ABSTRACT: Introduction

Sensitivity and specificity of the SCOFF were 53.7 and 93.5%, respectively.Conclusion
The SCOFF showed good levels of specificity but low sensitivity, resulting in a high percentage of false negatives. Given the low sensitivity found in our sample the SCOFF is likely to be a suboptimal measure for the identification of ED in the community.

Project description:ObjectiveEating disorders (EDs) represent a significant public health concern, with a high prevalence among adolescent girls. The COVID-19 pandemic has led to a marked increase in EDs, underlining the need for a brief screening tool in clinical settings. The present study compared ICD-10-based ED-diagnoses with SCOFF-identified at-risk female patients in a clinical setting and assessed the tool's reliability.MethodRoutine data (n = 955 participants) from a child and adolescent psychiatric hospital were evaluated regarding the incidence of ED-diagnoses before (n = 312) and during the pandemic (n = 643) and compared with SCOFF results. The SCOFF's sensitivity and specificity were examined.ResultsED-diagnoses significantly increased, almost doubling from before to during the pandemic. The number of at-risk patients identified by the SCOFF likewise differed between the two time points. The SCOFF showed high sensitivity at cut-offs of 2 (pre-pandemic) and 3 (peri-pandemic) but low specificity. Internal consistency was poor.DiscussionThe findings align with previous research indicating an elevated rate of admission to clinical care for ED-diagnoses during the pandemic, a trend also reflected in the SCOFF results. However, the SCOFF's low specificity and poor internal consistency raise concerns about its applicability among adolescent psychiatric inpatients.

Project description:BackgroundWe evaluated the performance of a clinical algorithm (Expali™), combining two or more positive answers to SCOFF questionnaire with Body Mass Index (BMI), to identify four Broad Categories of eating disorders (ED) derived from DSM-5.MethodsThe clinical algorithm (Expali™) was developed from 104 combinations of BMI levels and answers to five SCOFF questions with at least two positive answers. Two senior ED physicians allocated each combination to one of the four Broad Categories of ED derived from DSM-5: restrictive disorder, bulimic disorder, hyperphagic disorder and other unspecified ED diagnosed by ED clinicians. The performance of Expali™ was evaluated on data from 206 patients with ED. Sensitivity, specificity values and Youden index were calculated for each category.ResultsThe 206 patients were diagnosed as follows: 31.5% restrictive disorder, 18.9% bulimic disorder, 40.8% hyperphagic disorder and 8.8% other ED. The sensitivity of Expali™ for restrictive, bulimic, hyperphagic and other unspecified ED were respectively: 76.9, 69.2, 79.7 and 16.7%. The Youden index was respectively 0.73, 0.57, 0.67 and 0.07.ConclusionsIn a SCOFF-positive ED population (at least two positive answers), the clinical algorithm Expali™ demonstrated good suitability by correctly classifying three of the four Broad Categories of eating disorders (restrictive, bulimic and hyperphagic disorder). It could be useful both to healthcare professionals and the general population to enable earlier detection and treatment of ED and to improve patient outcomes.

Project description:BackgroundEating Disorders (EDs) are defined by abnormal eating habits. The SCOFF (Sick-Control-One stone-Fat-Food) is a simple screening questionnaire for EDs. This study was conducted to translate and evaluate the psychometric properties of the SCOFF questionnaire in Iranian university students.MethodsA total of 310 Iranian students of the Shahid Beheshti University of Medical Sciences completed a test battery of questionnaires including the well-known screener of eating disorder symptoms, the SCOFF. All measures were presented to the participants in Persian. The 5-item SCOFF questionnaire was translated to Persian using the forward-backward method. The face, content, criterion, and construct validity of the Persian version of the SCOFF were assessed. The validity and reliability of the Persian version of the SCOFF was assessed and factor analysis was conducted.ResultsAll five items of the translated questionnaire were approved after face validity. Content validity ratio was 0.73 (range 0.66-0.83) and content validity index was 0.96 (range 0.91-1), so all items were approved. Exploratory factor analysis revealed a 2-factor structure, which explained 52.47% of the variance. Confirmatory factor analysis showed a very good goodness-of-fit for the 2-factor model. 2-factor and 1-factor models indicate a very good goodness-of-fit in females and adequate goodness-of-fit in males. Criterion validity showed an acceptable correlation between the SCOFF and the EDE-Q. Reliability was acceptable based on the stability [ICC = 0.905(95% CI .760-.962 p < 0.001)] and the internal consistency (KR20 = 0.4).ConclusionAppropriate psychometric properties of the 5-item Persian version of the SCOFF (both models) were confirmed, suggesting its use as a valid questionnaire in EDs screening.

