ABSTRACT: Osteoarthritis (OA) is the most common form of arthritis in the United States and is a leading cause of disability. It is typically defined in epidemiologic studies by radiographic findings and consideration of symptoms. Its incidence and prevalence are rising, likely related to the aging of the population and increasing obesity. Risk factors for OA include numerous person-level factors, such as age, sex, obesity, and genetics, as well as joint-specific factors that are likely reflective of abnormal loading of the joints. In studying OA, several methodologic challenges exist that can hamper our ability to identify pertinent relationships.

Project description:ObjectivesThe present study investigated (1) trends in the prevalence and incidence of knee osteoarthritis over a 20-year period (1996-2015); (2) trends in multimorbidity and (3) trends in drug prescriptions.DesignRegistry-based study.SettingPrimary healthcare, Flanders, Belgium.ParticipantsData were collected from Intego, a general practice-based morbidity registration network. In the study period between 1996 and 2015, data from 440 140 unique patients were available.Outcome measuresTrends in prevalence and incidence rate of knee osteoarthritis were computed using joinpoint regression analysis. The mean disease count was calculated to assess trends in multimorbidity. In addition, the number of drug prescriptions was identified by the Anatomical Therapeutic Chemical Classification code and trends were equally recorded with joinpoint regression.ResultsThe total age-standardised prevalence of knee osteoarthritis increased from 2.0% in 1996 to 3.6% in 2015. An upward trend was observed with an average annual percentage change (AAPC) of 2.5 (95% CI 2.2 to 2.9). In 2015, the prevalence rates in the 10 year age groups from the 45-54 years age group onwards were 3.1%, 5.6%, 9.0% and 13.9%, to reach 15.0% in people aged 85 years and older. The incidence remained stable with 3.75‰ in 2015 (AAPC=-0.5, 95% CI -1.4 to 0.5). The mean disease count significantly increased from 1.63 to 2.34 (p<0.001) for incident cases with knee osteoarthritis. Finally, we observed a significantly positive trend in the overall prescription of acetaminophen (AAPC=6.7, 95% CI 5.6 to 7.7), weak opioids (AAPC=4.0, 95% CI 0.9 to 7.3) and glucosamine (AAPC=8.6, 95% CI 2.4 to 15.1). Oral non-steroidal anti-inflammatory drugs were most prescribed, with a prevalence rate of 29.8% in 2015, but remained stable during the study period (AAPC=0.0, 95% CI -1.1 to 1.1).ConclusionsIncreased prevalence, multimorbidity, and number of drug prescriptions confirm an increased burden of knee osteoarthritis. In future, these trends can be used to prioritise initiatives for improvement in care.

Project description:Osteoarthritis (OA) is a chronic degenerative joint disease with an increasing global burden, particularly among the working-age population. This study aims to analyze the temporal trends in OA burden by age and sex globally from 1990 to 2021, focusing on incidence, prevalence, and disability-adjusted life years (DALYs) rates, and to predict future trends. Using data from the Global Burden of Disease (GBD) database, which includes 204 countries and regions, we stratified the findings by the sociodemographic index (SDI). Age-standardized rates were used to calculate the estimated annual percentage change (EAPC) with corresponding 95% confidence intervals (95% CI). Additionally, a Bayesian Age-Period-Cohort (BAPC) model was employed to project future OA trends up to 2040.The results revealed a consistent increase in the global OA burden over the study period. Between 1990 and 2021, the number of OA cases in the working-age population increased from 16,420,160 to 35,494,218, representing a growth rate of 116.16%. Over the same period, prevalence and DALYs rose by 123.11% and 125%, respectively. Global age-standardized incidence rate (ASIR), prevalence rate (ASPR), and DALYs rate (ASDR) exhibited continuous upward trends, with annual percentage changes of 0.387%, 0.431%, and 0.46%, respectively. Notably, East Asia demonstrated the highest EAPC, reflecting a rapid rise in OA burden, while high-income North America exhibited minimal changes, indicating a relatively stable trend. Countries such as Equatorial Guinea, Mongolia, and Armenia also experienced significant increases in EAPC, underscoring emerging regional challenges.Further socioeconomic analysis highlighted disparities in OA burden. A significant positive correlation was observed between ASIR, ASPR, ASDR, and SDI. While low-SDI countries exhibited lower OA burdens, metrics were substantially higher in high-SDI countries. From 1990 to 2021, the gap between countries with the highest and lowest SDIs widened, underscoring growing global health inequalities. Projections based on the BAPC model suggest that by 2040, the incidence and prevalence of OA will continue to rise, with the number of cases expected to reach 38,800,395, particularly driven by notable increases among women.These findings highlight the urgent need for developing targeted public health strategies to mitigate the effect of OA on the working-age population and promote global health equity.
