Advances in Breast Cancer - Looking Back over the Year.

ABSTRACT: Treatment options as well as the characteristics for therapeutic decisions in patients with primary and advanced breast cancer are increasing in number and variety. New targeted therapies in combination with established chemotherapy schemes are broadening the spectrum, yet not every new, promising combination achieves a better result. New data from the field of pharmacogenomics point to prognostic and predictive factors that take not only the properties of the tumour but also the genetic disposition of the patient into consideration. Current therapeutic decision-making is thus based on a combination of classical clinical and modern molecular biomarkers. Health-economic concerns are also being taken into consideration more frequently, meaning political decisions may also become a factor. This review presents the trends over the past year.

Project description:GigaScience is now 5 years old, having been launched at the 2012 Intelligent Systems for Molecular Biology conference. Anyone who has attended what is the largest computational biology conference since then has had the opportunity to join us for each birthday celebration-and receive 1 of our fun T-shirts as a party prize. Since launching, we have pushed our agenda of openness, transparency, reproducibility, and reusability. Here, we look back at our first 5 years and what we have done to forward our open science goals in scientific publishing. Our mainstay has been to create a process that allows the availability and publication of as many "research objects" as possible to create a more complete way of communicating how the research process is done.

Looking back on forward-looking COVID models.

Project description:Covid Act Now (CAN) developed an epidemiological model that takes various non-pharmaceutical interventions (NPIs) into account and predicts viral spread and subsequent health outcomes. In this study, the projections of the model developed by CAN were back-tested against real-world data, and it was found that the model consistently overestimated hospitalizations and deaths by 25%-100% and 70%-170%, respectively, due in part to an underestimation of the efficacy of NPIs. Other COVID models were also back-tested against historical data, and it was found that all models generally captured the potential magnitude and directionality of the pandemic in the short term. There are limitations to epidemiological models, but understanding these limitations enables these models to be utilized as tools for data-driven decision-making in viral outbreaks. Further, it can be valuable to have multiple, independently developed models to mitigate the inaccuracies of or to correct for the incorrect assumptions made by a particular model.

The 1918 Influenza Pandemic: Looking Back, Looking Forward.

Project description:In commemoration of the centennial of the 1918 influenza pandemic, the American Journal of Epidemiology has convened a collection of 12 articles that further illuminate the epidemiology of that pandemic and consider whether we would be more prepared if an equally deadly influenza virus were to emerge again. In the present commentary, we place these 12 articles in the context of a growing body of work on the archeo-epidemiology of past pandemics, the socioeconomic and geographic drivers of influenza mortality and natality impact, and renewed interest in immune imprinting mechanisms and the development of novel influenza vaccines. We also highlight persisting mysteries in the origins and severity of the 1918 pandemic and the need to preserve rapidly decaying information that may provide treasure troves for future generations.

Impact of High-to-Moderate Penetrance Genes on Genetic Testing: Looking over Breast Cancer.

Project description:Breast cancer (BC) is the most common cancer and the leading cause of cancer death in women worldwide. Since the discovery of the highly penetrant susceptibility genes BRCA1 and BRCA2, many other predisposition genes that confer a moderate risk of BC have been identified. Advances in multigene panel testing have allowed the simultaneous sequencing of BRCA1/2 with these genes in a cost-effective way. Germline DNA from 521 cases with BC fulfilling diagnostic criteria for hereditary BC were screened with multigene NGS testing. Pathogenic (PVs) and likely pathogenic (LPVs) variants in moderate penetrance genes were identified in 15 out of 521 patients (2.9%), including 2 missense, 7 non-sense, 1 indel, and 3 splice variants, as well as two different exon deletions, as follows: ATM (n = 4), CHEK2 (n = 5), PALB2 (n = 2), RAD51C (n = 1), and RAD51D (n = 3). Moreover, the segregation analysis of PVs and LPVs into first-degree relatives allowed the detection of CHEK2 variant carriers diagnosed with in situ melanoma and clear cell renal cell carcinoma (ccRCC), respectively. Extended testing beyond BRCA1/2 identified PVs and LPVs in a further 2.9% of BC patients. In conclusion, panel testing yields more accurate genetic information for appropriate counselling, risk management, and preventive options than assessing BRCA1/2 alone.

Why do dogs look back at the human in an impossible task? Looking back behaviour may be over-interpreted.

Project description:The impossible task paradigm has been extensively used to study the looking back behaviour in dogs. This behaviour is commonly considered a social problem-solving strategy: dogs facing an unsolvable task, soon give up and look back at the experimenter to ask for help. We aimed to test if the looking back in an impossible task does indeed represent a social problem-solving strategy. We used a modified version of the classic impossible task, in which the subjects simultaneously faced three possible and one impossible trials. Additionally, subjects were tested in four different conditions: social condition (with an unknown experimenter); asocial condition (subject alone); 'dummy' human condition (with a 'dummy' human); object condition (with a big sheet of cardboard). Finally, we compared two populations of dogs differing in their experience of receiving help from humans: 20 pet dogs tested in their houses and 31 free-ranging dogs tested in Morocco. We found that the pet dogs and free-ranging dogs had similar persistence in interacting with the impossible task in all conditions. Moreover, subjects looked back with similar latencies at the human, at the dummy human and at the object. Overall, pet dogs looked back longer at the human than free-ranging dogs. This could be interpreted as pet dogs being more attracted to humans and/or having a stronger association between humans and food than free-ranging dogs. Concluding, the looking back in an impossible task does not represent a problem-solving strategy. This behaviour seems rather linked to the subject's persistence, to the salience of the stimuli presented, and potentially to the past reinforcement history.

Population Responses to Environmental Change: Looking Back, Looking Forward.

Project description:Over the past two decades, population researchers have engaged in a far-reaching and productive program of research on demographic responses to changes in the natural environment. This essay "looks back" to the origins of these developments, identifying pivotal agenda-setting moments in the 1990s and tracing the impact on contemporary research. The essay also "looks forward" to identify critical gaps and challenges that remain to be addressed and to set an agenda for future research on population responses to environmental change. It recommends that the multidimensionality of environmental contexts and change be fully embraced, long run as well as short term effects be investigated, variability in the effects of environmental change in relation to social institutions, policy implementation, and environmental context be examined, movement between contexts as well as change in situ as sources of environmental change be considered, and interconnections among demographic processes in response to environmental change be explored. Taking these steps will position demographers to contribute significantly to a larger and deeper understanding of environmental change and its consequences, locally, regionally, and globally.
