Single coronary artery: a familial clustering.

ABSTRACT: A 59 year old man undergoing investigation for chest pain was found at elective coronary angiography to have a single coronary artery; the left coronary had a normal distribution, with the right coronary originating as a continuation of the atrioventricular circumflex. His 30 year old daughter was admitted for elective coronary angiography for further investigation of a dilated cardiomyopathy. She was also found to have a single coronary artery. However, in her case, the right and left coronary arteries arose from the right sinus of Valsalva; the right coronary had a normal distribution, the left coronary passed anterior to the pulmonary trunk and aorta.

Unsupervised clustering based coronary artery segmentation.

Project description:BackgroundThe acquisition of 3D geometries of coronary arteries from computed tomography coronary angiography (CTCA) is crucial for clinicians, enabling visualization of lesions and supporting decision-making processes. Manual segmentation of coronary arteries is time-consuming and prone to errors. There is growing interest in automatic segmentation algorithms, particularly those based on neural networks, which require large datasets and significant computational resources for training. This paper proposes an automatic segmentation methodology based on clustering algorithms and a graph structure, which integrates data from both the clustering process and the original images.ResultsThe study compares two approaches: a 2.5D version using axial, sagittal, and coronal slices (3Axis), and a perpendicular version (Perp), which uses the cross-section of each vessel. The methodology was tested on two patient groups: a test set of 10 patients and an additional set of 22 patients with clinically diagnosed lesions. The 3Axis method achieved a Dice score of 0.88 in the test set and 0.83 in the lesion set, while the Perp method obtained Dice scores of 0.81 in the test set and 0.82 in the lesion set, decreasing to 0.79 and 0.80 in the lesion region, respectively. These results are competitive with current state-of-the-art methods.ConclusionsThis clustering-based segmentation approach offers a robust framework that can be easily integrated into clinical workflows, improving both accuracy and efficiency in coronary artery analysis. Additionally, the ability to visualize clusters and graphs from any cross-section enhances the method's explainability, providing clinicians with deeper insights into vascular structures. The study demonstrates the potential of clustering algorithms for improving segmentation performance in coronary artery imaging.

Coronary artery calcium among patients with heterozygous familial hypercholesterolaemia.

Project description:AimsWe aimed to determine if coronary artery calcium (CAC) is associated with cardiovascular disease (CVD) events, defined as CVD-related death, unstable angina, myocardial infarction, or staged revascularization among patients with heterozygous familial hypercholesterolaemia (HeFH) under primary prevention settings.Methods and resultsData of patients with FH admitted to Kanazawa University Hospital between 2000 and 2020, who underwent CAC measurement and were followed up (n = 622, male = 306, mean age = 54 years), were retrospectively reviewed. Risk factors for CVD events were determined using the Cox proportional hazard model. The median follow-up duration was 13.2 years (interquartile range: 9.8-18.4 years). We observed 132 CVD events during the follow-up period. The event rate per 1000 person-years for CAC scores of 0 [n = 283 (45.5%)], 1-100 [n = 260 (41.8%)], and >100 [n = 79 (12.7%)] was 1.2, 17.0, and 78.8, respectively. Log (CAC score + 1) was a significant predictor of the occurrence of CVD events (hazard ratio: 3.24; 95% confidence interval: 1.68-4.80; P < 0.0001) in the multivariate Cox regression analysis, independent of other factors. The risk discrimination of CVD events was enhanced by adding CAC information to other conventional risk factors (C-statistics: 0.833-0.934; P < 0.0001).ConclusionThe CAC score helps in further risk stratification in patients with HeFH.

Giant coronary artery aneurysm associated with familial retinal artery macroaneurysm: a case report.

