Characterizing treatment pathways at scale using the OHDSI network.

ABSTRACT: Observational research promises to complement experimental research by providing large, diverse populations that would be infeasible for an experiment. Observational research can test its own clinical hypotheses, and observational studies also can contribute to the design of experiments and inform the generalizability of experimental research. Understanding the diversity of populations and the variance in care is one component. In this study, the Observational Health Data Sciences and Informatics (OHDSI) collaboration created an international data network with 11 data sources from four countries, including electronic health records and administrative claims data on 250 million patients. All data were mapped to common data standards, patient privacy was maintained by using a distributed model, and results were aggregated centrally. Treatment pathways were elucidated for type 2 diabetes mellitus, hypertension, and depression. The pathways revealed that the world is moving toward more consistent therapy over time across diseases and across locations, but significant heterogeneity remains among sources, pointing to challenges in generalizing clinical trial results. Diabetes favored a single first-line medication, metformin, to a much greater extent than hypertension or depression. About 10% of diabetes and depression patients and almost 25% of hypertension patients followed a treatment pathway that was unique within the cohort. Aside from factors such as sample size and underlying population (academic medical center versus general population), electronic health records data and administrative claims data revealed similar results. Large-scale international observational research is feasible.

Project description:ObjectiveSupplementing the Spontaneous Reporting System (SRS) with Electronic Health Record (EHR) data for adverse drug reaction detection could augment sample size, increase population heterogeneity and cross-validate results for pharmacovigilance research. The difference in the underlying data structures and terminologies between SRS and EHR data presents challenges when attempting to integrate the two into a single database. The Observational Health Data Sciences and Informatics (OHDSI) collaboration provides a Common Data Model (CDM) for organizing and standardizing EHR data to support large-scale observational studies. The objective of the study is to develop and evaluate an informatics platform known as ADEpedia-on-OHDSI, where spontaneous reporting data from FDA's Adverse Event Reporting System (FAERS) is converted into the OHDSI CDM format towards building a next generation pharmacovigilance signal detection platform.MethodsAn extraction, transformation and loading (ETL) tool was designed, developed, and implemented to convert FAERS data into the OHDSI CDM format. A comprehensive evaluation, including overall ETL evaluation, mapping quality evaluation of drug names to RxNorm, and an evaluation of transformation and imputation quality, was then performed to assess the mapping accuracy and information loss using the FAERS data collected between 2012 and 2017. Previously published findings related to vascular safety profile of triptans were validated using ADEpedia-on-OHDSI in pharmacovigilance research. For the triptan-related vascular event detection, signals were detected by Reporting Odds Ratio (ROR) in high-level group terms (HLGT) level, high-level terms (HLT) level and preferred term (PT) level using the original FAERS data and CDM-based FAERS respectively. In addition, six standardized MedDRA queries (SMQs) related to vascular events were applied.ResultsA total of 4,619,362 adverse event cases were loaded into 8 tables in the OHDSI CDM. For drug name mapping, 93.9% records and 47.0% unique names were matched with RxNorm codes. Mapping accuracy of drug names was 96% based on a manual verification of randomly sampled 500 unique mappings. Information loss evaluation showed that more than 93% of the data is loaded into the OHDSI CDM for most fields, with the exception of drug route data (66%). The replication study detected 5, 18, 47 and 6, 18, 50 triptan-related vascular event signals in MedDRA HLGT level, HLT level, and PT level for the original FAERS data and CDM-based FAERS respectively. The signal detection scores of six standardized MedDRA queries (SMQs) of vascular events in the raw data study were found to be lower than those scores in the CDM study.ConclusionThe outcome of this work would facilitate seamless integration and combined analyses of both SRS and EHR data for pharmacovigilance in ADEpedia-on-OHDSI, our platform for next generation pharmacovigilance.

