Project description:BackgroundNigeria accounts for substantial proportions of global malaria infections and deaths, with children aged younger than 5 years being the most affected group. This suggests that access to lifesaving malaria interventions could be suboptimal, especially at public health facilities where most rural dwellers seek health care. We conducted this study to ascertain if public health facilities have the commodities and the robust supply chain management (SCM) system required to deliver malaria interventions to children younger than 5 years.MethodWe conducted a cross-sectional survey in 1,858 health facilities across 7 states in Nigeria. Using structured questionnaires, we assessed the availability of selected malaria commodities required by children aged younger than 5 years. We also interviewed health workers to evaluate other core SCM activities.ResultMore than 50% of health facilities in 5 states were stocked out of malaria rapid diagnostic tests (mRDTs), and stock-out rates for artemisinin-based combination therapies (ACTs) were over 50% for almost all assessed ACTs across all states. The percentage of health facilities that received malaria commodities within the recommended lead time was below average across most states (71%). States with a higher percentage of health workers who were aware of and placed orders following the national reporting timeline and those that delivered commodities to the last mile predominantly through third-party logistics service providers tended to have higher availability of mRDTs and artemether/lumefantrine combinations. The top 2 logistics challenges were insecurity and inadequate funding.ConclusionThe availability of lifesaving malaria commodities across the health facilities visited was suboptimal, possibly due to several SCM challenges. The results from this study underscore the urgent need to implement effective interventions to address the observed gaps. This will contribute to reducing malaria morbidity and mortality among children aged younger than 5 years in Nigeria.

Project description:BackgroundThe New Zealand (NZ) Government has made a strong commitment to reduce the incidence of rheumatic fever (RF) by two thirds, to 1.4 cases per 100,000, by mid-2017. We reviewed the NZ RF surveillance sector, aiming to identify potential improvements which would support optimal RF control and prevention activities.MethodsThis review used a recently developed surveillance sector review method. Interviews with 36 key informants were used to describe the sector, assess it and identify its gaps. Priorities for improvement and implementation strategies were determined following discussion with these key informants, with policy advisors and within the research team.ResultsKey improvements identified included the need for a comprehensive RF surveillance strategy, integrated reporting and an online national RF register. At a managerial level this review provided evidence for system change and built support for this across the surveillance sector.ConclusionsThe surveillance sector review approach can be added to the small set of tools currently available for developing and evaluating surveillance systems. This new approach is likely to prove useful as we confront the challenges of combating new emerging infectious diseases, responding to global environmental changes, and reducing health inequalities.

Project description:BackgroundWomen with a prior gestational diabetes have an increased lifetime risk of developing type 2 diabetes. Although post-partum follow-up for GDM women is essential to prevent progression to type 2 diabetes, it is poorly attended. The need for health systems interventions to support postpartum follow-up for GDM women is evident, but there is little knowledge of actual current practice. The aim of this study was to explore current policies and clinical practices relating to antenatal and post-natal care for women with GDM in South Africa, as well as health sector stakeholders' perspectives on the barriers to -- and opportunities for -- delivering an integrated mother - baby health service that extends beyond the first week post-partum, to the infant's first year of life.MethodsFollowing a document review of policy and clinical practice guidelines, in-depth interviews were conducted with 11 key informants who were key policy makers, health service managers and clinicians working in the public health services in South Africa's two major cities (Johannesburg and Cape Town). Data were analysed using qualitative content analysis procedures.ResultsThe document review and interviews established that it is policy that health services adhere to international guidelines for GDM diagnosis and management, in addition to locally developed guidelines and protocols for clinical practice. All key informants confirmed that lack of postpartum follow-up for GDM women is a significant problem. Health systems barriers include fragmentation of care and the absence of standardised postnatal care for post-GDM women. Key informants also raised patient - related challenges including lack of perceived future risk of developing type 2 diabetes and non-attendance for postpartum follow up, as barriers to postnatal care for GDM women. All participants supported integrated primary health services but cautioned against overloading health workers.ConclusionAlthough there is alignment between international guidelines, local policy and reported clinical practice in the management of GDM, there is a gap in continuation of care in the postpartum period. Health systems interventions that support and facilitate active follow-up for women with prior GDM are needed if high rates of progression to type 2 diabetes are to be avoided.

Project description:We use information on management practices in 1,183 hospitals in 7 different countries, collected in 2010 within the "World Management Survey" initiative, to estimate the role of public ownership on different management dimensions, such as monitoring performance, setting targets and incentivizing employees. A significant variation in management practices both between countries and, within countries, across hospitals is found. We show that managers in public sector hospitals tend to underperform, relative to private hospitals, in all the countries considered. Larger hospitals appear to be better managed, while there is no difference between teaching and other type of hospitals. Publicly owned hospitals appear less efficient in the provision of incentive schemes to promote and reward highly motivated employees, or remove poor performers. Overall, public ownership is associated with a reduction of about 10% in management score, which corresponds approximately to a half-standard deviation.

