Transesophageal echocardiographic guidance for percutaneous closure of aortic pseudoaneurysm using a type II Amplatzer vascular plug: a case report.

ABSTRACT: Aortic pseudoaneurysm after cardiac surgery is a rare entity, but it is potentially fatal due to its clinical course along with higher morbidity and mortality rates. Instead of open surgical repair, percutaneous procedures have been introduced as other options for managing an aortic pseudoaneurysm. In this case report, we describe transesophageal echocardiography guidance for successful percutaneous closure of an aortic pseudoaneurysm located in the left ventricular outflow tract by using a type II Amplatzer vascular plug in a patient in whom open surgical repair was not recommended.

Project description:Vessel embolization can be a valuable adjunct procedure in transjugular intrahepatic portosystemic shunt (TIPS). During the creation of a TIPS, embolization of portal vein collaterals supplying esophageal varices may lower the risk of secondary rebleeding. And after creation of a TIPS, closure of the TIPS itself may be indicated if the resulting hepatic encephalopathy severely impairs mental functioning. The Amplatzer Vascular Plug (AVP; AGA Medical, Golden Valley, MN) is well suited for embolization of large-diameter vessels and has been employed in a variety of vascular lesions including congenital arteriovenous shunts. Here we describe the use of the AVP in the context of TIPS to embolize portal vein collaterals (n = 8) or to occlude the TIPS (n = 2).

Percutaneous closure of aortic pseudoaneurysm in a young female with atrial septal occluder.

Project description:This case report represents a novel technique for the treatment of a pseudoaneurysm of the aorta. Pseudoaneurysm of the aorta has been reported in patients post heart surgery. This case report is about a patient who had a pseudoaneurysm most probably following tuberculosis. Traditionally, the treatment of choice is surgical correction; however, in the current era, there are case reports describing the use of either stent grafts or Amplatzer occluders for occlusion of the pseudoaneurysm in high-risk surgical cases. We performed successful closure of the aortic pseudoaneurysm using atrial septal occluder. <Learning objective: Tuberculous arteritis can result in pseudoaneurysm rarely. Our patient had a large aneurysm compressing adjoining vital structures and was very high surgical risk case. This is the first case report of closure of tuberculous pseudoaneurysm by using atrial septal occluder in a young patient. In this case we have demonstrated that large pseudoaneurysms with narrow neck can be closed very safely using atrial septal occluder with immediate symptom relief and thrombosis of pseudoaneurysm at follow-up of three months.>.

Case Report: The application of amplatzer vascular plug to repair aortic dissection intimal tears and false lumen.

Project description:In recent years, significant advancements have been made in endovascular therapy for aortic dissection, resulting in the development of various treatment methods. Nevertheless, there is a contentious discussion regarding the suitability of different treatment methods for addressing the unique features of individual lesions. Specifically, the matter of whether the presence of small intimal tears or multiple distal tears requires extensive aortic coverage with graft stents, which may block the blood supply to important organs, intercostal arteries, or lumbar arteries, remains unresolved. Further research is necessary to determine the need for complex fenestrated or branched endovascular aortic repair (F/B-EVAR) in the management of chronic post-dissection aneurysms involving visceral arteries. Our report highlights the successful endovascular repair of three patients with aortic dissection, encompassing one Stanford A type and two Stanford B types. Utilizing the Amplatzer Vascular Plug (AVP) alone or in combination with the coil embolization technique, we achieved favorable clinical outcomes. This article aims to provide valuable insights and new perspectives on the tailored management of aortic dissection by conducting a comprehensive examination of the subtle differences in the treatment processes and techniques applied to these three patients.

Percutaneous closure of an uncommon aortic pseudoaneurysm after arterial switch repair: a case report.

Project description:BackgroundThe development of an aortic pseudoaneurysm is a rather rare but potentially fatal complication after cardiac surgery for aortic valve and aorta disease. If a pseudoaneurysm is left untreated, it carries a substantial risk of rupture, thrombosis with subsequently systemic embolization, and compression of the surrounding structures.Case summaryWe describe a case of a transcatheter repair of a more complex and uncommon pseudoaneurysm following aortic valve replacement in a patient with a history of arterial switch repair for transposition of the great arteries. The pseudoaneurysm originated from the aortic wall and connected to the left ventricular outflow tract (LVOT). The connection to LVOT was closed with a duct occluder, the neck to the aortic wall with an atrial septal defect occluder. After 1 month, the complete pseudoaneurysm was successfully thrombosed.DiscussionPercutaneous closure of a complex pseudoaneurysm after arterial switch repair is feasible and safe. However, lifelong follow-up is needed to determine the late results after transcatheter interventions.

Single center experience of transcatheter closure of mitral and aortic Paravalvular leaks using the new rechristened rectangular Amplatzer PVL plug.

Project description:Amplatzer Paravalvular leak (PVL) plug is rectangular in shape, that might suit closure of crescentic PVL. Among 79 transcatheter PVL closures from a single center, a subgroup of 16 patients who received Amplatzer PVL plugs were analyzed. All procedures were successful, as the plug auto-oriented to the leak, without mechanical leaflet interference, though needing additional 31 devices. Two patients needed an elective re-intervention. NYHA class improved from III-IV before procedure to less than II after procedure. Complications were not directly related to the Amplatzer PVL plug. The rectangular shape for crescentic leaks, auto-orientation and non-interference with mechanical leaflets were good attributes.
