Autochthonous dengue fever, Tokyo, Japan, 2014.

ABSTRACT: After 70 years with no confirmed autochthonous cases of dengue fever in Japan, 19 cases were reported during August-September 2014. Dengue virus serotype 1 was detected in 18 patients. Phylogenetic analysis of the envelope protein genome sequence from 3 patients revealed 100% identity with the strain from the first patient (2014) in Japan.

Dengue Virus Infection in Aedes albopictus during the 2014 Autochthonous Dengue Outbreak in Tokyo Metropolis, Japan.

Project description:In 2014 in Japan, 162 autochthonous dengue cases were reported for the first time in nearly 70 years. Here, we report the results of the detection and isolation of dengue virus (DENV) from mosquitoes collected in Tokyo Metropolis in 2014 and 2015. The phylogenetic relationship among DENV isolates from mosquitoes and from patients based on both the entire envelope gene and whole coding sequences was evaluated. Herein, 2,298 female and 956 male Aedes albopictus mosquitoes were collected at six suspected locations of DENV infection in Tokyo Metropolis from August to October in 2014 and grouped into 124 and 35 pools, respectively, for viral genome detection and DENV isolation. Dengue virus RNA was detected using reverse transcription polymerase chain reaction and TaqMan assays from 49 female pools; 16 isolates were obtained using C6/36 and Vero cells. High minimum infection rates (11.2-66.7) persisted until mid-September. All DENV isolates belonged to the genotype I in serotype 1 (DENV-1), and its sequences demonstrated > 99% homology to the sequence of the DENV isolated from a patient in the vicinity of Tokyo Metropolis in 2014. Therefore, Ae. albopictus was a major DENV vector, and a single DENV-1 strain circulated in Tokyo Metropolis in 2014. Dengue virus was not detected from male mosquitoes in 2014 and wild larvae in April 2015. Thus, the possibility of both vertical transmission and overwintering of DENV was extremely low, even in dengue-epidemic areas. This study reports the first entomological information on a dengue outbreak in a temperate region, where no Aedes aegypti mosquitoes are distributed.

Assessing dengue control in Tokyo, 2014.

Project description:BackgroundIn summer 2014, an autochthonous outbreak of dengue occurred in Tokyo, Japan, in which Yoyogi Park acted as the focal area of transmission. Recognizing the outbreak, concerted efforts were made to control viral spread, which included mosquito control, public announcement of the outbreak, and a total ban on entering the park. We sought to assess the effectiveness of these control measures.Methodology/principal findingsWe used a mathematical model to describe the transmission dynamics. Using dates of exposure and illness onset, we categorized cases into three groups according to the availability of these datasets. The infection process was parametrically modeled by generation, and convolution of the infection process and the incubation period was fitted to the data. By estimating the effective reproduction number, we determined that the effect of dengue risk communication together with mosquito control from 28 August 2014 was insufficiently large to lower the reproduction number to below 1. However, once Yoyogi Park was closed on 4 September, the value of the effective reproduction number began to fall below 1, and the associated relative reduction in the effective reproduction number was estimated to be 20%-60%. The mean incubation period was an estimated 5.8 days.Conclusions/significanceRegardless of the assumed number of generations of cases, the combined effect of mosquito control, risk communication, and park closure appeared to be successful in interrupting the chain of dengue transmission in Tokyo.

Autochthonous Dengue Fever in 2 Patients, Rome, Italy.

Project description:Since August 2023, outbreaks of dengue virus (DENV) infection have occurred in Italy. We report 2 autochthonous case-patients and their extended follow-up. Despite persistent DENV detected in blood by PCR, results for antigenomic DENV RNA were negative after day 5, suggesting that a 5-day isolation period is adequate to avoid secondary cases.

Project description:Imported dengue into naive areas is a recognized but unquantified threat. Differentiating imported and autochthonous cases remains problematic. A threshold approach applied to Japan identified several aberrant incidences of dengue. Despite these alerts, no epidemics occurred other than 1 in Yoyogi Park in Tokyo, which was probably an unusual event.

A survey of the 2014 dengue fever epidemic in Guangzhou, China.

