Impact of Mild-to-Moderate Donor Lung Infiltration on Post-lung Transplant Survival Outcomes and Risk Factors: A National Cohort Study.

ABSTRACT:

Objectives

Amid donor shortages, chest X-ray (CXR) is widely used for lung donor screening due to its accessibility, low cost, and rapid imaging. We evaluated the impact of donor CXR infiltrates on post-transplant survival to inform donor lung utilization.

Methods

This retrospective cohort study included adult lung transplant recipients from the United Network for Organ Sharing (UNOS) registry from May 5, 2005 to December 31, 2022. Patients were grouped by the presence or absence of donor CXR infiltrates. Using propensity score matching (PSM) to control variable balance, Kaplan-Meier survival analysis estimated post-transplant survival (at 30 days, 90 days, 1 year, 5 years, and 10 years). Multivariable logistic regression models were applied to identify independent risk dono
