Immunization information systems: a decade of progress in law and policy.

ABSTRACT: This article reports on a study of laws, regulations, and policies governing Immunization Information Systems (IIS, also known as "immunization registries") in states and selected urban areas of the United States. The study included a search of relevant statutes, administrative codes and published attorney general opinions/findings, an online questionnaire completed by immunization program managers and/or their staff, and follow-up telephone interviews.The legal/regulatory framework for IIS has changed considerably since 2000, largely in ways that improve IIS' ability to perform their public health functions while continuing to maintain strict confidentiality and privacy controls. Nevertheless, the exchange of immunization data and other health information between care providers and public health and between entities in different jurisdictions remains difficult due in part to ongoing regulatory diversity.To continue to be leaders in health information exchange and facilitate immunization of children and adults, IIS will need to address the challenges presented by the interplay of federal and state legislation, regulations, and policies and continue to move toward standardized data collection and sharing necessary for interoperable systems.

Project description:BackgroundThe European Commission supports the initiation of health information related projects and networks serving comparative population health monitoring and health system performance assessment. Many of these projects and networks have produced relevant data, standards, methods, indicators and knowledge that may be lost as these networks become inactive. The aim of this project retrieval and review was to identify health information projects and networks and their produced output; and subsequently facilitate systematic access to this information for policy makers, researchers and interested others via a web-based repository.MethodsThe scope of this article covers 1. population health oriented topics and 2. health system/health services oriented topics. Out of scope are specific infectious diseases; individual rare diseases; and the occurrence and effects of specific medical treatments, interventions and diagnostics; cohort studies; or studies focusing on research methods. We searched bibliographic databases and EU project databases for policy supporting projects and networks and selected those fulfilling our inclusion criteria after more in-depth inspection. We searched for their outputs. In addition, we reviewed country participation in these projects and networks.ResultsWe identified 36 projects and networks, 16 of which are population health oriented, 6 are health systems and services oriented and 14 cover both. Their total volume of output is not easily retrievable, as many project websites have been discontinued. Some networks and/or their outputs have found continuance within European agencies and/or national institutions. Others are struggling or have gone lost, despite their policy relevance. Participation in the projects was not evenly distributed across Europe. Project information was made available through the Health Information Portal.ConclusionsEU funded projects and networks have contributed greatly to the evidence-base for policy by providing comparative health information. However, more action is needed to evaluate and conserve their outputs and facilitate continued contribution to the field after project funding stops. The realization of a sustainable infrastructure for these projects and networks is urgent. The Health Information Portal can play an important role in conserving and reusing health information. Information inequalities may exist across Europe but need further investigating.

Project description:Background/objectivesCommunity pharmacies may not consistently check patients' immunization records from Immunization Information Systems (IIS) before administering vaccines. Therefore, this study aimed to identify key factors associated with vaccine records retrieval from IIS among community pharmacies.MethodsA cross-sectional survey of 9446 members of the United States National Community Pharmacists Association was conducted from September-November 2022. There were 492 responses (response rate 5.2 %) received. The analysis included 202 participants who provided immunization services in 2021, administered vaccines besides COVID-19 or influenza, and completed ≥80 % of the questionnaire. The dependent variable was participants' self-reported frequency of retrieving immunization records from IIS (categorized as high or low IIS users). The independent variables considered were participant/pharmacy characteristics, IIS knowledge and perceptions about innovation characteristics, process, and inner settings, as identified by the Consolidated Framework for Implementation Research. Exploratory factor analysis identified ten perception scales, and all had Cronbach's Alpha coefficient of >0.70.ResultsApproximately 65 % of the respondents were high IIS users ("frequently"/"always") while 35 % were low IIS users ("never"/"rarely"/"occasionally"). A bivariate analyses of the relationship between the dependent and independent variables indicated significant differences in job title, state requirement for documentation in IIS, and eight scales: perceived benefit of IIS in consolidating patient records, optimizing immunization service delivery process, perceived IIS usability, process of engaging staff in IIS implementation, leadership support in encouraging IIS utilization, team values, open communications, and organizational needs fulfillment. A multivariable logistic regression analysis revealed that process optimization (p = .009) and leadership support (p = .018) were the two significant predictors of being in the high IIS user group when accounting for other variables.ConclusionInterventions to encourage independent community pharmacies to retrieve vaccine records from IIS should focus on enhancing the efficiency of the IIS process and leadership support.
