Brentwood community health care assessment.

ABSTRACT:

Background

The Community Alliance for Research Empowering Social Change (CARES) is an academic-community research partnership designed to train community members on research methods and develop the infrastructure for community-based participatory research (CBPR) to examine and address racial/ethnic health disparities. The Brentwood Community Health Assessment (BCHA) was developed through a CBPR pilot project grant from CARES.

Objectives

The purpose of the BCHA is to assess health care utilization and identify existing barriers to health care access among a multi-ethnic community in the Hamlet of Brentwood, New York.

Methods

Using CBPR approaches, the community-academic research partnership develop the study design and survey instrument. Trained Bilingual (English/Spanish) data collectors verbally administered surveys door-to-door to residents of Brentwood from October 2010 to May 2011. Inclusion criteria required participants to be at least 18 years of age and speak either English or Spanish.

Results

Overall, 232 residents completed the BCHA; 49% were male, 66% Hispanic, 13% non-Hispanic White, 13% non-Hispanic Black, 29% had less than a high school education, and 33% were born in United States. The assessment results revealed that most residents are able to access health care when needed and the most significant barriers to health care access are insurance and cost.

Conclusions

We describe the community-academic partnered process used to develop and implement the BCHA and report assessment findings; the community-partnered approach improved data collection and allowed access into one of Suffolk County's most vulnerable communities.

Project description:BackgroundCommunity Health Workers (CHWs) have a positive impact on the provision of community-based primary health care through screening, treatment, referral, psychosocial support, and accompaniment. With a broad scope of work, CHW programs must balance the breadth and depth of tasks to maintain CHW motivation for high-quality care delivery. Few studies have described the CHW perspective on intrinsic and extrinsic motivation to enhance their programmatic activities.MethodsWe utilized an exploratory qualitative study design with CHWs employed in the household model in Neno District, Malawi, to explore their perspectives on intrinsic and extrinsic motivators and dissatisfiers in their work. Data was collected in 8 focus group discussions with 90 CHWs in October 2018 and March-April 2019 in seven purposively selected catchment areas. All interviews were audiotaped, transcribed verbatim, coded, and analyzed using Dedoose.ResultsThemes of complex intrinsic and extrinsic factors were generated from the perspectives of the CHWs in the focus group discussions. Study results indicate that enabling factors are primarily intrinsic factors such as positive patient outcomes, community respect, and recognition by the formal health care system but can lead to the challenge of increased scope and workload. Extrinsic factors can provide challenges, including an increased scope and workload from original expectations, lack of resources to utilize in their work, and rugged geography. However, a positive work environment through supportive relationships between CHWs and supervisors enables the CHWs.ConclusionThis study demonstrated enabling factors and challenges for CHW performance from their perspective within the dual-factor theory. We can mitigate challenges through focused efforts to limit geographical distance, manage workload, and strengthen CHW support to reinforce their recognition and trust. Such programmatic emphasis can focus on enhancing motivational factors found in this study to improve the CHWs' experience in their role. The engagement of CHWs, the communities, and the formal health care system is critical to improving the care provided to the patients and communities, along with building supportive systems to recognize the work done by CHWs for the primary health care systems.

Project description:BackgroundAndalusia is the second largest region in Spain, and it has developed a comprehensive mental health (MH) plan that encourages the consolidation of the balanced care model. However, its geographical and socioeconomic disparity is a great challenge for a community-based MH system. Both the assessment of the implementation of the MH plan and the development of new tools to support decision-making can be considered critical.ObjectivesThe present study aims (i) to assess how the integration of different types of MH care may influence system performance and (ii) to check the performance evolution of the integration process geographically regarding the small MH areas of Andalusia.MethodsThe performance of the Andalusian MH system was assessed by combining Monte Carlo simulation, fuzzy inference and data envelopment analysis. The relative technical efficiency was the main performance indicator.ResultsA correct integration of appropriate types of MH care, according to population needs, increases the performance of the Andalusian MH system both from global and regional perspectives. The spatial representation (based on small MH areas) of the results highlights how the performance depends on specific geographical characteristics. By analyzing the identified spatial clusters, defined by different management patterns depending on user and socioeconomic characteristics, benchmark areas and areas for improvement can be studied to design evidence-informed policies and interventions.ConclusionsA global analysis of MH system performance was carried out, including both the successive integration of different types of care and its spatial evolution. Although an appropriate integration of different types of MH care has a positive effect on the Andalusian MH system, this process has different profiles depending on specific geographically based user and socioeconomic characteristics. The balanced care model can be considered the paradigm for assessing the performance of a large and populated territory such as Andalusia, which has a community-based MH system. This methodological approach (performance assessment and spatial analysis) may be used as a guide for developing future evidence-informed policies and managerial interventions.

Project description:BackgroundPrimary Health Care (PHC) is the cornerstone of any healthcare system, with nurses forming the largest workforce involved in care. This study aimed to assess the current use of core competencies among community-based nurses, identify their learning needs, and assess factors associated with training needs within PHC centers.MethodsA quantitative cross-sectional survey design was used, targeting community health nurses working within primary healthcare centers. Data were collected using a survey instrument adapted from the Canadian Community Health Nurses' Standards of Practice and informed by a validated tool, then piloted for clarity in the Lebanese context. Data were collected between September and November 2018. Mean, standard deviation (SD), frequency, and percentage data were computed for descriptive purposes. The generalized estimating equation (GEE) was used to identify the factors associated with nurses' training needs clustered within centers. Odds ratios (ORs) and 95% confidence intervals (CIs) were estimated using logistic GEE regression models that accounted for cluster effects.ResultsThe total number of PHCs that agreed to participate was 206, of which 173 returned completed surveys. Given that we do not have an accurate number of the population of nurses working in those centers, we assumed that there would be two nurses in each PHC. Thus, for a total of 251 surveys completed by nurses, the response rate is estimated to be 61%. Of the 173 surveys, 139 were included in the final analysis after deleting those that were incomplete. Descriptive results showed that nurses were competent in providing continuous care (60.0%), electronic technology use (55.08%), and clinical nursing assessment (54.01%). They reported a need for more training on community health promotion (65.12%), patient-centered care (PCC) (58.30%), and patient self-management of chronic diseases (52.0%). In comparison to nurses working in accredited centers, nurses working in centers in the process of becoming accredited required three times more training to become competent in PCC (OR = 3.39, 95% CI: 1.26-9.31, p = 0.016). Registered nurses required three times less training in PCC than senior/head nurses (OR = 0.30, 95% CI: 0.11-0.80, p = 0.016). Education level was statistically significantly associated with most training needs. Nurses with Baccalaureate and Technique Superior degrees needed six times more training (OR = 6.07, 95% CI: 1.81-31.16, p = 0.031) than those with a bachelor's or master's degree in nursing.ConclusionThis study provided a baseline assessment for the competencies that nurses reported implementing and those that they requested more training on. Future steps would be to develop interventions to empower nurses with the competencies they requested as priorities and to conduct a post intervention assessment to test the effect of the training on nursing adoption of those skills.
