A Risk Algorithm for the Persistence of Suicidal Thoughts and Behaviors During College.

ABSTRACT:

Objective

The primary aims of this study are to (a) identify patterns of suicidal thoughts and behaviors (STB) during college among students with lifetime pre-matriculation STB and (b) develop a risk-screening algorithm for persistence of pre-matriculation STB during college.

Methods

Data come from the Leuven College Surveys, a series of prospective cohort studies of all incoming KU Leuven University freshmen. In the academic year 2012-2013, 4,889 incoming freshmen (73.2% response rate) provided baseline data on sociodemographic variables, childhood-adolescent traumatic experiences, 12-month stressful experiences, 12-month mental disorders, 12-month STB, and severity markers of pre-matriculation STB. A total of 2,566 students (69.3% conditional response rate) participated in 12- and 24-month follow-up surveys during the first 2 college years.

Results

Thirteen percent (weighted n = 535) of incoming freshmen reported lifetime pre-matriculation STB. Of those, 28.0% reported 12-month STB in 1 follow-up assessment, and another 27.7%, in both follow-up assessments. High persistence of STB (ie, 12-month STB in 2 follow-up assessments) was most strongly associated with severity markers of pre-matriculation STB, with odds ratios in the 2.4-10.3 range and population attributable risk proportions between 9.2% and 50.8%. When the aim was for less than 50% of false-positive cases (positive predictive value = 54.4%), a multivariate predictive risk algorithm (cross-validated area under the curve = 0.79) situated 59.9% of highly persistent cases among the 30% respondents with highest baseline predicted risk.

Conclusions

An individualized web-based screening approach is a promising strategy to identify students at the time of university entrance who may be at high risk for STB persistence during their academic career.

Project description:ImportanceSuicide rates among adolescents continue to rise, but there are a lack of clinical tools to predict when youths may be at risk for suicidal behaviors.ObjectiveTo identify whether geolocation metrics, assessed through an app installed on adolescents' personal smartphones, could detect the risk of next-week suicidal events and clinically meaningful suicidal ideation.Design, setting, and participantsThis case series study included high-risk adolescents aged 13 to 18 years reporting a current affective and/or substance use disorder, oversampled for suicidal thoughts and behaviors (STB). Participants were recruited from the greater New York City and Pittsburgh communities through psychiatric outpatient programs, emergency departments, medical center research registries, and social media. Participants installed the Effortless Assessment Research System (EARS) software application onto their personal smartphones, which obtained passive sensor data, including geolocation metrics (via the global positioning system [GPS]), as well as weekly experience sampling data probing STB for the duration of the 6-month study. Adolescents also completed clinical assessments at baseline as well as during the 1-, 3-, and 6-month follow-up assessments. Statistical analysis was performed from March 2023 to November 2024.Main outcomes and measuresRepeated measures mixed-effects logistic models estimated whether weekly aggregates of geolocation features (ie, entropy, homestay, distance traveled) were associated with next-week suicidal events (ie, suicide attempts, psychiatric hospitalizations, emergency department visits for suicide concerns) and clinically meaningful ideation (via weekly experience sampling).ResultsOverall, 186 participants were included in this study (148 [79.6%] female; 19 [10.2%] Asian, 23 [12.4%] Black, and 106 [57.0%] White), with a mean (SD) age of 16.4 (1.7) years. Greater homestay (amount of time spent at home) on a given week, relative to one's own mean, was associated with 2-fold greater odds of suicidal events during the subsequent week (odds ratio, 1.99 [95% CI, 1.15-3.45]). Results were not significant for entropy and distance traveled metrics. However, using leave-future-out validation, the accuracy of the homestay model was modest (area under the receiver operating characteristic curve, 0.64 [95% CI, 0.50-0.78]).Conclusions and relevanceAdvancements in smartphone technology afford unique opportunities to capture affective and behavioral dynamics that presage suicide risk. This case series study found that greater homestay obtained through smartphone GPS data over the course of a week, relative to one's own mean, was associated with greater odds of a suicidal event in the subsequent week. Although accuracy was modest, these findings offer a novel starting point for suicide prevention research, particularly as smartphone sensor data may have the capacity to identify who is at risk while also providing an opportunity to deliver clinical tools when that risk is greatest.

Project description:ImportancePopulation-level reports of suicide-related emergency department (ED) encounters among youth during the COVID-19 pandemic are lacking, along with youth characteristics and preexisting psychiatric service use.ObjectiveTo characterize population-level and relative change in suicide-related ED encounters among youth during the COVID-19 pandemic compared with 2019.Design, setting, and participantsThis cross-sectional study evaluated ED encounters in 2019 and 2020 at Kaiser Permanente Northern California-a large, integrated, community-based health system. Youth aged 5 to 17 years who presented to the ED with suicidal thoughts or behaviors were included.ExposureThe COVID-19 pandemic.Main outcomes and measuresPopulation-level incidence rate ratios (IRRs) and percent relative effects for suicide-related ED encounters as defined by the US Centers for Disease Control and Prevention-recommended International Statistical Classification of Diseases, Tenth Revision, Clinical Modification (ICD-10-CM) codes in 4 periods in 2020 compared with the same periods in 2019.ResultsThere were 2123 youth with suicide-related ED encounters in 2020 compared with 2339 in 2019. In the 2020 group, 1483 individuals (69.9%) were female and 1798 (84.7%) were aged 13 to 17 years. In the 2019 group, 1542 (65.9%) were female, and 1998 (85.4%) were aged 13 to 17 years. Suicide-related ED encounter incidence rates were significantly lower in March through May 2020 compared with this period in 2019 (IRR, 0.57; 95% CI, 0.51-0.63; P < .001), then returned to prepandemic levels. However, suicide-related ED visits among female youth from June 1 to August 31, 2020, and September 1 through December 15, 2020, were significantly higher than in the corresponding months in 2019 (IRR, 1.19; 95% CI, 1.04-1.35; P = .04 and IRR, 1.22; 95% CI, 1.11-1.35; P < .001, respectively), while suicide-related ED visits for male youth decreased from September 1 through December 15, 2020 (IRR, 0.81; 95% CI, 0.69 to 0.94). Youth with no history of outpatient mental health or suicide encounters (129.4%; 95% CI, 41.0-217.8) and those with comorbid psychiatric conditions documented at the ED encounter (6.7%; 95% CI, 1.0-12.3) had a higher risk of presenting with suicide-related problems from September to December 2020 vs the same period in 2019.Conclusions and relevanceIn this cross-sectional study of youth experiencing suicidal thoughts and behaviors, suicide-related presentations to the ED initially decreased during the COVID-19 pandemic, likely owing to shelter-in-place orders, then were similar to 2019 levels. However, a greater number of female youth, youth with no psychiatric history, and youth with psychiatric diagnoses at the time of the ED encounter presented for suicide-related concerns during the pandemic, suggesting these may be vulnerable groups in need of further interventions. Adjustments in care may be warranted to accommodate these groups during periods of crisis.
