A unique in vivo approach for investigating antimicrobial materials utilizing fistulated animals.

ABSTRACT: Unique in vivo tests were conducted through the use of a fistulated ruminant, providing an ideal environment with a diverse and vibrant microbial community. Utilizing such a procedure can be especially invaluable for investigating the performance of antimicrobial materials related to human and animal related infections. In this pilot study, it is shown that the rumen of a fistulated animal provides an excellent live laboratory for assessing the properties of antimicrobial materials. We investigate microbial colonization onto model nanocomposites based on silver (Ag) nanoparticles at different concentrations into polydimethylsiloxane (PDMS). With implantable devices posing a major risk for hospital-acquired infections, the present study provides a viable solution to understand microbial colonization with the potential to reduce the incidence of infection through the introduction of Ag nanoparticles at the optimum concentrations. In vitro measurements were also conducted to show the validity of the approach. An optimal loading of 0.25 wt% Ag is found to show the greatest antimicrobial activity and observed through the in vivo tests to reduce the microbial diversity colonizing the surface.

Project description:There are global concerns regarding the transmission of antimicrobial-resistant pathogens from animals to humans. Especially, companion animals are increasingly recognized as a potential source due to their close interactions with people, despite a limited number of reported cases. Although, social demands regarding comprehensive surveillance for antimicrobial resistance (AMR) among companion animals are highlighted, there is a lack of a relevant system in South Korea. In this regard, we conducted preliminary investigation on antimicrobial use (AMU) among small animal clinics, along with veterinary practitioner's knowledge and attitude regarding this issue in South Korea. We collected data on 684,153 antimicrobial prescription visits for canine and feline patients from 2019 to 2022 at 100 veterinary facilities in South Korea, using electronic medical records. To evaluate antimicrobial use (AMU) and facilitate comparisons across institutions and time periods, we developed the Defined Daily Dose for Animals (DDDA) and the Defined Animal Daily Dosages per 1000 Animal-Days (DAPD). In addition, we conducted an online survey of 362 veterinary practitioners, which included questions on their perceptions, attitudes, and practices regarding antimicrobial prescriptions. Simple frequency analyses were performed to examine temporal trends, regional differences and variations by facility size in AMU, and to summarize survey responses. Descriptive analysis using data from 100 veterinary clinics revealed a rising trend in AMU between 2019 and 2022, with higher usage observed in larger clinics and non-capital regions. DDDA values for dogs were generally higher than for cats. Survey results highlighted that, while veterinarians exhibited high awareness of AMR, prescribing practices were significantly influenced by clinical judgments and owner demands, often deviating from established guidelines. The adoption of an electronic veterinary prescription management system (e-Vet) was proposed to enhance antimicrobial stewardship. However, concerns regarding the system's efficiency and administrative burden were prominent. To our best knowledge, this study provided DDDA for companion animals for the first time in South Korea. Although the indicator should be improved with more comprehensive data and expert opinion, our study showed that it enables reasonable situation analysis regarding AMU in companion animals. The identified factors that affect veterinarians' prescription practices can also be used to design an effective strategy for promoting appropriate antimicrobial usage.

Project description:ImportanceInappropriate antimicrobial prescribing leads to antimicrobial resistance and suboptimal clinical outcomes. Changing antimicrobial prescribing is a complex behavioral process that is not often taken into account in antimicrobial stewardship programs.ObjectiveTo examine whether an antimicrobial stewardship approach grounded in behavioral theory and focusing on preserving prescriber autonomy and participation is effective in improving appropriateness of antimicrobial prescribing in hospitals.Design, setting, and participantsThe Dutch Unique Method for Antimicrobial Stewardship (DUMAS) study was a prospective, stepped-wedge, participatory intervention study performed from October 1, 2011, through December 31, 2015. Outcomes were measured during a baseline period of 16 months and an intervention period of 12 months. The study was performed at 7 clinical departments (2 medical, 3 surgical, and 2 pediatric) in a tertiary care medical center and a general teaching hospital in the Netherlands. Physicians prescribing systemic antimicrobial drugs for any indication for patients admitted to the participating departments during the study period were included in the study.InterventionsWe offered prescribers a free choice of how to improve their antimicrobial prescribing. Prescribers were stimulated to choose interventions with higher potential for success based on a root cause analysis of inappropriate prescribing.Main outcomes and measuresAppropriateness of antimicrobial prescriptions was determined using a validated approach based on guideline adherence and motivated guideline deviation and measured with repeated point prevalence surveys (6 per year). Appropriateness judgment was masked for the study period. Antimicrobial consumption was extracted from pharmacy records and measured as days of therapy per admission. We used linear and logistic mixed-model regression analysis to model outcomes over time.ResultsA total of 1121 patient cases with 700 antimicrobial prescriptions were assessed during the baseline period and 882 patient cases with 531 antimicrobial prescriptions during the intervention period. The mean antimicrobial appropriateness increased from 64.1% at intervention start to 77.4% at 12-month follow-up (+13.3%; relative risk, 1.17; 95% CI, 1.04-1.27), without a change in slope. No decrease in antimicrobial consumption was found.Conclusions and relevanceUse of a behavioral approach preserving prescriber autonomy resulted in an increase in antimicrobial appropriateness sustained for at least 12 months. The approach is inexpensive and could be easily transferable to various health care environments.

Project description:IntroductionAs antimicrobial resistance (AMR) represents a substantial threat to the efficacy of available antimicrobial options, it is important to understand how to implement effective and practical mitigation efforts, including antimicrobial stewardship (AMS), across human, animal, and environmental sectors.MethodsA mixed-methods questionnaire was distributed virtually to attendees of the virtual One Health Antimicrobial Stewardship Conference (March 10-12, 2021) and their professional networks. Respondents (n = 81) were largely from the veterinary (75%) or human (19%) health sectors. Qualitative data were analyzed in NVivo using template analysis whereas quantitative data were analyzed in STATA using Kruskall-Wallis tests. The questionnaire asked respondents about their perceptions of AMS, as well as the perceived barriers and drivers of AMS efforts.ResultsPerceptions of what AMS meant to the respondents personally and their profession as a whole were grouped into 3 main themes: 1) AMS strategies or considerations in antimicrobial prescribing and use; 2) responsibility to maintain health and preserve antimicrobial effectiveness; and 3) reducing antimicrobial use (AMU) as a goal of AMS efforts. Identified AMS barriers had 3 main themes: 1) lack of various prescribing and AMU support mechanisms; 2) shift in prescriber attitudes to drive change; and 3) stronger economic considerations to support shifting prescribing practices. Drivers of AMS had the following themes: 1) leadership to guide change; 2) education to support optimizing AMU; and 3) research to identify best practices and opportunities for action. Across all questions, 2 cross-cutting themes emerged: 1) a One Health understanding of AMS; and 2) blame placed on others for a lack of AMS success.ConclusionOverall, sector-specific, but particularly cross-sectoral AMS drivers and barriers were identified, highlighting the importance of a One Health approach in AMR research and mitigation.
