Family violence among older adult patients consulting in primary care clinics: results from the ESA (Enquete sur la sante des aines) services study on mental health and aging.

ABSTRACT:

Objective

To document the reliability and construct validity of the Family Violence Scale (FVS) in the older adult population aged 65 years and older.

Method

Data came from a cross-sectional survey, the Enquête sur la santé des aînés et l'utilisation des services de santé (ESA Services Study), conducted in 2011-2013 using a probabilistic sample of older adults waiting for medical services in primary care clinics (n = 1765). Family violence was defined as a latent variable, coming from a spouse and from children.

Results

A model with 2 indicators of violence; that is, psychological and financial violence, and physical violence, adequately fitted the observed data. The reliability of the FVS was 0.95. According to our results, 16% of older adults reported experiencing some form of family violence in the past 12 months of their interview, and 3% reported a high level of family violence (FVS > 0.36). Our results showed that the victim's sex was not associated with the degree of violence (β = 0.02). However, the victim's age was associated with family violence (β = -0.12). Older adults, aged 75 years and older, reported less violence than those aged between 65 and 74 years.

Conclusion

Our results lead us to conclude that family violence against older adults is common and warrants greater public health and political attention. General practitioners could play an active role in the detection of violence among older adults.

Project description:ObjectiveYouth experiencing homelessness represent a major social problem in Canada and, as demonstrated in the first national survey of this population conducted in 2015, are experiencing significant mental health challenges. The present study examines the findings of a second national survey completed in 2019. These findings afford the opportunity to examine the reliability of the findings of the first study with another large, representative sample and to attempt to articulate the unique characteristics of youth experiencing the greatest distress among this at-risk population.MethodsThis study analyzed the mental-health-related data from the 2019 Without a Home-National Youth Homelessness Survey that was administered through convenience sampling at 98 agencies serving homeless youth in 49 communities across Canada. The survey was cross-sectional and self-administered, assessing a range of demographic information, pre- and post-homelessness variables, and mental health indicators. Multinomial logistic regression and linear regression were implemented to evaluate associations with distress level.ResultsSurvey data were obtained from 1,375 youth accessing Canadian homeless services in 9 provinces. Thirty-five percent reported at least 1 suicide attempt, and 33.1% reported a drug overdose requiring hospitalization. The findings of this survey replicated most of the key findings from the 2015 survey. The current findings emphasized, for this high-risk population, the heightened adversity faced by young women, Lesbian, Gay, Bisexual, Transgender, Queer, Two-Spirit (LGBTQ2S), and Indigenous subpopulations, as well as the centrality of violence exposure in determining risk and distress. Sexual violence, in particular, emerged as a key factor in the identification of youth experiencing the greatest distress with risk buffered by contact with family.ConclusionsThese findings can inform prevention and intervention policies and services and reinforce the importance of attending to violence exposure and trauma as central to the mental health trajectories of youth who have experienced homelessness.

Project description:Introduction La pandémie du coronavirus a bouleversé les pratiques médicales avec une rupture de la continuité des soins et un retard diagnostic en oncologie avec des diagnostics à des stades plus avancés donc de plus mauvais pronostic. Nous nous sommes intéressés à l’impact de ce retard pour les mélanomes invasifs de stade précoce (stades I et II 8e classification AJCC). Matériel et méthodes Nous avons réalisé une étude rétrospective en région Rhône-Alpes, basée sur des données anatomopathologiques de Cypath-Dermapath (réseau de dermatopathologie privé drainant une large population hospitalière et libérale). Tous les nouveaux mélanomes cutanés primitifs invasifs diagnostiqués entre le 28 août 2019 et le 3 octobre 2020 étaient inclus. Les données analysées étaient cliniques : âge, sexe, topographie (tête/cou, membres supérieurs, membres inférieurs, tronc), et histologiques : épaisseur en mm, présence d’une ulcération, sous-type de mélanome (mélanome à extension superficielle, malin lentigineux, nodulaire, acrolentigineux, ou autre). Nous avons comparé les données sur 200 jours : pré-COVID-19 (28 août 2019 au 16 mars 2020) et COVID-19 (17 mars 2020 au 3 octobre 2020, le 17 mars 2020 date du premier confinement. Résultats Huit cent quatre vingt dix neuf mélanomes ont été inclus, 491 avant le confinement et 408 après. Le nombre de mélanome diagnostiqué par jour diminuait de 17 % après le confinement, surtout pendant le premier confinement du 17 mars 2020 au 12 mai 2020. Respectivement avant et après le confinement, l’âge moyen était de 62,3 ans et le Breslow moyen de 1,125 mm puis respectivement 64,7 ans et 1,194 mm. Les mélanomes ulcérés étaient plus fréquents après le confinement (11,8 %) qu’avant (7,5 %), p-value = 0,0311. Il n’était pas observé de différence significative entre les 2 périodes pour l’âge moyen, le sex-ratio, la topographie, l’épaisseur tumorale et le stade AJCC. L’analyse par classe d’âge montrait une tendance pour les plus jeunes, 18–44 ans, de mélanome plus épais après le confinement (0,78 mm contre 1,07 mm, p = 0,13) et plus souvent ulcéré (5,6 % contre 13,6 %, p = 0,15), avec plus de stade II AJCC après confinement (7,4 % contre 18,6 %, p = 0,099). Les autres classes d’âge, 45-65 et plus de 65 ans, ne montraient pas de différence significative. Discussion Notre étude ne montre pas un effet majeur de la pandémie à COVID-19 sur la sévérité des mélanomes diagnostiqués après le confinement. L’impact le plus notable apparaît chez les patients les plus jeunes, 18–44 ans, avec plus de stade II AJCC après le confinement qu’avant. Une étude sur une période plus longue permettra de préciser ces résultats.

