3D printing of a wearable personalized oral delivery device: A first-in-human study.

ABSTRACT: Despite the burgeoning interest in three-dimensional (3D) printing for the manufacture of customizable oral dosage formulations, a U.S. Food and Drug Administration-approved tablet notwithstanding, the full potential of 3D printing in pharmaceutical sciences has not been realized. In particular, 3D-printed drug-eluting devices offer the possibility for personalization in terms of shape, size, and architecture, but their clinical applications have remained relatively unexplored. We used 3D printing to manufacture a tailored oral drug delivery device with customizable design and tunable release rates in the form of a mouthguard and, subsequently, evaluated the performance of this system in the native setting in a first-in-human study. Our proof-of-concept work demonstrates the immense potential of 3D printing as a platform for the development and translation of next-generation drug delivery devices for personalized therapy.

3D Printing of Personalized Artificial Bone Scaffolds.

Project description:Additive manufacturing technologies, including three-dimensional printing (3DP), have unlocked new possibilities for bone tissue engineering. Long-term regeneration of normal anatomic structure, shape, and function is clinically important subsequent to bone trauma, tumor, infection, nonunion after fracture, or congenital abnormality. Due to the great complexity in structure and properties of bone across the population, along with variation in the type of injury or defect, currently available treatments for larger bone defects that support load often fail in replicating the anatomic shape and structure of the lost bone tissue. 3DP could provide the ability to print bone substitute materials with a controlled chemistry, shape, porosity, and topography, thus allowing printing of personalized bone grafts customized to the patient and the specific clinical condition. 3DP and related fabrication approaches of bone grafts may one day revolutionize the way clinicians currently treat bone defects. This article gives a brief overview of the current advances in 3DP and existing materials with an emphasis on ceramics used for 3DP of bone scaffolds. Furthermore, it addresses some of the current limitations of this technique and discusses potential future directions and strategies for improving fabrication of personalized artificial bone constructs.

Scenarios for 3D printing of personalized medicines - A case study.

Project description:Background3D printing is a promising new technology for medicines' production. It employs additive manufacturing techniques, and is ideal for producing personalized medicines (e.g., patient-tailored dose, dosage form, drug release kinetics).ObjectiveTo investigate how 3D printing technologies can be implemented in a European pharmaceutical system, by suggesting different scenarios and assessing aspects that could affect its implementation.MethodQualitative, semi-structured interviews were conducted with key stakeholders (e.g., from ministry, authorities, research organizations, pharmacies) in the Netherlands to elicit perspectives on 3D printing of personalized medicines. The Netherlands were chosen since it has a strong tradition in compounding. Five general scenarios were investigated: placing the 3D printers in industry, community pharmacies, hospital pharmacies, compounding facilities, and in patients' homes. Content analysis was used, building on verbatim transcripts.ResultsFifteen stakeholders were interviewed. Regulatory, economic, ethical and organizational challenges were identified to varying degrees in the different scenarios. The industry and home scenarios were associated with the most challenges, hospital pharmacies and compounding facilities with the least. Other important aspects identified were the role of community pharmacies, and who should design the tablets to be printed.ConclusionAll potential scenarios for 3D printing of personalized medicines include challenges. These should be taken into account when pursuing the use of 3D printing of medicine.

Innovative color jet 3D printing of levetiracetam personalized paediatric preparations.

