Joining forces to prevent dementia: The International Research Network On Dementia Prevention (IRNDP).

ABSTRACT: Dementia is a neurodegenerative disorder with global impact, with the largest proportion of cases occurring in low- and middle-income countries. It is estimated that there are 46.8 million cases globally with approximately 10 million new cases each year or a new case occurring every 3 sec (Prince et al., 2015). For comparison there are 36.7 million HIV cases with an estimated 2 million new cases each year (WHO, 2017). The rise in dementia prevalence is largely due to population ageing, with the oldest being at highest risk. To date there are no diseases modifying medications for Alzheimer's disease or the other causes of dementia. Academics and research groups are increasingly focused on prevention or delay of dementia (Brayne and Miller, 2017) and a number of organizations now prioritize dementia, indicating a strong and coherent international effort to address this problem. Examples include the World Health Organisation (WHO), which has established a Global Dementia Observatory; the World Dementia Council; the Organisation for Economic Co-operation and Development (OECD); the U.S. National Alzheimer's Project Act (NAPA); and the Global Council on Brain Health.

Project description:Proactive approaches that anticipate the long-term effects of current and future conservation threats could increase the effectiveness and efficiency of biodiversity conservation. However, such approaches can be obstructed by a lack of knowledge of habitat requirements for wildlife. To aggregate and assess the suitability of current information available on habitat requirements needed for proactive conservation, we conducted a systematic review of the literature on elephant and rhinoceros habitat requirements and synthesized data by combining a vote counting assessment with bibliometric and term maps. We contextualized these numeric and terminological results with a narrative review. We mapped current methods, results, terminology, and collaborations of 693 studies. Quantitative evidence for factors that influence the suitability of an area for elephants and rhinoceros was biased toward African savanna elephants and ecological variables. Less than one third of holistic approaches considered equal amounts of ecological and anthropogenic variables in their assessments. There was a general lack of quantitative evidence for direct proxies of anthropogenic variables that were expected to play an important role based on qualitative evidence and policy documents. However, there was evidence for a segregation in conceptual frameworks among countries and species and between science versus policy literature. There was also evidence of unused potential for collaborations among southern hemisphere researchers. Our results indicated that the success of proactive conservation interventions can be increased if ecological and anthropogenic dimensions are integrated into holistic habitat assessments and holistic carrying capacities and quantitative evidence for anthropogenic variables is improved. To avoid wasting limited resources, it is necessary to form inclusive collaborations within and across networks of researchers studying different species across regional and continental borders and in the science-policy realm.

Project description:IntroductionProfiles of high risk for future dementia are well understood and are likely to concern mostly those in low-income and middle-income countries and people at greater disadvantage in high-income countries. Approximately 30%-40% of dementia cases have been estimated to be attributed to modifiable risk factors, including hypertension, smoking and sedentary lifestyle. Tailored interventions targeting these risk factors can potentially prevent or delay the onset of dementia. Mobile health (mHealth) improves accessibility of such prevention strategies in hard-to-reach populations while at the same time tailoring such approaches. In the current study, we will investigate the effectiveness and implementation of a coach-supported mHealth intervention, targeting dementia risk factors, to reduce dementia risk.Methods and analysisThe prevention of dementia using mobile phone applications (PRODEMOS) randomised controlled trial will follow an effectiveness-implementation hybrid design, taking place in the UK and China. People are eligible if they are 55-75 years old, of low socioeconomic status (UK) or from the general population (China); have ≥2 dementia risk factors; and own a smartphone. 2400 participants will be randomised to either a coach-supported, interactive mHealth platform, facilitating self-management of dementia risk factors, or a static control platform. The intervention and follow-up period will be 18 months. The primary effectiveness outcome is change in the previously validated Cardiovascular Risk Factors, Ageing and Incidence of Dementia dementia risk score. The main secondary outcomes include improvement of individual risk factors and cost-effectiveness. Implementation outcomes include acceptability, adoption, feasibility and sustainability of the intervention.Ethics and disseminationThe PRODEMOS trial is sponsored in the UK by the University of Cambridge and is granted ethical approval by the London-Brighton and Sussex Research Ethics Committee (reference: 20/LO/01440). In China, the trial is approved by the medical ethics committees of Capital Medical University, Beijing Tiantan Hospital, Beijing Geriatric Hospital, Chinese People's Liberation Army General Hospital, Taishan Medical University and Xuanwu Hospital. Results will be published in a peer-reviewed journal.Trial registration numberISRCTN15986016.

Project description:BackgroundErgometrine is part of the current guideline-directed management of post-partum haemorrhage (PPH). However, it is also a potent vasoconstrictor capable of causing significant coronary artery vasospasm in susceptible individuals.Case summaryA 31-year-old primigravida suffered from post-partum haemorrhagic shock and disseminated intravascular coagulation following vaginal delivery. She was urgently resuscitated with intravenous fluids and blood products. Intramuscular ergometrine was administered, and surgery was required due to retained placenta. Two days later upon extubation, she demonstrated symptoms of speech apraxia. Brain magnetic resonance imaging (MRI) revealed extensive cerebral and cerebellar infarction with subcortical sparring. Her electrocardiogram showed diffuse T-wave inversions, and a high-sensitivity troponin-I peaked at 37 000 ng/L. Echocardiography showed severe left ventricular (LV) failure, apical akinesia, and thrombi formation. One week later, she suffered a middle cerebral artery stroke causing aphasia and right-sided hemiparesis, necessitating emergency thrombectomy. Cardiovascular MRI showed moderate LV systolic impairment and focal apical infarction. Coronary angiography was unremarkable. The most likely unifying diagnosis was severe ergotamine-induced coronary vasospasm causing acute myocardial infarction in the setting of life-threatening PPH. Following three weeks of multi-disciplinary care, her speech and motor abilities improved. She was discharged on long-acting nitrates, oral anticoagulation, and heart failure therapy with close outpatient monitoring. Subsequent echocardiograms showed marked improvement in LV ejection fraction (45%-50%).DiscussionThis case highlights the potential life-threatening complications of ergometrine and the importance of recognizing pregnancy-associated myocardial infarction as a significant cause of maternal morbidity and mortality. The cardio-obstetrics team plays a pivotal role in improving patient outcomes.