Project description:Introduction Fin décembre 2020, les vaccins anti-Covid-19 à ARNm Comirnaty© de Pfizer-BioNTech et Moderna© de Moderna Biotech ont obtenu l’AMM conditionnelle en France. Ces vaccins étaient contre-indiqués chez les personnes ayant présenté des manifestations allergiques graves telles que des réactions anaphylactiques par l’HAS, le ministère de la Santé, l’Assurance maladie et l’ANSM. Nous avons vacciné avec Comirnaty© sous surveillance allergologique rapprochée les patients supposés à risque allergique et évalué la tolérance de cette vaccination. Matériel et méthodes Entre le 20 janvier et le 8 avril 2021, les patients étaient adressés via la hot line téléphonique mise en place spécialement pour les avis vis-à-vis de la vaccination pour les centres de vaccination d’Ile de France, médecins traitants ou allergologues. Jusqu’au 12/03/21, la vaccination était réalisée avec voie d’abord veineux, arrêt des bêtabloquants, surveillance des constantes toutes les 30 min et totale de 2 h. Le protocole a ensuite été allégé, sans voie veineuse et surveillance de 1 h. Résultats Un total de 225 patients supposés à haut risque allergique ont été vaccinés (1re dose), 48 hommes et 177 femmes, avec un âge moyen de 73 ans. Quatre-vingt-dix-huit avaient un terrain atopique (43,6 %). Deux cent trois patients avaient des antécédents allergiques suspectés ou prouvés à type d’hypersensibilité immédiate (90,2 %), seize d’hypersensibilité retardée (7,1 %), deux avaient une mastocytose systémique indolente sans réaction médicamenteuse spécifique et pour quatre la réaction n’était pas connue. Les molécules en cause étaient en majorité les produits de contraste (32,4 %) ou les antibiotiques (27,5 %). Trois réactions post vaccinales ont été notées (patients 1 à 3) sans anaphylaxie de grade II ou plus. Il faut noter que les réactions urticariennes étaient toutes retardées de plus d’1 heure ce qui est peu évocateur d’anaphylaxie IgE médiée. Parmi les 225 premières doses, 217 patients ont eu une deuxième dose (dont 75 dans le service), 7 n’étaient pas éligibles ayant eu une infection Covid-19 et 1 patient a été perdu de vue. Deux patients ont présenté une réaction (patients 4 et 5). Aucune anaphylaxie n’a été rapportée. Par ailleurs la hotline a permis d’autoriser la vaccination en centre de vaccination d’environ 2000 patients. Discussion Malgré le petit effectif, principalement lié à une pénurie de vaccin dans la même période et des capacités d’accueil limitées, nous n’avons pas observé de réactions anaphylactiques chez les patients étiquetés « à haut risque allergique ». Cette étude nous a permis de modifier nos pratiques et restreindre les indications de vaccination sous surveillance « allergologique » et cette mobilisation d’équipe a été d’une grande aide sanitaire pour vacciner des patients récusés par les centres de vaccination.

Project description:Rationale: Learning styles (LS) of medical residents change over time from the more passive Assimilator toward more active preferences like Accommodator or Converger. We have shown pharmacy residents to be predominantly Assimilators at the start of their residency program. Whether learning styles of pharmacy residents change after they enter practice has never been studied. Objective: To describe the evolution of learning styles of pharmacy residents as they transition from residency into practice. Study Design and Methods: Prospective observational survey and semi-structured interviews. A complete provincial cohort of former pharmacy residents (n=28) who had their LS characterized during their residency and were now 1 year post-residency were invited to repeat the Pharmacists’ Inventory of Learning Styles (PILS). Interviews were administered to consenting participants to gain additional insights. Results: 27 residents (96%) completed the PILS survey and 16 (59%) completed the interview. 13 (48%) changed their dominant LS and 20 (74%) changed their secondary LS. Six (22%) participants did not change either LS. The overall proportion of dominant Assimilators (59%) and Convergers (26%) remained similar to baseline (52% and 26%, respectively), meaning participants had adopted and abandoned different learning styles in similar numbers. Change in LS was associated with receiving preceptor training (p<0.05). Of the 12 preceptors interviewed, 58% stated that they had adjusted their teaching practices based on knowledge of their LS. Conclusions: Change in dominant and/or secondary learning style is common after 1 year in practice compared to during pharmacy practice residency. There is no overall direction to the shifts, however, with residents transitioning in and out of more active learning styles with similar frequency. Overall after a year in practice, the cohort of former residents we studied remained mostly Assimilators, who prefer passive learning approaches. These results contrast with medical students, who adopt more active preferences like Accommodator.
