Challenges in Biometry and Intraocular Lens Power Calculations in Keratoconus: A Review.

ABSTRACT: Purpose: The purpose of this work was to conduct a comprehensive literature review of the challenges encountered in ocular biometry and intraocular lens (IOL) power calculations in patients with keratoconus undergoing cataract surgery and to evaluate the performance of various biometric techniques and IOL power calculation formulas in this population. Methods: A comprehensive literature search was conducted in PubMed for studies published until October 2025. Keywords included "keratoconus", "biometry", "IOL power calculation", "cataract surgery", "keratometry", and related terms. Studies evaluating the repeatability of biometric measurement, the accuracy of IOL formulas, and surgical outcomes in keratoconus patients were included.