Project description:BackgroundThe Versus Arthritis Musculoskeletal Health Questionnaire (MSK-HQ) measures symptom severity and health-related quality of life (HRQoL) of people with musculoskeletal (MSK) conditions. We aimed to test the psychometric properties of the MSK-HQ among the general adult population and identify the determinants of MSK-HQ states. In addition, we aimed to explore the relationship between MSK-HQ and standard well-being measurement tools.MethodsThe translation proccess of the MSK-HQ into Hungarian followed the standard methods provided by the developer. A cross-sectional online survey was performed in Hungary involving a population normative sample (N = 2004, women: 53.1%; mean age: 48.3, SD = 16.6 years). Socio-demographic characteristics and self-reported MSK disorders were recorded. Alongside the MSK-HQ, standard measures of HRQoL (EQ-5D-5L), physical functioning (HAQ-DI) and well-being (ICECAP-A/O, WHO-5, Happiness VAS) were applied. Clinical and convergent validity were assessed by subgroup comparisons (Mann-Whitney-U and Kruskal-Wallis tests) and Spearman's rank correlations. Internal consistency was assessed by Cronbach's alpha. Test-retest reliability (N = 50) was evaluated by intraclass correlation coefficient (ICC). Predictors of MSK-HQ were analysed by ordinary least square multiple regressions.ResultsThe mean MSK-HQ index score was 44.1 (SD = 9.9). MSK-HQ scores were significantly lower in subgroups with self-reported MSK disorders. Correlations were strong between MSK-HQ and EQ-5D-5L (0.788), EQ VAS (0.644) and HAQ-DI (-0.698) and moderate with the well-being measures (p < 0.05). Cronbach's alpha was 0.924 and ICC was 0.936 (p < 0.05). Being a man, living in the capital, having higher income and education were positively associated with MSK-HQ scores.ConclusionsThis is the first study to prove the validity and reliability of the MSK-HQ among the general public. The impact of socio-demographic characteristics on MSK-HQ scores deserves consideration in clinical studies.

Project description:Findings from studies investigating cognitive flexibility in eating disorders (EDs) are inconsistent, and although neuropsychological tests are commonly used to measure these skills, they may not be particularly effective in predicting everyday functioning. Also, extant studies have largely focused on flexibility in anorexia nervosa (AN), with assessments targeting general rather than specific flexibility, and cognitive, rather than behavioral flexibility. Knowledge regarding ED specific flexibility and flexibility in bulimia nervosa (BN) and binge eating disorder (BED) is still scarce. The aim of this study was to develop and validate a novel measure assessing general and ED specific flexibility in a diagnostically diverse sample, and in healthy controls (HCs). A sample of 207 adult individuals with EDs (55% AN, 29% BN, 16% BED) and 288 HCs responded to an online, 51-item, pilot questionnaire on ED specific and general flexibility. In addition, participants completed the shift subscale from the Behavior Rating Inventory of Executive Function Adult version (BRIEF-A), and the Eating Disorder Diagnostic Scale (EDDS). A principal component analysis (PCA) in the clinical sample yielded a 36-item, three-factor solution capturing general flexibility, flexibility related to food and exercise, and flexibility concerning body shape and weight. Results showed that the measure had good to excellent internal consistency, and good convergent validity. A confirmatory factor analysis (CFA) using data from HCs revealed good fit indexes, supporting the original factor solution. A receiver operating characteristics analysis (ROC) demonstrated excellent accuracy in distinguishing scores from those with and without EDs. A cutoff score of 136 yielded the most balanced sensitivity and specificity. Significant differences in general and ED specific flexibility were found between individuals with and without EDs. Overall, HCs achieved the highest flexibility scores, followed by those with BED, BN, and AN. In sum this novel measure, the Eating Disorder Flexibility Index (EDFLIX) questionnaire, was found to be reliable and valid in the assessment of cognitive and behavioral flexibility, with results offering support for the conceptual distinction between general and ED related flexibility. The study also provides strong evidence for the discriminant validity of the EDFLIX with results revealing significant differences in flexibility in people with and without EDs. In addition, significant differences in flexibility also emerged when comparing diagnostic groups, indicating the utility of the assessment instrument for classification purposes.