Project description:BackgroundIn the healthcare sector, the quality of medical services largely depends on the work of medical staff. Improving employee performance can impact the efficiency and productivity of a healthcare entity. Therefore, proper talent management practices are needed to achieve good outcomes. It is also worth answering what other factors affect this performance. The aim of the study was to investigate the impact of talent management practices and other factors on employee performance in the public healthcare sector.MethodsData was collected using a questionnaire method with a Likert scale. The study population consisted of 558 employees in the public Polish healthcare sector. The research instrument's structure, reliability, and validity were assessed using exploratory and confirmatory factor analysis. Descriptive analysis was used to analyze the structure of respondents, and a covariance-based structural equation modelling (CB-SEM) was used to test the model and verify hypotheses. Survey data were analyzed using SPSS v8 and AMOS v. 29 (Predictive Solution Poland).ResultsIn the structural equation model, CFI = 0.997, GFI = 0.992, AGFI = 0.973, and RMSEA = 0.033, showing the model has a good fit. Significant relationships were found between job mobility and employee performance (β = 0.195, p < 0.001) between talent management practices and employee performance (β = 0.246, p < 0.001). This analysis also showed a positive, significant and strong relationship between age and employee performance (β = 0.230; ρ value < 0.001).ConclusionsThe structural equation model showed that talent management practices consisting of talent attraction, talent development, and talent evaluation had a significant and positive impact on medical staff performance. We also found that, in addition to these three talent management practices, such factors as job mobility, and age were significant predictors of employee performance. It is, therefore, important to create formal talent management processes that support personal development and appreciate the individual performance of employees.

Project description:BackgroundDengue fever is a rapidly emerging infection throughout the tropics and subtropics with extensive public health burden. Adequate training of healthcare providers is crucial to reducing infection incidence through patient education and collaboration with public health authorities. We examined how public sector healthcare providers in a dengue-endemic region of Ecuador view and manage dengue infections, with a focus on the 2009 World Health Organization (WHO) Dengue Guidelines.MethodsA 37-item questionnaire of dengue knowledge, attitudes, and practices was developed and administered to dengue healthcare providers in Machala, Ecuador. Survey focus areas included: "Demographics," "Infection and Prevention of Dengue," "Dengue Diagnosis and the WHO Dengue Guide," "Laboratory Testing," "Treatment of Dengue," and "Opinions Regarding Dengue."ResultsA total of 76 healthcare providers participated in this study, of which 82 % were medical doctors and 14 % were nurses. Fifty-eight percent of healthcare professionals practiced in ambulatory clinics and 34 % worked in a hospital. Eighty-nine percent of respondents were familiar with the 2009 WHO Dengue Guidelines, and, within that group, 97 % reported that the WHO Dengue Guide was helpful in dengue diagnosis and clinical management. Knowledge gaps identified included Aedes aegypti mosquito feeding habits and dengue epidemiology. Individuals with greater dengue-related knowledge were more likely to consider dengue a major health problem. Only 22 % of respondents correctly reported that patients with comorbidities and dengue without warning signs require hospital admission, and 25 % of providers reported never admitting patients with dengue to the hospital. Twenty percent of providers reported rarely (≤25 % of cases) obtaining laboratory confirmation of dengue infection. Providers reported patient presumptive self-medication as an ongoing problem. Thirty-one percent of healthcare providers reported inadequate access to resources needed to diagnose and treat dengue.ConclusionParticipants demonstrated a high level of knowledge of dengue symptoms and treatment, but additional training regarding prevention, diagnosis, and admission criteria is needed. Interventions should not only focus on increasing knowledge, but also encourage review of the WHO Dengue Guidelines, avoidance of presumptive self-medication, and recognition of dengue as a major health problem. This study provided an assessment tool that effectively captured healthcare providers' knowledge and identified critical gaps in practice.

Project description:In South Africa, primary eye care is largely challenged in its organisational structure, availability of human and other resources, and clinical competency. These do meet the standard required by the National Department of Health. This study seeks to assess the levels of knowledge, attitudes, and practices on eye health amongst Human Resources for eye health (HReH) and their managers, as no study has assessed this previously. A cross-sectional study was conducted in 11 districts of a South African province. A total of 101 participants completed self-administered, close-ended, Likert-scaled questionnaires anonymously. Binary logistic regression analysis was conducted, and values of p < 0.05 were considered statistically significant. Most participants had adequate knowledge (81.6%), positive attitudes (69%), and satisfactory practices (73%) in eye health. HReH showed better knowledge than their managers (p < 0.01). Participants with a university degree, those aged 30-44 years, and those employed for <5 years showed a good attitude (p < 0.05) towards their work. Managers, who supervise and plan for eye health, were 99% less likely to practice adequately in eye health when compared with HReH (aOR = 0.012; p < 0.01). Practices in eye health were best amongst participants with an undergraduate degree, those aged 30-44 years (aOR = 2.603; p < 0.05), and participants with <5 years of employment (aOR = 26.600; p < 0.01). Knowledge, attitudes, and practices were found to be significantly moderately correlated with each other (p < 0.05). Eye health managers have poorer knowledge and practices of eye health than the HReH. A lack of direction is presented by the lack of adequately trained directorates for eye health. It is therefore recommended that policymakers review appointment requirements to ensure that adequately trained and qualified directorates be appointed to manage eye health in each district.

