Bayesian semiparametric estimation of covariate-dependent ROC curves.

ABSTRACT: Receiver operating characteristic (ROC) curves are widely used to measure the discriminating power of medical tests and other classification procedures. In many practical applications, the performance of these procedures can depend on covariates such as age, naturally leading to a collection of curves associated with different covariate levels. This paper develops a Bayesian heteroscedastic semiparametric regression model and applies it to the estimation of covariate-dependent ROC curves. More specifically, our approach uses Gaussian process priors to model the conditional mean and conditional variance of the biomarker of interest for each of the populations under study. The model is illustrated through an application to the evaluation of prostate-specific antigen for the diagnosis of prostate cancer, which contrasts the performance of our model against alternative models.

Semiparametric estimation of the covariate-specific ROC curve in presence of ignorable verification bias.

Project description:Covariate-specific receiver operating characteristic (ROC) curves are often used to evaluate the classification accuracy of a medical diagnostic test or a biomarker, when the accuracy of the test is associated with certain covariates. In many large-scale screening tests, the gold standard is subject to missingness due to high cost or harmfulness to the patient. In this article, we propose a semiparametric estimation of the covariate-specific ROC curves with a partial missing gold standard. A location-scale model is constructed for the test result to model the covariates' effect, but the residual distributions are left unspecified. Thus the baseline and link functions of the ROC curve both have flexible shapes. With the gold standard missing at random (MAR) assumption, we consider weighted estimating equations for the location-scale parameters, and weighted kernel estimating equations for the residual distributions. Three ROC curve estimators are proposed and compared, namely, imputation-based, inverse probability weighted, and doubly robust estimators. We derive the asymptotic normality of the estimated ROC curve, as well as the analytical form of the standard error estimator. The proposed method is motivated and applied to the data in an Alzheimer's disease research.

Estimation of multiple ordered ROC curves using placement values.

Project description:In many diagnostic accuracy studies, a priori orders may be available on multiple receiver operating characteristic curves. For example, being closer to delivery, fetal ultrasound measures in the third trimester should be no less accurate than those in the second trimester in predicting small-for-gestational-age births. Such an a priori order should be incorporated in estimating receiver operating characteristic curves and associated summary accuracy statistics, as it can potentially improve statistical efficiency of these estimates. Early work in the literature has mainly taken an indirect approach to this task and has induced the desired a priori order through modeling test score distributions. We instead propose a new strategy that incorporates the order directly through the modeling of receiver operating characteristic curves. We achieve this by exploiting the link between placement value (the relative position of a diseased test score in the healthy score distribution), the cumulative distribution function of placement value, and receiver operating characteristic curve, and by building stochastically ordered random variables through mixture distributions. We take a Bayesian semiparametric approach in using Dirichlet process mixture models so that the placement values can be flexibly modeled. We conduct extensive simulation studies to examine the performance of the proposed methodology and apply the new framework to data from obstetrics and women's health studies.

Semiparametric estimation exploiting covariate independence in two-phase randomized trials.

Project description:Recent results for case-control sampling suggest when the covariate distribution is constrained by gene-environment independence, semiparametric estimation exploiting such independence yields a great deal of efficiency gain. We consider the efficient estimation of the treatment-biomarker interaction in two-phase sampling nested within randomized clinical trials, incorporating the independence between a randomized treatment and the baseline markers. We develop a Newton-Raphson algorithm based on the profile likelihood to compute the semiparametric maximum likelihood estimate (SPMLE). Our algorithm accommodates both continuous phase-one outcomes and continuous phase-two biomarkers. The profile information matrix is computed explicitly via numerical differentiation. In certain situations where computing the SPMLE is slow, we propose a maximum estimated likelihood estimator (MELE), which is also capable of incorporating the covariate independence. This estimated likelihood approach uses a one-step empirical covariate distribution, thus is straightforward to maximize. It offers a closed-form variance estimate with limited increase in variance relative to the fully efficient SPMLE. Our results suggest exploiting the covariate independence in two-phase sampling increases the efficiency substantially, particularly for estimating treatment-biomarker interactions.

Estimation of smooth ROC curves for biomarkers with limits of detection.

Project description:Protein biomarkers found in plasma are commonly used for cancer screening and early detection. Measurements obtained by such markers are often based on different assays that may not support detection of accurate measurements due to a limit of detection. The ROC curve is the most popular statistical tool for the evaluation of a continuous biomarker. However, in situations where limits of detection exist, the empirical ROC curve fails to provide a valid estimate for the whole spectrum of the false positive rate (FPR). Hence, crucial information regarding the performance of the marker in high sensitivity and/or high specificity values is not revealed. In this paper, we address this problem and propose methods for constructing ROC curve estimates for all possible FPR values. We explore flexible parametric methods, transformations to normality, and robust kernel-based and spline-based approaches. We evaluate our methods though simulations and illustrate them in colorectal and pancreatic cancer data.

Semiparametric modelling and estimation of covariate-adjusted dependence between bivariate recurrent events.

