Outside-in Repair Technique for a Complete Radial Tear of the Lateral Meniscus.

ABSTRACT: The lateral meniscus is vital in dissipating the force in the lateral compartment of the knee. A complete radial tear of the meniscus can lead to extrusion, rendering it nonfunctional and resulting in deleterious arthritic changes to the lateral compartment. Arthroscopic repair of a complete radial tear of the lateral meniscus poses a challenge to orthopaedic surgeons. Although some would advocate for meniscectomy, we present a technique for an outside-in repair using 3 sutures and standard arthroscopic portals. Overall, this provides for an excellent reduction of the meniscus.

Outside-In Repair of Longitudinal Tear of Medial Meniscus: Suture Shuttle Technique.

Project description:Meniscal tears are commonly encountered conditions of the knee. In the past, torn menisci were treated by excision of the loose flap. A better understanding of the meniscus anatomy and its biomechanical characteristics has led to the concept of meniscus preservation in eligible cases. Several suture-based repair techniques have been described in literature, including the outside-in technique. Although the outside-in technique of meniscus repair is commonly indicated for the anterior two-thirds of the meniscus, it can be used to repair the posterior part of the meniscus as well. Several modifications of this technique have been described in the literature. We hereby describe our modification of the outside-in technique of meniscus repair with the help of an epidural needle and high-strength sutures that is readily available in operating rooms. The advantages of our technique are that no large incision is required around knee joint, it's inexpensive, it can be performed with basic instruments, and even the tear of meniscus extending up to the posterior horn can be repaired. A supplemental video demonstration of the technique is included with this article.

Anterolateral Radial Meniscus Tear Repair using Traction Suture and Super-Hashtag Technique.

Project description:Radial meniscus tears occur commonly as traumatic tears in younger patients, as well as in association with concomitant degenerative changes. Traditional management of these tears has centered around partial meniscectomy; however, there has been a more recent trend toward preserving the meniscus and attempting repair. Because of the gapping and displacement that frequently occurs with these tears, repair is often challenging and is done under high amounts of tension. The following article describes a technique using a traction suture to aid in maintaining reduction throughout repair. This is followed by the use of a combination of techniques to form a "super-hashtag" configuration of both vertical and horizontal mattress sutures, leading to a secure repair under little tension.

Ultrasound-Assisted Arthroscopic All-Inside Repair Technique for Posterior Lateral Meniscus Tear.

Project description:Arthroscopic repair of the posterior horn of the lateral meniscus (LM) from an anterolateral portal has a risk of popliteal artery injury. Here, we present an ultrasound-assisted, arthroscopic, all-inside repair technique for a posterior LM tear to reduce the risk of neurovascular injury. An ultrasound probe covered with a sterile sleeve is placed horizontally at the popliteal fossa by an assistant surgeon, and the popliteal artery and posterior LM are confirmed. From the anterolateral portal, an arthroscopic probe is inserted to push the posterior capsule of the lateral compartment, while an ultrasound image detects the tip of the probe. After the probe is confirmed not to be directed toward the popliteal artery, an all-inside suture device is introduced from the anterolateral portal. While the meniscus is penetrated, the surgeon can confirm by ultrasound images that the needle is directed away from the popliteal artery. The guide suture is pulled anteriorly to secure the anchors tightly, and an ultrasound confirms that the anchors are positioned behind the posterior portion of the LM. All sutures are secured under the assistance of ultrasound images, followed by arthroscopic confirmation of a properly secured LM by the all-inside repair technique.

Arthroscopic Repair Technique of Lateral Meniscus Tear at the Popliteal Hiatus Area.

Project description:Arthroscopic treatment of lateral meniscus tear at popliteal hiatus area remains challenging due to the complex anatomy of the popliteal hiatus and limited operating space of the lateral compartment. Subtle injuries to attachments of the popliteal hiatus may be overlooked. Thus, this article presents an all-inside suture technique stitching the torn lateral meniscus to popliteomeniscal fascicles that avoids leaving the lateral meniscus unstable in at popliteal hiatus area postoperatively. Concern about entrapping the popliteal tendon or all-inside meniscus suture anchor migration in the joint cavity is not necessary.

Bridge-Enhanced Anterior Cruciate Ligament Repair for Mid-Substance Tear With Concomitant Lateral Meniscus Radial Repair.