Project description:IntroductionProlificacy has become an important breeding goal in sheep farming to increase farm profitability. With the adoption of improved genetics and management practices leading to increased lambing percentages, the proportion of triplet-born lambs has also increased on farms. However, mortality rates of triplet lambs are higher than for single- and twin-born lambs, and additional management inputs may be needed to support survival. The aim of this study was to identify factors that affect management practices that are considered important for triplet lamb survival by commercial farmers from the United Kingdom (UK), the Ireland (IRE), and New Zealand (NZ).MethodsAn online survey was developed and disseminated to farmers in each country, focusing on farmer demographics, flock characteristics, management practices and production outcomes. A total of 448 farmers completed the survey, from the UK (n = 168), IRE (n = 218), and NZ (n = 62).ResultsRespondents had larger flocks, higher scanning and lambing percentages than the country average for the UK and IRE. The mean percentage of triplet litters born within flocks was 9%, and lambs lost between scanning and lambing were 14% for UK, 15% for IRE, and 25% for NZ respondents (P = 0.063). Overall, 60% of all respondents reported to lamb indoors and 40% lambed outdoors, however NZ farmers almost exclusively lambed outdoors, whereas UK and IRE farmers lambed in both systems (P < 0.001). NZ farmers were more likely to rear all triplet lambs with the ewe, whereas UK and IRE farmers were more likely to remove a lamb to rear by another ewe or artificially (P < 0.001). Factors that influenced triplet lamb management practices of respondents in this study were respondent country of origin, flock size, age, and gender. In general, younger respondents (P < 0.001), and female respondents (P < 0.05), were more likely to engage in management activities that were considered to promote better triplet lamb survival, compared to older and male respondents respectively. These practices were associated with better lamb survival reported by respondents but were less likely to be carried out when flock size increased (P < 0.001).DiscussionThe results of this survey highlight future priorities or communication strategies needed to improve triplet lamb survival.

Project description:The rationale for this article is that often, decision-makers in waste management (wm) tend to neglect goals and confuse them with means like circular economy or waste hierarchy. Because clear goals are crucial for developing effective wm strategies, the objectives of this mini review are (1) to clarify wm goals in a historical context by a literature review, (2) to investigate how (a) these goals have been observed in general scientific publishing and (b) specifically in Waste Management and Research (WM&R) and (3) to recommend measures for better consideration of wm goals by the publication sector. Based on general as well as specific bibliographic analyses of databases in Scopus and Google Scholar, the study confirms that little attention was given to wm goals in scientific publishing. For instance, during the first 40 years of WM&R, 63 publications and eight editorials were found containing terms related to wm goals, but only 14 respectively and eight explicitly discuss wm goals. We recommend focussing more on wm goals. Editors, authors, reviewers and professional associations in the field of wm should become aware of this challenge and react. If WM&R decides to become a strong platform for the issue wm goals, it will be in a unique selling proposition and more authors, articles and readers are likely to result. This article aims at setting a starting signal for such an endeavour.

Project description:BackgroundSupportive supervision has been found to be more effective than corrective fault-oriented inspections. Uganda's Ministry of Health in 2012 implemented a comprehensive strategy (SPARS) to build medicines management capacity in public sector health facilities. The approach includes supportive supervision. This structured observational study assesses supportive supervision competency among medicines management supervisors (MMS).MethodThe study used structured observations of two groups of five purposely selected MMS-one group supervising facilities with greater medicines management improvement during one year of SPARS and one group with less improvement, based on quantitative metrics. We observed and scored behaviors and skills of supervisors in 11 categories deemed critical for effective and supportive supervision.ResultsSupportive supervision was not evenly or adequately implemented, with the median supportive supervision competency score for all observed supervisors being 38%. Supervisors' main strengths were problem identification, data interpretation, education, and providing constructive feedback (45%-47%). Their weakest areas were assuring continuity and setting targets (17%), and most MMS were fair to strong in effective communication, use of tools, and problem solving. MMS of facilities with little improvement in medicines management over time were weak in setting targets and promoting participation. There was a 33 percentage point difference in the median supportive supervision competency scores between MMS of facilities with more versus less improvement (57%-24%) and a 77 percentage point difference in competency between the highest and lowest scoring MMS (77%-0%). We did not find a relationship between MMS experience (number of visits implemented) and their supportive supervision competency or facility improvement in medicines management. However, there is a likely relationship between supportive supervision competency and facility improvement.ConclusionCompetency of MMS in supportive supervision among the sampled MMS was generally weak, but with much individual variation. Our results suggest that MMS' supportive supervision competency is positively related to the SPARS effectiveness scores of the facilities they supervise. We recommend strategies to strengthen supportive supervision behaviors and skills.
