The combined effect of cigarette smoking and occupational noise exposure on hearing loss: evidence from the Dongfeng-Tongji Cohort Study.

ABSTRACT: Combined effect of cigarette smoking and occupational noise exposure on hearing loss has rarely been evaluated among Chinese population, especially among females. This cross-sectional study was conducted in 11196 participants of Dongfeng-Tongji cohort study. Smoking status was self-reported through questionnaire and occupational noise exposure was evaluated through workplace noise level and/or the job titles. Hearing loss was defined as a pure-tone mean of 25 dB or higher at 0.5, 1, 2, and 4 kHz in both ears. Compared with participants without occupational noise exposure, the risk of hearing loss was significantly higher for noise exposure duration ≥20 (OR = 1.45, 95%CI = 1.28-1.65). The association was particularly evident among individuals who were males (OR = 1.74, 95%CI = 1.45-2.08) and aged ≥ 70 (OR = 1.74, 95%CI = 1.30-2.33). Similarly, the risks increased with the increasing of pack-years in males and all age groups except for those aged <60. As to the combined effect, the hearing loss risk was highest for noise exposure duration ≥20 and pack-years ≥25 (OR = 2.41, 95%CI = 1.78-3.28), especially among males (OR = 2.42, 95%CI = 1.74-3.37) and those aged ≥70 (OR = 2.76, 95%CI = 1.36-5.60). Smoking may be an independent risk factor for hearing loss. And it may synergistically affect hearing when combined with occupational noise exposure, especially among males and older participants.

Project description:To investigate the current situation of hearing loss and the associations of occupational noise exposure with hearing loss in petrochemical enterprise workers. This was a cross-sectional study that employed cluster sampling methods, and 951 workers aged 20-59 years from a petrochemical enterprise were included between June and December 2022. The subjects underwent health status surveys and occupational health examinations. To assess the associations between noise level and hearing loss, a logistic regression model was used to calculate odds ratios (ORs) and 95% confidence intervals (CIs). A total of 951 participants (837 male and 114 female) were included in this study. And 68.3% of workers had a noise exposure level greater than ≥ 80 dB (A). The prevalence of speech frequency hearing loss, high-frequency hearing loss, and bilateral hearing loss among participants was 2.3% (22/951), 10.2% (97/951), and 10.3% (98/951), respectively. The prevalences of high-frequency hearing loss and binaural hearing loss were compared among subjects grouped by gender, age, family history of hyperlipidaemia, smoking status, systolic blood pressure, diastolic blood pressure, duration of noise exposure, and level of occupational noise exposure, and all differences were statistically significant (all P < 0.01). Compared with that of participants with noise levels < 80 dB(A), the risk of speech frequency hearing loss was significantly greater in the ≥ 90 dB(A) noise exposure group (OR: 5.33). The noise exposure level 80-89 dB(A) group (OR: 2.75) and the noise exposure level ≥ 90 dB(A) group (OR: 2.16) were both associated with a higher prevalence of high-frequency hearing loss. The noise hazards in petrochemical enterprises are severe. Long-term exposure to occupational noise environments significantly increases the risk of hearing loss among workers. Therefore, it is essential to strengthen noise control and enhance hearing protection efforts to effectively prevent excessive occupational noise exposure, thereby reducing the risk of hearing loss in workers.