Project description:BackgroundThe term 'Giant' coronary aneurysm is usually defined as any coronary aneurysm more than 8 mm in maximum diameter. The form of familial retinal arterial macroaneurysms (FRAMs) is a rare autosomal recessive disease that is described by arterial aneurysm formation in the retina. Here, we report an association of coronary artery aneurysms with FRAM in a young male who presented with the acute coronary syndrome.Case summaryA 31-year-old male smoker presented to the emergency department with atypical chest pain for 5 days. Blood investigations showed raised troponin enzymes. Review of his past medical history revealed decreased vision in the left eye, starting at the age of 10 years which progressed to blindness. He was diagnosed to have IGFBP7 mutation which causes eye manifestations in the form of FRAM. Fundoscopy showed bleeding retinal artery macroaneurysms in the right retina and sub-retinal gliosis suggesting laser treatment for the prior retinal arterial aneurysm. Coronary angiogram revealed a large aneurysm in the proximal segment of the left anterior descending (LAD) artery. Cardiac computed tomography scan with contrast was done which showed a 2.28 × 1.64 cm coronary aneurysm at the proximal segment of the LAD artery with peripheral calcification with a narrow neck of about 0.6 cm. After the heart team discussion, he underwent surgical ligation of the LAD and coronary aneurysm with implantation of the saphenous venous graft to distal LAD.DiscussionCoronary artery aneurysms can be a part of multisystem diseases like FRAM. The management should be individualized based on symptoms at presentation, size of coronary aneurysms, and local expertise.

Familial hypercholesterolemia and vulnerability of coronary plaque in patients with coronary artery disease.

Project description:ObjectivesPatients with familial hypercholesterolemia (FH) are at a very high risk of coronary artery diseases. The aim of the present study was to clarify the characteristics of coronary plaque in patients with FH.Designand Methods: A total of 569 patients who underwent optical coherence tomography (OCT) imaging of culprit plaque were included. The characteristics of culprit plaque were compared between patients with and without FH.ResultsA total of 38 patients (6.7%) were clinically diagnosed with FH. The location of the culprit plaque was significantly different (p < 0.001) with a trend toward higher frequency of left main lesion in the FH group than in the group with no FH (7.9 vs. 0%). Culprit plaque was significantly shorter in patients with FH than those without FH (28.1 vs. 33.2 mm, p = 0.016). A trend toward higher prevalence of plaque with macrophage accumulation in patients with FH than those without FH (50.0 vs. 34.7%, p = 0.056) was observed, although the prevalence of other vulnerable characteristics including thin-cap fibroatheroma (TCFA) was comparable between patients with and without FH. Among patients with FH, significant increases in the prevalence of lipid-rich plaque (p = 0.028) and TCFA (p = 0.003) were observed according to the increase in low-density lipoprotein cholesterol (LDL-C) levels.ConclusionsPatients with FH had shorter culprit plaque without significant difference in the prevalence of vulnerable plaque components compared with patients without FH. A higher LDL-C level was associated with higher prevalence of vulnerable plaque in patients with FH.

GATA2 is associated with familial early-onset coronary artery disease.

Project description:The transcription factor GATA2 plays an essential role in the establishment and maintenance of adult hematopoiesis. It is expressed in hematopoietic stem cells, as well as the cells that make up the aortic vasculature, namely aortic endothelial cells and smooth muscle cells. We have shown that GATA2 expression is predictive of location within the thoracic aorta; location is suggested to be a surrogate for disease susceptibility. The GATA2 gene maps beneath the Chromosome 3q linkage peak from our family-based sample set (GENECARD) study of early-onset coronary artery disease. Given these observations, we investigated the relationship of several known and novel polymorphisms within GATA2 to coronary artery disease. We identified five single nucleotide polymorphisms that were significantly associated with early-onset coronary artery disease in GENECARD. These results were validated by identifying significant association of two of these single nucleotide polymorphisms in an independent case-control sample set that was phenotypically similar to the GENECARD families. These observations identify GATA2 as a novel susceptibility gene for coronary artery disease and suggest that the study of this transcription factor and its downstream targets may uncover a regulatory network important for coronary artery disease inheritance.

Project description:Atresia of the right coronary artery ostium is a rare anatomic variant of the coronary circulation. It is often difficult to differentiate from single coronary artery. Its presence unassociated with any other anomaly has never been described in an adult individual. We report this unusual anomaly and discuss its anatomical and pathophysiological significance and possible ways to differentiate from single coronary artery.