Project description:3D printing is a promising technology used in the fabrication of complex oral dosage delivery pharmaceuticals. This study first reports an innovative color jet 3D printing (CJ-3DP) technology to produce colorful cartoon levetiracetam pediatric preparations with high accuracy and reproducibility. For this study, the ideal printing ink consisted of 40% (v/v) isopropanol aqueous solution containing 0.05% (w/w) polyvinylpyrrolidone and 4% (w/w) glycerin, which was satisfied with scale-up of the production. The external and internal spatial structures of the tablets were designed to control the appearance and release, and cartoon tablets with admirable appearances and immediate release characteristics were printed. The dosage model showed a good linear relationship between the model volume and the tablet strength (r > 0.999), which proved the potential of personalized administration. The surface roughness indicated that the appearance of the CJ-3DP tablets was significantly better than the first listed 3D printed drug (SpritamⓇ). Moreover, the scanning electron microscopy and porosity results further showed that the tablets have a structure of loose interior and tight exterior, which could ensure good mechanical properties and rapid dispersion characteristics simultaneously. In conclusion, the innovative CJ-3DP technology can be used to fabricate personalized pediatric preparations for improved compliance. Due to the stable formulation and fabrication process, this technology has the potential in scale-up production.

Ultrasensitive Wearable Strain Sensors of 3D Printing Tough and Conductive Hydrogels.

Project description:In this study, tough and conductive hydrogels were printed by 3D printing method. The combination of thermo-responsive agar and ionic-responsive alginate can highly improve the shape fidelity. With addition of agar, ink viscosity was enhanced, further improving its rheological characteristics for a precise printing. After printing, the printed construct was cured via free radical polymerization, and alginate was crosslinked by calcium ions. Most importantly, with calcium crosslinking of alginate, mechanical properties of 3D printed hydrogels are greatly improved. Furthermore, these 3D printed hydrogels can serve as ionic conductors, because hydrogels contain large amounts of water that dissolve excess calcium ions. A wearable resistive strain sensor that can quickly and precisely detect human motions like finger bending was fabricated by a 3D printed hydrogel film. These results demonstrate that the conductive, transparent, and stretchable hydrogels are promising candidates as soft wearable electronics for healthcare, robotics and entertainment.

3D Printing of Personalized Thick and Perfusable Cardiac Patches and Hearts.

Project description:Generation of thick vascularized tissues that fully match the patient still remains an unmet challenge in cardiac tissue engineering. Here, a simple approach to 3D-print thick, vascularized, and perfusable cardiac patches that completely match the immunological, cellular, biochemical, and anatomical properties of the patient is reported. To this end, a biopsy of an omental tissue is taken from patients. While the cells are reprogrammed to become pluripotent stem cells, and differentiated to cardiomyocytes and endothelial cells, the extracellular matrix is processed into a personalized hydrogel. Following, the two cell types are separately combined with hydrogels to form bioinks for the parenchymal cardiac tissue and blood vessels. The ability to print functional vascularized patches according to the patient's anatomy is demonstrated. Blood vessel architecture is further improved by mathematical modeling of oxygen transfer. The structure and function of the patches are studied in vitro, and cardiac cell morphology is assessed after transplantation, revealing elongated cardiomyocytes with massive actinin striation. Finally, as a proof of concept, cellularized human hearts with a natural architecture are printed. These results demonstrate the potential of the approach for engineering personalized tissues and organs, or for drug screening in an appropriate anatomical structure and patient-specific biochemical microenvironment.

A Wearable Textile 3D Gesture Recognition Sensor Based on Screen-Printing Technology.

Project description:Research has developed various solutions in order for computers to recognize hand gestures in the context of human machine interface (HMI). The design of a successful hand gesture recognition system must address functionality and usability. The gesture recognition market has evolved from touchpads to touchless sensors, which do not need direct contact. Their application in textiles ranges from the field of medical environments to smart home applications and the automotive industry. In this paper, a textile capacitive touchless sensor has been developed by using screen-printing technology. Two different designs were developed to obtain the best configuration, obtaining good results in both cases. Finally, as a real application, a complete solution of the sensor with wireless communications is presented to be used as an interface for a mobile phone.

3D-Printed Hydrogel-Based Flexible Electrochromic Device for Wearable Displays.