Project description:In 2014, a serious dengue outbreak in Guangzhou occurred, consisting of 37 354 laboratory confirmed cases of infection. In this study, the clinical picture of dengue fever due to dengue virus (DENV) type 1 in Guangzhou was described. Clinical and laboratory data collected by studying 726 sera of suspected clinical cases from hospitals and 328 sera of healthy persons from two residence communities were analyzed during the outbreak, and 484 patients were diagnosed with an acute dengue infection. Fever, headache, congestion of the throat, and myalgia were the most typical symptoms in DENV-infected patients. Thrombocytopenia, leukopenia, and an increase in liver enzymes were significantly more common in the infected patients than in the healthy controls. Fourteen cases of silent infection were discovered among the 328 healthy persons, suggesting a DENV inapparent infection rate of 4.27% among healthy individuals. The data obtained by analyzing 212 positive sera with three methods indicated different results with different detection methods. DENV RNA should be used for early diagnoses during days 1-6 after symptom onset, immunoglobulin M (IgM) can be easily recognized after four days have passed since symptom onset and DENV isolation has a peak positive rate during days 1-3 after the onset of symptoms. A phylogenetic analysis of viral NS1 gene sequences from this outbreak indicated that the predominant isolates could be categorized as DENV-1 genotype III and had the highest homology with the India genotypes from 2009 to 2011. However, this analysis also revealed a co-epidemic of the 2013 Zhongshan and 2003 Singapore genotypes, both belonging to DENV-1 genotype I, which suggested multiple geographic origins for the 2014 epidemic of dengue 1 strains in Guangzhou.

Prospective forecasts of annual dengue hemorrhagic fever incidence in Thailand, 2010-2014.

Project description:Dengue hemorrhagic fever (DHF), a severe manifestation of dengue viral infection that can cause severe bleeding, organ impairment, and even death, affects between 15,000 and 105,000 people each year in Thailand. While all Thai provinces experience at least one DHF case most years, the distribution of cases shifts regionally from year to year. Accurately forecasting where DHF outbreaks occur before the dengue season could help public health officials prioritize public health activities. We develop statistical models that use biologically plausible covariates, observed by April each year, to forecast the cumulative DHF incidence for the remainder of the year. We perform cross-validation during the training phase (2000-2009) to select the covariates for these models. A parsimonious model based on preseason incidence outperforms the 10-y median for 65% of province-level annual forecasts, reduces the mean absolute error by 19%, and successfully forecasts outbreaks (area under the receiver operating characteristic curve = 0.84) over the testing period (2010-2014). We find that functions of past incidence contribute most strongly to model performance, whereas the importance of environmental covariates varies regionally. This work illustrates that accurate forecasts of dengue risk are possible in a policy-relevant timeframe.

Epidemiological survey to establish thresholds for influenza among children in satellite cities of Tokyo, Japan, 2014-2018.

Project description:ObjectiveWe described the characteristics of children reported as having influenza across five consecutive influenza seasons and investigated the usefulness of setting influenza thresholds in two satellite cities of Tokyo, Japan.MethodsAn annual survey was conducted among parents of children at preschools (kindergartens and nursery schools), elementary schools and junior high schools in Toda and Warabi cities, Saitama prefecture, at the end of the 2014-2018 influenza seasons. Using the World Health Organization method, we established seasonal, high and alert thresholds.ResultsThere were 64 586 children included in the analysis. Over the five seasons, between 19.1% and 22% of children annually were reported as having tested positive for influenza. Influenza type A was reported as the dominant type, although type B was also reported in more than 40% of cases in the 2015 and 2017 seasons. The median period of the seasonal peak was 3 weeks in mid-January, regardless of school level. Of the five surveyed seasons, the high threshold was reached in 2014 and 2018, with no season exceeding the alert threshold.DiscussionThis study provides insights into the circulation of influenza in children in the study areas of Toda and Warabi, Japan, from 2014 to 2018. Although we were able to utilize these annual surveys to calculate influenza thresholds from five consecutive seasons, the prospective usefulness of these thresholds is limited as the survey is conducted at the end of the influenza season.
