UTI or pyelonephritis diagnosis were identified. Propensity score matching (1:1) was applied to balance baseline covariates. The primary outcome was mortality. Secondary outcomes included major adverse kidney events (MAKE), major adverse cardiovascular events (MACE), hospitalisation, dialysis dep