Project description:ObjectiveMost of the Chinese occupational population are becoming at risk of noise-induced hearing loss (NIHL). However, there is a limited number of literature reviews on occupational NIHL in China. This study aimed to analyse the prevalence and characteristics of occupational NIHL in the Chinese population using data from relevant studies.DesignSystematic review and meta-analysis.MethodsFrom December 2019 to February 2020, we searched the literature through databases, including Web of Science, PubMed, MEDLINE, Scopus, the China National Knowledge Internet, Chinese Sci-Tech Journal Database (weip.com), WanFang Database and China United Library Database, for studies on NIHL in China published in 1993-2019 and analysed the correlation between NIHL and occupational exposure to noise, including exposure to complex noise and coexposure to noise and chemicals.ResultsA total of 71 865 workers aged 33.5±8.7 years were occupationally exposed to 98.6±7.2 dB(A) (A-weighted decibels) noise for a duration of 9.9±8.4 years in the transportation, mining and typical manufacturing industries. The prevalence of occupational NIHL in China was 21.3%, of which 30.2% was related to high-frequency NIHL (HFNIHL), 9.0% to speech-frequency NIHL and 5.8% to noise-induced deafness. Among manufacturing workers, complex noise contributed to greater HFNIHL than Gaussian noise (overall weighted OR (OR)=1.95). Coexposure to noise and chemicals such as organic solvents, welding fumes, carbon monoxide and hydrogen sulfide led to greater HFNIHL than noise exposure alone (overall weighted OR=2.36). Male workers were more likely to experience HFNIHL than female workers (overall weighted OR=2.26). Age, noise level and exposure duration were also risk factors for HFNIHL (overall weighted OR=1.35, 5.63 and 1.75, respectively).ConclusionsThe high prevalence of occupational NIHL in China was related to the wide distribution of noise in different industries as well as high-level and long-term noise exposure. The prevalence was further aggravated by exposure to complex noise or coexposure to noise and specific chemicals. Additional efforts are needed to reduce occupational noise exposure in China.

Project description:BackgroundHigh-intensity noise is associated with noise-induced hearing loss (NIHL). There is also evidence that noise exposure is related to cognitive impairment. This research aimed to analyze the associations and potential pathways of cumulative noise exposure (CNE), hearing loss and cognitive impairment.MethodsA total of 560 research subjects were included in this research from May 2021 to April 2022 in western China. The demographic features, occupational features, and CNE were investigated and examined. Hearing loss was evaluated according to the National standard GB/T 7583-1987 of China. The Mini-Mental State Examination (MMSE) and Montreal Cognitive Assessment (MoCA) were used to assess cognitive function. Structural equation modeling (SEM) was used to analyze the potential pathways and specific effect sizes of CNE, hearing loss and cognitive impairment.ResultsThe mean age of the research subjects was 34.3 (SD, 9.89). Men accounted for 96.4% (540/560) and women accounted for 3.6% (20/560). A total of 62.3% (349/560) held a college degree or above. The regression analysis showed that high dose CNE was related to MMSE (β = -1.069 (-1.539, -0.600)) and MoCA (β = -1.040 (-1.726, -0.355)) scores. The monaural threshold weighted value of the right ear (MTWVR) greater than 40 dB was associated with both MMSE (β = -1.183 (-2.033, -0.333)) and MoCA (β = -1.420 (-2.647, -0.193)) scores. The monaural threshold weighted value of the left ear (MTWVL) greater than 40 dB was also associated with MMSE (β = -1.540 (-2.389, -0.690)) and MoCA (β = -1.685 (-2.915, -0.456)) scores. The SEM result (Model C) showed that the standard effect of CNE- hearing loss path, CNE-MMSE path, and hearing loss-MMSE path were 0.142 (p < 0.001), -0.151 (p < 0.001), and -0.030 (p = 0.376). The Model D showed that the standard effect of CNE- hearing loss path, CNE- MoCA path, and hearing loss- MoCA path were 0.143 (p < 0.001), -0.048 (p = 0.267), and - 0.050 (p = 0.047). The CNE had only a direct effect on the MMSE score. Conversely, the CNE had only an indirect effect on the MoCA score, while hearing loss was borderline associated with MoCA. The total effects of CNE on MMSE and MoCA scores were -0.151 and -0.007, respectively.ConclusionJob-related noise exposure is not only associated with NIHL but also with early cognitive impairment in occupational groups. However, there is not enough evidence indicating that NLHL mediates the associations.
