ABSTRACT: Background
Cardiogenic shock (CS) due to papillary muscle rupture causing acute mitral regurgitation (MR) after acute coronary syndrome carries poor prognosis and frequently necessitates urgent MR reduction.Case summary
A 70-year-old man presented in CS due to papillary muscle rupture at a hospital without cardiac surgery or expertise in structural heart interventions. He was deemed at high risk for transfer by the heart team, and a percutaneous treatment approach was chosen. A mobile mitral transcatheter edge-to-edge repair (m-TEER) team was dispatched from a tertiary center, and within hours the patient underwent coronary intervention and successful m-TEER, leading to marked hemodynamic and clinical improvement.Discussion