Project description:In recent years, repair of the injured anterior cruciate ligament (ACL) has been subject to a renewed interest as novel arthroscopic techniques have been developed. Specifically, the bridge-enhanced ACL repair is a technique composed of a resorbable protein-based implant combined with autologous blood that is used to bridge the gap between 2 torn edges of a mid-substance ACL tear. This implant is believed to help facilitate healing of a primary suture repair and suture cinch and has since demonstrated noninferiority to ACL reconstruction with autograft at 2-year follow-up. The purpose of this Technical Note is to describe a step-by-step surgical technique of a mid-substance ACL repair using the bridge-enhanced ACL repair system in a case with a concomitant lateral meniscus radial repair.

Modified Centralization Technique of Tibial Side Capsulodesis for Radial Tear of Incomplete Discoid Lateral Meniscus.

Project description:An incomplete discoid lateral meniscus is often associated with radial tears, which cause meniscal extrusion and result in poor healing outcomes. Centralization has recently been used as a surgical method to reduce extrusion. However, various repair techniques use single point of fixation sutures exclusively on the femoral side, potentially hindering healing. In this study, a method is devised in which 2 anchor sutures are placed on the tibia and guided out of the capsule from the tibial side of the meniscus, and another anchor is placed distally to tighten the meniscotibial ligament in a plane. This method supports the repaired site against hoop stress by tightening the meniscotibial ligament and is considered a physiological repair because the sutures are confirmed to the tibial side of the meniscus rather than on the femoral side. Furthermore, an inside-out suture of the torn area with a fibrin clot derived from the bone marrow aspirate, rich in tissue-healing components, is also performed.

Medial Meniscus Radial Tear: A Transtibial 2-Tunnel Technique.

Project description:Complete radial tears of the medial meniscus significantly decrease the meniscal tissue's ability to dissipate tibiofemoral loads and have been described as functionally similar to a total meniscectomy, predisposing patients to early osteoarthritis. At present, no consensus exists regarding the optimal surgical treatment of a radial meniscal tear. Current repair techniques have led to a reportedly high rate of incomplete healing or healing of the meniscus in a nonanatomic, gapped position, which compromises its ability to withstand hoop stresses. Improvement regarding the ability to repair and heal medial meniscus radial tears has the potential to result in enhanced preservation of the articular cartilage in the medial compartment of the knee. This technical description details a method for repairing radial tears of the medial meniscus using a transtibial 2-tunnel technique.

Arthroscopic Repair for Parrot Beak Tear of Lateral Meniscus with Reduction Suture and Inside-Out Technique.

Project description:Parrot beak tear is a white-white meniscal injury that often occurs in isolated injuries. Partial meniscectomy for parrot beak tears is often recommended, owing to the avascular zone; however, partial meniscectomy, especially with the lateral meniscus, has a high failure rate for return to sports, leading to residual meniscus extrusion and lateral compartment osteoarthritis. Thus, we have developed a repair technique to preserve the parrot beak tear of the avascular zone. This is a modification of the inside-out repair with additional reduction sutures. We recommend this procedure as a technique for repairing avascular parrot beak tears of the lateral meniscus.

Modified Outside-in Repair Technique for Chronic Retracted, Unstable Bucket-Handle Anterior Horn Lateral Meniscal Tear.

Project description:The meniscus plays a very important role in normal functioning of the knee joint. It acts as a shock absorber, gives stability to the knee joint, delays the onset of osteoarthritis, provides load sharing, and so on. It is a well-known fact that after meniscectomy, there is a significant increase in joint reaction forces leading to early osteoarthritis. Hence, in the past decade, meniscal repair has gain enormous importance and various techniques have been developed to repair the meniscus. Conventionally, posterior-third meniscal tears are repaired with all-inside and outside-in techniques whereas middle-third meniscal tears are repaired with all 3 methods: inside out, outside in, and all inside. For tears of the anterior horn of the meniscus, only the outside-in method is used. We have developed a method for repair of chronic unreduced, retracted bucket-handle anterior horn lateral meniscal tears, in which we combined the use of both the outside-in and all-inside methods of repair. Hence, this method is called modified outside-in meniscal repair for chronic retracted bucket-handle anterior horn lateral meniscal tears.
