ABSTRACT:

Background

Metformin, statin and aspirin use seem associated with decreased mortality in cancer patients, though, without adjusting for one another. Independent associations of these drugs with overall mortality after colorectal cancer (CRC) diagnosis within glucose-lowering drugs (GLDs) users were assessed.

Methods

Patients starting GLDs before CRC diagnosis (1998-2011) were selected from the Eindhoven Cancer Registry linked with the PHARMO Database Network. The Cox regression model, with time since CRC diagnosis, included time-dependent variables of cumulative exposure to metformin, statins and aspirin after cancer diagnosis and time-dependent ever-never terms for drug exposure.

Results

A total of 1043 patients used GLDs before CRC diagnosis; 666 (64%) used metformin, 639 (61%) used statins and 490 (47%) used aspirin after CRC diagnosis. Multivariable analyses revealed that longer cumulative exposure to metformin was not associated with overall mortality (HRCumulative exposure/6 months 1.02; 95% CI 0.97-1.07), whereas the favourable effect of statins increased with cumulative exposure (HRCumulative exposure/6 months 0.93; 95% CI 0.89-0.98). No association between aspirin use and overall mortality was seen (HRCumulative exposure/6 months 0.98; 95% CI 0.93-1.03).

Conclusions

No independent association between cumulative exposure to metformin, aspirin and overall mortality was found. Cumulative exposure to statins after CRC diagnosis was associated with lower overall mortality, supporting a drug effect of statins among GLDs users.

Project description:BackgroundAspirin use reduces colorectal cancer (CRC) incidence, but there is limited evidence regarding associations of aspirin and non-aspirin non-steroidal anti-inflammatory drugs (NSAIDs) with CRC-specific survival.MethodsThis prospective analysis includes women and men from the Cancer Prevention Study-II Nutrition Cohort who were cancer free at baseline (1992 or 1993) and diagnosed with CRC during incidence follow-up through 2015. Detailed information on aspirin and non-aspirin NSAID use was self-reported on questionnaires at baseline, in 1997, and every 2 years thereafter. Pre- and postdiagnosis data were available for 2686 and 1931 participants without distant metastases, respectively, among whom 512 and 251 died from CRC during mortality follow-up through 2016. Secondary analyses examined associations between prediagnosis aspirin use and stage at diagnosis (distant metastatic vs localized or regional). All statistical tests were 2-sided.ResultsLong-term regular use of aspirin (>15 times per month) before diagnosis was associated with lower CRC-specific mortality (multivariable-adjusted hazard ratio [HR] = 0.69, 95% confidence interval [CI] = 0.52 to 0.92). Postdiagnosis regular aspirin use was not statistically significantly associated with risk of CRC-specific mortality overall (HR = 0.82, 95% CI = 0.62 to 1.09), although participants who began regular aspirin use only after their diagnosis were at lower risk than participants who did not use aspirin at both the pre- and postdiagnosis periods (HR = 0.60, 95% CI = 0.36 to 0.98). Long-term aspirin use before diagnosis was also associated with lower odds of diagnosis with distant metastases (multivariable-adjusted odds ratio = 0.73, 95% CI = 0.53 to 0.99).ConclusionsOur results suggest that long-term aspirin use before a diagnosis of nonmetastatic colorectal cancer may be associated with lower CRC-specific mortality after diagnosis, consistent with possible inhibition of micrometastases before diagnosis.

Project description:Abstract Old‐growth forests host a rich diversity of invertebrate assemblages. Among them, saproxylic insects play a fundamental role in the nutrient cycle and ecosystem functioning. In these environments, coevolution between insect and plants have reached a stable equilibrium over millions of years. These delicate ecosystems are threatened mainly by habitat loss and fragmentation, and to date, they have to face the new “plastic threat.” Plastics are widespread in all biomes and ecosystems accumulating throughout the years due to their low degradation rate. Once accumulated, large pieces of plastics can be degraded into smaller particles, the latter representing a great threat to biodiversity and ecosystem health, producing detrimental effects on biota. Since the effects of plastics on terrestrial systems remain largely unexplored, this study aimed at contributing to increasing the knowledge on the interaction between plastics and terrestrial biota. We put our emphasis on the novel and broad topic of plastic degradation by saproxylic beetle larvae, describing how they fragmented macroplastics into microplastics. To investigate whether saproxylic cetonid larvae could degrade expanded polystyrene, we performed an experiment. Thus, we put larvae collected in the field in an expanded polystyrene box. We observed that larvae dug in the thickness of the box fragmenting macroplastics into microplastics and producing a total of 3441 particles. Then, we removed the larvae from the EPS box and isolated them in glass jars filled with natural substrate. The substrate was checked for EPS microplastics previously ingested and now egested by larvae. Additionally, we pointed out that plastics remained attached to cetonid larvae setae, with a mean number of 30.7 ± 12.5 items. Although preliminary, our results highlighted that microplastics attached to saproxylic cetonid larvae might be transported into habitats and transferred along the food web. In conclusion, plastic pollution might affect vulnerable species and ecosystem services representing a risk also for human health. Macroplastics are degraded by abiotic factors (e.g., UV rays, wind, and rain) but also by biotic ones (e.g., larvae). Thus, this fragmentation process originates in microplastics that can be ingested by organisms and remain attached to cetonid larvae setae after digging into plastics. Then, microplastics may be transferred along with the food web when larvae are eaten by predators, such as hole‐nesting birds.