Project description:ObjectivesAlthough common, sleep disorders often remain undiagnosed in psychiatric patients. A screening instrument, like the Holland Sleep Disorders Questionnaire (HSDQ) could improve this. Previous work indicated a 6-factor structure for the HSDQ, but this hasn't been investigated in psychiatric patients.MethodsHSDQ data was collected in a psychiatric-outpatient sample (n = 1082) and general-population sample (n = 2089). Internal reliability of the HSDQ was investigated and Confirmatory Factor Analyses (CFA) were used to compare 1-, 6-, and second-order 6-factor models in both samples. Next, multigroup-CFA was used to investigate measurement invariance.ResultsExcept for one subscale, internal reliability was acceptable in both samples. The 6-factor structure model fitted best in both samples and investigation of measurement invariance showed evidence for equality of the overall factor structure (configural invariance). Addition of equality constraints on factor loadings (metric invariance) and item thresholds (scalar invariance) showed good fit for all fit statistics, except for one. Exploratory analyses identified three items for metric and three different items for scalar invariance explaining this non-invariance.ConclusionThe HSDQ has a 6-factor structure in psychiatric patients, which is comparable to the general population. However, due to the observed non-invariance, users should be cautious with comparing HSDQ scores between psychiatric and general populations.

Project description:(1) Background: Obesity, part of the triple global burden of disease, is increasingly attracting research on its preventive and curative management. Knowledge of eating behavior can be useful both at the individual level (to individualize treatment for obesity) and the population level (to implement more suitable food policies). The Dutch Eating Behavior Questionnaire (DEBQ) is a widely used international tool to assess eating behavior, i.e., emotional, external and restricted eating styles. The aim of this study was to validate the Romanian version of DEBQ, as obesity is a major concern in Romania. (2) Methods: Our study tested the psychometric properties of the Romanian version of DEBQ on an adult population and explored the associations of eating behavior with weight status (3) Results: The study showed a factor load similar to the original version of the questionnaire and a very good internal validity (Cronbach's alpha fidelity coefficient greater than 0.8 for all scales of the questionnaire) for the Romanian version of DEBQ and showed that all of the scales positively correlated with body mass index in both men and women. (4) Conclusions: This study will enable the use of the DEBQ Romanian version on the adult population of Romania where the findings could be incorporated into developing better strategies to reduce the burden of nutrition-related diseases.

Project description:BACKGROUND AND OBJECTIVES:The Eating Disorder Examination Questionnaire 6.0 (EDE-Q 6.0) is one of the most broadly used self-report tools that assesses attitudes and behaviors associated with eating disorders (EDs). The aim of the present study was to examine the reliability, validity, and factor structure of the Lithuanian version of the EDE-Q 6.0 (LT-EDE-Q 6.0) in a nonclinical student sample. MATERIALS AND METHODS:A sample of 382 students (mean age 24.0 ± 6.4) participated in the study. The students completed a self-report questionnaire measuring the risk of EDs (LT-EDE-Q 6.0), body image (LT-MBSRQ-AS), quality of life (LT-WHOQOL-BREF), and self-esteem (RSES). Cronbach's alpha assessed the internal consistency of the EDE-Q 6.0. Pearson's correlations were used for the analyses of the construct and concurrent validity with the subscales of LT-MBSRQ-AS, LT-WHOQOL-BREF, and RSES. Intraclass correlation coefficients (ICC) were calculated for assessing test-retest reliability. RESULTS:The mean score of the LT-EDE-Q 6.0 in the mixed sample was 1.5 ± 1.02. For women and men, the general mean scores were higher than in the majority of the samples of Western Europe but lower than in the United States. Acceptable internal consistency for the four subscales (0.75-0.88) and the LT-EDE-Q 6.0 general score (0.94) was obtained. Test-retest reliability was good to excellent for all subscales (0.66-0.91) and for the items that assessed essential behavioral features of EDs (0.84-0.90, except item 14 ICC = 0.4). The LT-EDE-Q 6.0 scores had adequate concurrent validity. However, the original 4-factor structure or other proposed models of EDE-Q were not obtained by CFA. CONCLUSIONS:The results of the current study support the applicability, validity, and reliability of the LT-EDE-Q 6.0 in a nonclinical Lithuanian student sample. However, we recommend assessing the general scale score without the application of the subscales. The Lithuanian version of this instrument should be further investigated with clinical samples to identify clinically diagnosed cases.