Project description:A time-dependent measure, termed the rate ratio, was proposed to assess the local dependence between two types of recurrent event processes in one-sample settings. However, the one-sample work does not consider modeling the dependence by covariates such as subject characteristics and treatments received. The focus of this paper is to understand how and in what magnitude the covariates influence the dependence strength for bivariate recurrent events. We propose the covariate-adjusted rate ratio, a measure of covariate-adjusted dependence. We propose a semiparametric regression model for jointly modeling the frequency and dependence of bivariate recurrent events: the first level is a proportional rates model for the marginal rates and the second level is a proportional rate ratio model for the dependence structure. We develop a pseudo-partial likelihood to estimate the parameters in the proportional rate ratio model. We establish the asymptotic properties of the estimators and evaluate the finite sample performance via simulation studies. We illustrate the proposed models and methods using a soft tissue sarcoma study that examines the effects of initial treatments on the marginal frequencies of local/distant sarcoma recurrence and the dependence structure between the two types of cancer recurrence.

Bayesian semiparametric copula estimation with application to psychiatric genetics.

Project description:This paper proposes a semiparametric methodology for modeling multivariate and conditional distributions. We first build a multivariate distribution whose dependence structure is induced by a Gaussian copula and whose marginal distributions are estimated nonparametrically via mixtures of B-spline densities. The conditional distribution of a given variable is obtained in closed form from this multivariate distribution. We take a Bayesian approach, using Markov chain Monte Carlo methods for inference. We study the frequentist properties of the proposed methodology via simulation and apply the method to estimation of conditional densities of summary statistics, used for computing conditional local false discovery rates, from genetic association studies of schizophrenia and cardiovascular disease risk factors.

ROC curve estimation under test-result-dependent sampling.

Project description:The receiver operating characteristic (ROC) curve is often used to evaluate the performance of a biomarker measured on continuous scale to predict the disease status or a clinical condition. Motivated by the need for novel study designs with better estimation efficiency and reduced study cost, we consider a biased sampling scheme that consists of a SRC and a supplemental TDC. Using this approach, investigators can oversample or undersample subjects falling into certain regions of the biomarker measure, yielding improved precision for the estimation of the ROC curve with a fixed sample size. Test-result-dependent sampling will introduce bias in estimating the predictive accuracy of the biomarker if standard ROC estimation methods are used. In this article, we discuss three approaches for analyzing data of a test-result-dependent structure with a special focus on the empirical likelihood method. We establish asymptotic properties of the empirical likelihood estimators for covariate-specific ROC curves and covariate-independent ROC curves and give their corresponding variance estimators. Simulation studies show that the empirical likelihood method yields good properties and is more efficient than alternative methods. Recommendations on number of regions, cutoff points, and subject allocation is made based on the simulation results. The proposed methods are illustrated with a data example based on an ongoing lung cancer clinical trial.

Hierarchical Bayesian estimation of covariate effects on airway and alveolar nitric oxide.

Project description:Exhaled breath biomarkers are an important emerging field. The fractional concentration of exhaled nitric oxide (FeNO) is a marker of airway inflammation with clinical and epidemiological applications (e.g., air pollution health effects studies). Systems of differential equations describe FeNO-measured non-invasively at the mouth-as a function of exhalation flow rate and parameters representing airway and alveolar sources of NO in the airway. Traditionally, NO parameters have been estimated separately for each study participant (Stage I) and then related to covariates (Stage II). Statistical properties of these two-step approaches have not been investigated. In simulation studies, we evaluated finite sample properties of existing two-step methods as well as a novel Unified Hierarchical Bayesian (U-HB) model. The U-HB is a one-step estimation method developed with the goal of properly propagating uncertainty as well as increasing power and reducing type I error for estimating associations of covariates with NO parameters. We demonstrated the U-HB method in an analysis of data from the southern California Children's Health Study relating traffic-related air pollution exposure to airway and alveolar airway inflammation.

plotROC: A Tool for Plotting ROC Curves.

Project description:Plots of the receiver operating characteristic (ROC) curve are ubiquitous in medical research. Designed to simultaneously display the operating characteristics at every possible value of a continuous diagnostic test, ROC curves are used in oncology to evaluate screening, diagnostic, prognostic and predictive biomarkers. I reviewed a sample of ROC curve plots from the major oncology journals in order to assess current trends in usage and design elements. My review suggests that ROC curve plots are often ineffective as statistical charts and that poor design obscures the relevant information the chart is intended to display. I describe my new R package that was created to address the shortcomings of existing tools. The package has functions to create informative ROC curve plots, with sensible defaults and a simple interface, for use in print or as an interactive web-based plot. A web application was developed to reach a broader audience of scientists who do not use R.

A Bayesian Sample Size Estimation Procedure Based on a B-Splines Semiparametric Elicitation Method.

Project description:Sample size estimation is a fundamental element of a clinical trial, and a binomial experiment is the most common situation faced in clinical trial design. A Bayesian method to determine sample size is an alternative solution to a frequentist design, especially for studies conducted on small sample sizes. The Bayesian approach uses the available knowledge, which is translated into a prior distribution, instead of a point estimate, to perform the final inference. This procedure takes the uncertainty in data prediction entirely into account. When objective data, historical information, and literature data are not available, it may be indispensable to use expert opinion to derive the prior distribution by performing an elicitation process. Expert elicitation is the process of translating expert opinion into a prior probability distribution. We investigated the estimation of a binomial sample size providing a generalized version of the average length, coverage criteria, and worst outcome criterion. The original method was proposed by Joseph and is defined in a parametric framework based on a Beta-Binomial model. We propose a more flexible approach for binary data sample size estimation in this theoretical setting by considering parametric approaches (Beta priors) and semiparametric priors based on B-splines.
