ABSTRACT: Scimitar syndrome is a rare congenital anomaly where the right pulmonary veins return to the inferior vena cava (IVC) just below the diaphragm. On chest X-ray (CXR), an IVC catheter will be in a bizarre location outside the heart if it inadvertently passes into the scimitar vein rather than into the right atrium.

Real-time ultrasound for tip location of umbilical venous catheter in neonates: a pre/post intervention study.

Project description:BackgroundRecent guidelines advocate the use of real-time ultrasound to locate umbilical venous catheter tip. So far, training programs are not well established.MethodsA pre/post interventional study was carried out in our tertiary neonatal intensive care unit centre to evaluate the efficacy of a training protocol in the use of real-time ultrasound. Primary outcome was the percentage in the use of real-time ultrasound.ResultsFifty-four patients were enrolled. The use of real-time ultrasound for tip location significantly increased after the training program (15.3% vs 89.2%, p < 0.0001). After the training the tip of the catheters was more frequently placed at the junction of the inferior vena cava and right atrium (75% vs 30.7%, p = 0.0023). Twenty-two catheters were also evaluated with serial scans during the intervention phase to assess migration rate which was 50%.Conclusiona multimodal, targeted training on the use of real-time ultrasound for umbilical venous catheter placement is feasible. Real-time ultrasound is easily teachable, increases the number of umbilical venous catheters placed in a correct position, reduces the number of line manipulations and the need of chest-x-rays.

Pleural and Pericardiac Effusion as a Complication of Properly Placed Umbilical Venous Catheter.

Project description:Pleural and pericardial effusions are extremely rare complications of umbilical venous catheterization in newborns. A preterm male infant weighing 850g, with insertion of an umbilical venous catheter (UVC) developed massive right pleural and pericardial effusions. The position of catheter tip was verified by chest radiography and echocardiography. The effusions were drained by thoracentesis and pericardiocentesis without complication, and were biochemically similar as total parenteral infusion which infused through catheter.

Low cardiac output secondary to a malpositioned umbilical venous catheter: value of targeted neonatal echocardiography.

Project description:Systemic hypotension is common in very low birthweight preterm infants but the nature of the precipitating cause may be unclear. Targeted neonatal echocardiography (TnEcho) is being increasingly used to support hemodynamic decisions in the neonatal intensive care unit (NICU), including identifying impairments in the transitional circulation of preterm infants, providing timely re-evaluation after institution of therapies and evaluating the placement of indwelling catheters. We present a case of a preterm infant with systemic hypotension and low cardiac output secondary to a large transatrial shunt induced by a malpositioned umbilical venous catheter. Repositioning of the line led to resolution of the hemodynamic disturbance and clinical instability, highlighting the utility of TnEcho in the NICU.

Successful retrieval of deep intracardiac migrated broken umbilical venous catheter in a preterm infant: Case report.

Project description:Umbilical catheters serve as indispensable tools in the realm of neonatal intensive care, contributing significantly to the well-being of premature infants. While rare, it is essential to approach their handling with utmost caution, as it can lead to fatal complications. We report a case of a preterm 9-day-old male infant, who was referred to our center for specialized treatment following an unsuccessful surgical attempt to address a fractured umbilical venous catheter (UVC). This case underscores the value of employing imaging techniques for prompt identification of such complications. Furthermore, the utilization of endovascular therapy emerges as a promising intervention in managing such complexities, thereby expanding the horizons of interventional radiology in elevating the standard of patient care.

Infantile scimitar syndrome with unusual associations.

Project description:Scimitar syndrome is a variant of partial anomalous pulmonary venous connection (PAPVC), in which all or part of the right lung is drained by right pulmonary veins that anomalously connect to the inferior vena cava (IVC). The affected lung and its associated airways are often hypoplastic. In addition, aortopulmonary collateral vessels may be involved on the affected side, causing sequestration of that side; such involvement is commonly associated with cardiac defects. We report a case of infantile scimitar syndrome that involved a typical association with the right lung, but with extremely unusual associations with congenital hydrocephalus and heart blockage. The presentation of this case and the role of different diagnostic approaches and management are discussed.

Total transcatheter correction of scimitar variant having dual pulmonary venous drainage.

Project description:Scimitar syndrome is a clinical triad of anomalous pulmonary venous drainage, lung hypoplasia, and anomalous aortic blood supply to the lung segment. When there is dual pulmonary venous drainage both to inferior vena cava and left atrium, it is called scimitar variant. A young child presenting with recurrent chest infections, dextroposition of the heart, and scimitar shadow on chest X-ray was identified to have scimitar variant after a detailed evaluation and managed successfully by catheter interventions. This report discusses the embryogenesis and angiographic findings of scimitar variants, indications for interventions, and issues in its management.
