Successful treatment of biofilm infections using shock waves combined with antibiotic therapy.

ABSTRACT: Many bacteria secrete a highly hydrated framework of extracellular polymer matrix on suitable substrates and embed within the matrix to form a biofilm. Bacterial biofilms are observed on many medical devices, endocarditis, periodontitis and lung infections in cystic fibrosis patients. Bacteria in biofilm are protected from antibiotics and >1,000 times of the minimum inhibitory concentration may be required to treat biofilm infections. Here, we demonstrated that shock waves could be used to remove Salmonella, Pseudomonas and Staphylococcus biofilms in urinary catheters. The studies were extended to a Pseudomonas chronic pneumonia lung infection and Staphylococcus skin suture infection model in mice. The biofilm infections in mice, treated with shock waves became susceptible to antibiotics, unlike untreated biofilms. Mice exposed to shock waves responded to ciprofloxacin treatment, while ciprofloxacin alone was ineffective in treating the infection. These results demonstrate for the first time that, shock waves, combined with antibiotic treatment can be used to treat biofilm infection on medical devices as well as in situ infections.

Project description:BackgroundVibrio vulnificus (V. vulnificus) is a halophilic marine Gram-negative bacterium. Necrotizing fasciitis caused by V. vulnificus is a rapidly progressing clinical emergency often accompanied by septic shock. Despite advances in antibiotics and infection control measures, it remains a highly fatal and disabling infection. The incidence of V. vulnificus infection has increased due to climate warming and expanded global seafood trade in recent years. However, pediatric cases of V. vulnificus infection remain rare, leading to limited clinical experience in their management.MethodsThis report analyzes the clinical data of a pediatric case of V. vulnificus necrotizing fasciitis with septic shock, treated at Zhuhai Center for Maternal and Child Health Care in April 2024. The report also reviews the literature on pediatric V. vulnificus infection.ResultsA 26-month-old boy developed a V. vulnificus infection after being scratched by a sea bass. The patient experienced an acute onset of illness that quickly worsened, presenting with a fever, mental fatigue, soft tissue edema, and pain, necrosis of the fascia and foot, coagulation dysfunction, and even shock. Laboratory results revealed white blood cell count(5.0 × 109/L), neutrophilia %(65%), thrombocytopenia (56 × 109/L), elevated CRP (200 mg/L), PCT (67.4 ng/mL), and IL-6 (> 4000 pg/mL), hypoalbuminemia (17.4 g/L), prolonged PT (17.5 s), reduced total T and NK cell counts, and a significantly reduced proportion of Treg cells. Initial treatment included surgical debridement and drainage, empirical antibiotic therapy, and rapid diagnosis of V. vulnificus via bacterial wound culture. Next-generation sequencing (NGS) of the blood microbial macrogenome and high-throughput sequencing of wound microbial pathogens (MetCap) were performed. Antibiotics were selected based on drug sensitivity testing, accompanied by hemopurification and measures to prevent disseminated intravascular coagulation. The patient's condition stabilized gradually post-treatment, and he was discharged.ConclusionPrompt diagnosis is important for children with seafood exposure. Early hemopurification, surgical intervention, effective antibiotic therapy, and DIC prevention significantly improve prognosis and survival rates. These findings provide a practical reference for managing pediatric V. vulnificus infections.

Project description:Staphylococcus aureus infections, particularly those from methicillin-resistant strains (i.e., MRSA), are reaching epidemic proportions, with no effective vaccine available. The vast number and transient expression of virulence factors in the infectious course of this pathogen have made the discovery of protective antigens particularly difficult. In addition, the divergent planktonic and biofilm modes of growth with their accompanying proteomic changes also demonstrate significant hindrances to vaccine development. In this study, a multicomponent vaccine was evaluated for its ability to clear a staphylococcal biofilm infection. Antigens (glucosaminidase, an ABC transporter lipoprotein, a conserved hypothetical protein, and a conserved lipoprotein) were chosen since they were found in previous studies to have upregulated and sustained expression in a biofilm, both in vitro and in vivo. Antibodies against these antigens were first used in microscopy studies to localize their expression in in vitro biofilms. Each of the four antigens showed heterogeneous production in various locations within the complex biofilm community in the biofilm. Based upon these studies, the four antigens were delivered simultaneously as a quadrivalent vaccine in order to compensate for this varied production. In addition, antibiotic treatment was also administered to clear the remaining nonattached planktonic cells since the vaccine antigens may have been biofilm specific. The results demonstrated that when vaccination was coupled with vancomycin treatment in a biofilm model of chronic osteomyelitis in rabbits, clinical and radiographic signs of infection significantly reduced by 67 and 82%, respectively, compared to infected animals that were either treated with vancomycin or left untreated. In contrast, vaccination alone resulted in a modest, and nonsignificant, decrease in clinical (34% reduction) and radiographic signs (9% reduction) of infection, compared to nonvaccinated animal groups untreated or treated with vancomycin. Lastly, MRSA biofilm infections were significantly cleared in 87.5% of vaccinated and antibiotic-treated animals, while antibiotics or vaccine alone could not significantly clear infection compared to controls (55.6, 22.2, and 33.3% clearance rates, respectively). This approach to vaccine development may lead to the generation of vaccines against other pathogenic biofilm bacteria.
