6MWT Performance and its Correlations with VO₂ and Handgrip Strength in Home-Dwelling Mid-Aged and Older Chinese.

ABSTRACT: Six-minute walk test (6MWT) performance is more commonly used in clinic patients with chronic cardiopulmonary diseases but not in home-dwelling individuals of similar age, and its correlations with oxygen uptake (VO₂) and muscle strength require further investigation. The current study determined the 6MWT performance of 106 home-dwelling residents (mean age of 62 years) in Suzhou, China. VO₂ at a respiratory exchange ratio (R) of 1 was measured through graded cycling exercise tests on 46 participants. Handgrip strength of all participants was tested. 6MWT distance measured 543.4 ± 67.2 m (total work 351.0 ± 62.8 kJ) with similar distances ambulated each minute. Heart rate, blood pressure, and rate of perceived exertion scores significantly increased after 6MWT. VO₂ at R = 1 reached 1238 ± 342 mL/min (18.6 ± 4.7 mL/kg/min), whereas handgrip strength totaled 29.8 ± 9.6 kg. 6MWT distance showed strong correlations with VO₂ (r = 0.549, p ≤ 0.001) and handgrip strength (r = 0.359, p < 0.001). Aside from providing reference values for 6MWT performance (~543 m, ~559 m in males and ~533 in females) for home-dwelling Chinese residents, our results suggest that as a parameter of exercise endurance, 6MWT performance correlates with both aerobic capacity and muscle fitness.

Project description:BackgroundIncreased arterial stiffness and low handgrip strength (HGS) are associated with poor health outcomes and are a severe health risk for older adults. However, there is limited evidence and mixed results on whether there is an association between them. Therefore, this study focused on the association between arterial stiffness and HGS in relatively healthy older adults in Beijing, China.MethodsIn 2016, 2,217 adult volunteers were recruited in Beijing. Brachial-ankle pulse wave velocity (baPWV) and the ankle-brachial index were measured using an automatic vascular profiling system. Carotid artery intima-media thickness and common carotid artery-internal diameter (CCAID) were evaluated using Doppler ultrasound, and HGS was measured with a dynamometer. Low HGS was determined using the Asian Sarcopenia Working Group 2019 criteria. Multivariate linear and logistic regressions evaluated the relationship between arterial stiffness and HGS.ResultsUltimately, 776 relatively healthy older adults (mean age 69.05 ± 6.46 years) were included. Based on the AWGS2019 criteria, 137 participants were defined as having low HGS. Compared to the normal HGS group, the low HGS group was older and had higher baPWV (p < 0.001) but lower CCAID, body mass index (BMI) and hemoglobin (Hb) (p < 0.05). The multiple linear regression analysis revealed that baPWV was negatively correlated with HGS (β = -0.173, t = -2.587, p = 0.01). Multivariate logistic regression analysis showed that baPWV and CCAID were associated with an increased risk of low HGS (odds ratio (OR) per SD increase: 1.318, p = 0.007; OR per SD increase: 0.541, p < 0.001).ConclusionArterial stiffness and HGS were significantly negatively correlated in relatively healthy Chinese older adults. Low HGS is associated with increased arterial stiffness. Encouraging exercise training to improve HGS, thereby reducing arterial stiffness and the risk of cardiovascular events, may be a simple and effective intervention.

Project description:BackgroundNeurodegenerative disorders, as the irreversible condition, have a long, silent preclinical period. Recognition of early physical signs of neurodegenerative disorders had important practical implications for identifying at-risk population. The aim of this study was to investigate whether handgrip strength (HGS) asymmetry was associated with the incidence of neurodegenerative disorders among Chinese older adults.MethodsThis study used the data of participants aged 60 years and over from three waves (2011-2015) of China Health and Retirement Longitudinal Study. HGS asymmetry was measured with HGS ratio (maximal non-dominant HGS/maximal dominant HGS), with the value less than 0.9 or more than 1.1 considered as HGS asymmetry. Physician-diagnosed neurodegenerative disorders were identified by self-reported or proxy-reported information. Competing risk analysis was conducted to examine the association between HGS asymmetry and incident neurodegenerative disorders, with mortality treated as the competing event.ResultsA total of 4925 participants were included in the analysis [mean (SD) age: 68.1(6.68); female: 49.7%]. Eight hundred and eighty-eight (18.0%) participants had low HGS and 2227 (45.2%) had HGS asymmetry. During the 4 years of follow-up, there were 156 cases of neurodegenerative disorders and 422 cases of mortality. The incidence of neurodegenerative disorders was 8.7 per 1000 person-years [95% confidence interval (CI): 7.4-10.2], and the incidence of mortality was 23.5 per 1000 person-years (95% CI: 21.4-25.9). Both the cause-specific model and the Fine-Gray subdistribution hazard model showed that participants with HGS asymmetry had increased hazard of neurodegenerative disorders [hazard ratio (HR) = 1.66, P = 0.002, 95% CI: 1.202-2.297; subdistribution hazard ratio (SHR) = 1.65, P = 0.002, 95% CI: 1.202-2.285]. Low HGS, but not HGS asymmetry, was related to the higher hazard of mortality (HR = 1.61, P < 0.001, 95% CI: 1.297-1.995; SHR = 1.58, P < 0.001, 95% CI: 1.286-1.951).ConclusionsHandgrip strength asymmetry was associated with the future risk of neurodegenerative disorders among Chinese older adults. Public healthcare providers could consider examining HGS asymmetry along with the maximal HGS as a way to identify those at elevated risk of neurodegenerative disorders.