Project description:Background and purposeHemorrhagic transformation (HT) after cerebral infarction is a complex and multifactorial phenomenon in the acute stage of ischemic stroke, and often results in a poor prognosis. Thus, identifying risk factors and making an early prediction of HT in acute cerebral infarction contributes not only to the selections of therapeutic regimen but also, more importantly, to the improvement of prognosis of acute cerebral infarction. The purpose of this study was to develop and validate a model to predict a patient's risk of HT within 30 days of initial ischemic stroke.MethodsWe utilized a retrospective multicenter observational cohort study design to develop a Lasso Logistic Regression prediction model with a large, US Electronic Health Record dataset which structured to the Observational Medical Outcomes Partnership (OMOP) Common Data Model (CDM). To examine clinical transportability, the model was externally validated across 10 additional real-world healthcare datasets include EHR records for patients from America, Europe and Asia.ResultsIn the database the model was developed, the target population cohort contained 621,178 patients with ischemic stroke, of which 5,624 patients had HT within 30 days following initial ischemic stroke. 612 risk predictors, including the distance a patient travels in an ambulance to get to care for a HT, were identified. An area under the receiver operating characteristic curve (AUC) of 0.75 was achieved in the internal validation of the risk model. External validation was performed across 10 databases totaling 5,515,508 patients with ischemic stroke, of which 86,401 patients had HT within 30 days following initial ischemic stroke. The mean external AUC was 0.71 and ranged between 0.60-0.78.ConclusionsA HT prognostic predict model was developed with Lasso Logistic Regression based on routinely collected EMR data. This model can identify patients who have a higher risk of HT than the population average with an AUC of 0.78. It shows the OMOP CDM is an appropriate data standard for EMR secondary use in clinical multicenter research for prognostic prediction model development and validation. In the future, combining this model with clinical information systems will assist clinicians to make the right therapy decision for patients with acute ischemic stroke.

Project description:BackgroundPredictive models show promise in healthcare, but their successful deployment is challenging due to limited generalizability. Current external validation often focuses on model performance with restricted feature use from the original training data, lacking insights into their suitability at external sites. Our study introduces an innovative methodology for evaluating features during both the development phase and the validation, focusing on creating and validating predictive models for post-surgery patient outcomes with improved generalizability.MethodsElectronic health records (EHRs) from 4 countries (United States, United Kingdom, Finland, and Korea) were mapped to the OMOP Common Data Model (CDM), 2008-2019. Machine learning (ML) models were developed to predict post-surgery prolonged opioid use (POU) risks using data collected 6 months before surgery. Both local and cross-site feature selection methods were applied in the development and external validation datasets. Models were developed using Observational Health Data Sciences and Informatics (OHDSI) tools and validated on separate patient cohorts.ResultsModel development included 41 929 patients, 14.6% with POU. The external validation included 31 932 (UK), 23 100 (US), 7295 (Korea), and 3934 (Finland) patients with POU of 44.2%, 22.0%, 15.8%, and 21.8%, respectively. The top-performing model, Lasso logistic regression, achieved an area under the receiver operating characteristic curve (AUROC) of 0.75 during local validation and 0.69 (SD = 0.02) (averaged) in external validation. Models trained with cross-site feature selection significantly outperformed those using only features from the development site through external validation (P < .05).ConclusionsUsing EHRs across four countries mapped to the OMOP CDM, we developed generalizable predictive models for POU. Our approach demonstrates the significant impact of cross-site feature selection in improving model performance, underscoring the importance of incorporating diverse feature sets from various clinical settings to enhance the generalizability and utility of predictive healthcare models.

Project description:BackgroundComplex diseases may have multiple pathways leading to disease. E.g. coronary artery disease evolves from arterial damage to their epithelial layers, but has multiple causal pathways. More challenging, those pathways are highly correlated within metabolic syndrome. The challenge is to identify specific clusters of phenotype characteristics (composite phenotypes) that may reflect these different etiologies. Further, GWAS seeking to identify SNPs satisfying multiple composite phenotype descriptions allows for lower false positive rates at lower α thresholds, allowing for the possibility of reducing false negatives. This may provide a window into the missing heritability problem.MethodsWe identify significant phenotype patterns, and identify fuzzy redescriptions among those patterns using Jaccard distances. Further, we construct Vietoris-Rips complexes from the Jaccard distances and compute the persistent homology associated with those. The patterns comprising these topological features are identified as composite phenotpyes, whose genetic associations are explored with logistic regression applied to pathways and to GWAS.ResultsWe identified several phenotypes that tended to be dominated by metabolic syndrome descriptions, and which were distinct among the combinations of metabolic syndrome conditions. Among SNPs marking the RAAS complex, various SNPs associated specifically with different groups of composite phenotypes, as well as distinguishing between the composite phenotypes and simple phenotypes. Each of these showed different genetic associations, namely rs6693954, rs762551, rs1378942, and rs1133323. GWAS identified SNPs that associated with composite phenotypes included rs12365545, rs6847235, and rs701319. Eighteen GWAS identified SNPs appeared in combinations supported in composite combinations with greater power than for any individual phenotype.ConclusionsWe do find systematic associations among metabolic syndrome variates that show distinctive genetic association profiles. Further, the systematic characterization involves composite phenotype descriptions that allow for combined power of individual phenotype GWAS tests, yielding more significance for lower individual thresholds, permitting the exploration of SNPs that would otherwise show as false negatives.