Project description:Flexible electrochromic devices (FECDs) are widely explored for diverse applications including wearable electronics, camouflage, and smart windows. However, the manufacturing process of patterned FECDs remains complex, costly, and non-customizable. To address this challenge, a strategy is proposed to prepare integrated FECDs via multi-material direct writing 3D printing. By designing novel viologen/polyvinyl alcohol (PVA) hydrogel inks and systematically evaluating the printability of various inks, seamless interface integration can be achieved, enabling streamlined manufacturing of patterned FECDs with continuous production capabilities. The resultant 3D-printed FECDs exhibit excellent electrochromic and mechanical properties, including high optical contrast (up to 54% at 360 nm), nice cycling stability (less than 5% electroactivity reduction after 10 000 s), and mechanical stability (less than 19% optimal contrast decrease after 5000 cycles of bending). The potential applications of these 3D-printed hydrogel-based FECDs are further demonstrated in wearable electronics, camouflage, and smart windows.

First experiences of a hospital-based 3D printing facility - an analytical observational study.

Project description:PurposeTo identify the clinical impact and potential benefits of in-house 3D-printed objects through a questionnaire, focusing on three principal areas: patient education; interdisciplinary cooperation; preoperative planning and perioperative execution.Materials and methodsQuestionnaires were sent from January 2021 to August 2022. Participants were directed to rate on a scale from 1 to 10.ResultsThe response rate was 43%. The results of the rated questions are averages. 84% reported using 3D-printed objects in informing the patient about their condition/procedure. Clinician-reported improvement in patient understanding of their procedure/disease was 8.1. The importance of in-house placement was rated 9.2. 96% reported using the 3D model to confer with colleagues. Delay in treatment due to 3D printing lead-time was 1.8. The degree with which preoperative planning was altered was 6.9. The improvement in clinician perceived preoperative confidence was 8.3. The degree with which the scope of the procedure was affected, in regard to invasiveness, was 5.6, wherein a score of 5 is taken to mean unchanged. Reduction in surgical duration was rated 5.7.ConclusionClinicians report the utilization of 3D printing in surgical specialties improves procedures pre- and intraoperatively, has a potential for increasing patient engagement and insight, and in-house location of a 3D printing center results in improved interdisciplinary cooperation and allows broader access with only minimal delay in treatment due to lead-time.

Post-printing surface modification and functionalization of 3D-printed biomedical device.

Project description:3D printing is a technology well-suited for biomedical applications due to its ability to create highly complex and arbitrary structures from personalized designs with a fast turnaround. However, due to a limited selection of 3D-printable materials, the biofunctionality of many 3D-printed components has not been paid enough attention. In this perspective, we point out that post-3D printing modification is the solution that could close the gap between 3D printing technology and desired biomedical functions. We identify architectural reconfiguration and surface functionalization as the two main post-3D printing modification processes and discuss potential techniques for post-3D printing modification to achieve desired biofunctionality.

A Modular Microfluidic Device via Multimaterial 3D Printing for Emulsion Generation.

Project description:3D-printing (3DP) technology has been developing rapidly. However, limited studies on the contribution of 3DP technology, especially multimaterial 3DP technology, to droplet-microfluidics have been reported. In this paper, multimaterial 3D-printed devices for the pneumatic control of emulsion generation have been reported. A 3D coaxial flexible channel with other rigid structures has been designed and printed monolithically. Numerical and experimental studies have demonstrated that this flexible channel can be excited by the air pressure and then deform in a controllable way, which can provide the active control of droplet generation. Furthermore, a novel modular microfluidic device for double emulsion generation has been designed and fabricated, which consists of three modules: function module, T-junction module, and co-flow module. The function module can be replaced by (1) Single-inlet module, (2) Pneumatic Control Unit (PCU) module and (3) Dual-inlet module. Different modules can be easily assembled for different double emulsion production. By using the PCU module, double emulsions with different number of inner droplets have been successfully produced without complicated operation of flow rates of different phases. By using single and dual inlet module, various double emulsions with different number of encapsulated droplets or encapsulated droplets with different compositions have been successfully produced, respectively.