Project description:Assessing the validity of a psychometric test is fundamental to ensure a reliable interpretation of its outcomes. Few attempts have been made recently to complement classical approaches (e.g., factor models) with a novel technique based on network analysis. The objective of the current study is to carry out a network-based validation of the Eating Disorder Inventory 3 (EDI-3), a questionnaire designed for the assessment of eating disorders. Exploiting a reliable, open source sample of 1206 patients diagnosed with an eating disorder, we set up a robust validation process encompassing detection and handling of redundant EDI-3 items, estimation of the cross-sample psychometric network, resampling bootstrap procedure and computation of the median network of the replica samples. We then employed a community detection algorithm to identify the topological clusters, evaluated their coherence with the EDI-3 subscales and replicated the full validation analysis on the subpopulations corresponding to patients diagnosed with either anorexia nervosa or bulimia nervosa. Results of the network-based analysis, and particularly the topological community structures, provided support for almost all the composite scores of the EDI-3 and for 2 single subscales: Bulimia and Maturity Fear. A moderate instability of some dimensions led to the identification of a few multidimensional items that should be better located in the intersection of multiple psychological scales. We also found that, besides symptoms typically attributed to eating disorders, such as drive for thinness, also non-specific symptoms like low self-esteem and interoceptive deficits play a central role in both the cross-sample and the diagnosis-specific networks. Our work adds insights into the complex and multidimensional structure of EDI-3 by providing support to its network-based validity on both mixed and diagnosis-specific samples. Moreover, we replicated previous results that reinforce the transdiagnostic theory of eating disorders.

Project description:The Eating Disorder Examination Questionnaire is a widely used self-report questionnaire for eating disorders. An Eating Disorder Examination Questionnaire model that is not lacking in relevant content has been supported in three different samples, but existing studies on this model present shortcomings regarding generalizations to the general population. Therefore, the general purpose of the current research was to test the reliability and interpretation validity of the Eating Disorder Examination Questionnaire 6.0 scores in adults of both sexes from the Mexican general population. After translating, adapting, and assessing the Eating Disorder Examination Questionnaire 6.0 in the target population through three pilot studies, we conducted two independent studies. In Study 1, 684 women and 433 men aged 18-83 participated, whereas in Study 2, 591 women and 382 men aged 18-86 did it. They answered the Eating Disorder Examination Questionnaire 6.0 and a measure of either body dissatisfaction (Study 1) or self-esteem (Study 2). According to confirmatory factor analyses, the 14-item Eating Disorder Examination Questionnaire model that we tested fit acceptably for the four samples (two female, two male) and was invariant across sex. All 14-item Eating Disorder Examination Questionnaire 6.0 scores were reliable according to Cronbach's alpha and McDonald's omega, except for only one factor score in men. Pearson's correlations of the 14-item Eating Disorder Examination Questionnaire 6.0 scores with body dissatisfaction and self-esteem were positive and negative, respectively. This new Latin American Spanish translation of the Eating Disorder Examination Questionnaire 6.0 works broadly as expected and provides evidence to extend the generalization of previous studies to the general population. Thus, the present translation of the Eating Disorder Examination Questionnaire 6.0 may be a valuable tool in the field of eating disorders for researchers and practitioners studying or serving Latin American Spanish speakers of either sex from the general population.
