ABSTRACT: Pericardiocentesis is a challenging procedure and complications may vary depending on the patient-specific risk factors and procedural indications. Cardiac chamber perforation and the subsequent insertion of pigtail catheter into the main pulmonary artery are an unreported mishap during attempted pericardiocentesis. This potentially life-threatening complication is completely preventable by identification of high-risk patients and appropriate use of available technologies. Adjunctive imaging decreases procedural risk for difficult-to-access pericardial fluid collections and must be used to prevent inadvertent morbidities.

The nasty neighbor effect in humans.

Project description:Like other group-living species, humans often cooperate more with an in-group member than with out-group members and strangers. Greater in-group favoritism should imply that people also compete less with in-group members than with out-group members and strangers. However, in situations where people could invest to take other's resources and invest to protect against exploitation, we observed the opposite. Akin to what in other species is known as the "nasty neighbor effect," people invested more when facing an in-group rather than out-group member or stranger across 51 nations, in different communities in Kenya, and in representative samples from the United Kingdom. This "nasty neighbor" behavior is independent of in-group favoritism in trust and emerges when people perceive within-group resource scarcity. We discuss how to reconcile that humans exhibit nastiness and favoritism toward in-group members with existing theory on in-group favoritism.

Pericardiocentesis: ultrasound guidance is essential.

Project description:BackgroundPericardial effusion is a common entity which may have important implications in patient's prognosis. In several cases, pericardiocentesis is indicated for diagnostic and/or therapeutic purposes.Case presentationA blind pericardiocentesis failed in a 95-year-old woman admitted to the emergency department with a large pericardial effusion incidentally diagnosed in the ambulatory setting. Ultrasound-guided pericardiocentesis aided in easily accessing to the pericardial cavity, without periprocedural complications.ConclusionsUltrasound-guided pericardiocentesis is simple, safe and effective, and should replace the blind technique. This procedure should be part of the armamentarium of ultrasound-guided practices of emergency or critical care physicians.

Pneumopericardium: A Rare Complication of Pericardiocentesis.

Project description:Pneumopericardium is defined by the presence of air in the pericardial cavity. It is a rare entity occurring most commonly after trauma. Pneumopericardium resulting after pericardiocentesis is even rarer. We report a case of 46-year-old man, with end-stage renal disease on chronic hemodialysis and who developed a large circumferential pericardial effusion of 40 mm in diastole with swinging heart and diastolic right atrium collapse requiring pericardiocentesis. Few days after, the patient complained of pleuritic chest pain and echocardiogram revealed several tiny sparkling echogenic spots swirling in the pericardial sac. Computed tomography scans revealed a marked anterior pneumopericardium that was conservatively managed.

Efficacy and mortality of rotating sheaths versus laser sheaths for transvenous lead extraction: a meta-analysis.

Project description:BackgroundRotating and laser sheaths are both routinely used in transvenous lead extraction (TLE) which can lead to catastrophic complications including death. The efficacy and risk of each approach are uncertain. To perform a meta-analysis to compare success and mortality rates associated with rotating and laser sheaths.MethodsWe searched electronic academic databases for case series of consecutive patients and randomized controlled trials published 1998-2017 describing the use of rotating and laser sheaths for TLE. Among 48 studies identified, rotating sheaths included 1,094 patients with 1,955 leads in 14 studies, and laser sheaths included 7,775 patients with 12,339 leads in 34 studies. Patients receiving rotating sheaths were older (63 versus 60 years old) and were more often male (74% versus 72%); CRT-P/Ds were more commonly extracted using rotating sheaths (12% versus 7%), whereas ICDs were less common (37% versus 42%), p > 0.05 for all. Infection as an indication for lead extraction was higher in the rotating sheath group (59.8% versus 52.9%, p = 0.002). The mean time from initial lead implantation was 7.2 years for rotating sheaths and 6.3 years for laser sheaths (p > 0.05).ResultsSuccess rates for complete removal of transvenous leads were 95.1% in rotating sheaths and 93.4% in laser sheaths (p < 0.05). There was one death among 1,094 patients (0.09%) in rotating sheaths and 66 deaths among 7,775 patients (0.85%) in laser sheaths, translating to a 9.3-fold higher risk of death with laser sheaths (95% CI 1.3 to 66.9, p = 0.01).ConclusionsLaser sheaths were associated with lower complete lead removal rate and a 9.3-fold higher risk of death.

Ultrasound-guided pericardiocentesis: a novel parasternal approach.

Project description:ObjectiveThe aim of this study was to evaluate a novel pericardiocentesis technique using an in-plane parasternal medial-to-lateral approach with the use of a high-frequency probe in patients with cardiac tamponade.BackgroundEchocardiography is pivotal in the diagnosis of pericardial effusion and tamponade physiology. Ultrasound guidance for pericardiocentesis is currently considered the standard of care. Several approaches have been described recently, which differ mainly on the site of puncture (subxiphoid, apical, or parasternal). Although they share the use of low-frequency probes, there is absence of complete control of needle trajectory and real-time needle visualization. An in-plane and real-time technique has only been described anecdotally.Methods and resultsA retrospective analysis of 11 patients (63% men, mean age: 37.7±21.2 years) presenting with cardiac tamponade admitted to the tertiary-care emergency department and treated with parasternal medial-to-lateral in-plane pericardiocentesis was carried out. The underlying causes of cardiac tamponade were different among the population. All the pericardiocentesis were successfully performed in the emergency department, without complications, relieving the hemodynamic instability. The mean time taken to perform the eight-step procedure was 309±76.4 s, with no procedure-related complications.ConclusionThe parasternal medial-to-lateral in-plane pericardiocentesis is a new technique theoretically free of complications and it enables real-time monitoring of needle trajectory. For the first time, a pericardiocentesis approach with a medial-to-lateral needle trajectory and real-time, in-plane, needle visualization was performed in a tamponade patient population.

'Nasty neighbours' rather than 'dear enemies' in a social carnivore.

Project description:Territorial animals typically respond less aggressively to neighbours than to strangers. This 'dear enemy effect' has been explained by differing familiarity or by different threat levels posed by neighbours and strangers. In most species, both the familiarity and the threat-level hypotheses predict a stronger response to strangers than to neighbours. In contrast, the threat-level hypothesis predicts a stronger response to neighbours than to strangers in species with intense competition between neighbours and with residents outnumbering strangers, as commonly found in social mammals such as the banded mongoose (Mungos mungo). The familiarity hypothesis predicts reduced aggression towards neighbours also in these species. We exposed free-living banded mongoose groups to translocated scent marks of neighbouring groups and strangers. Groups vocalized more and inspected more samples in response to olfactory cues of the neighbours than to the strangers. Our results support the threat-level hypothesis and contradict the familiarity hypothesis. We suggest that increased aggression towards neighbours is more common in social species with intense competition between neighbours, as opposed to reduced aggression towards neighbours typical for most solitary species.

Nasty and Noble Notes: Interdependence Structures Drive Self-Serving Gossip.

Project description:Much information people receive about others reaches them via gossip. But is this gossip trustworthy? We examined this in a scenario study (Nsenders = 350, Nobservations = 700) and an interactive laboratory experiment (Nsenders = 126; Nobservations = 3024). In both studies, participants played a sequential prisoner's dilemma where a gossip sender observed a target's (first decider's) decision and could gossip about this to a receiver (second decider). We manipulated the interdependence structure such that gossipers' outcomes were equal to targets' outcomes, equal to receivers' outcomes, or independent. Compared to no interdependence, gossip was more often false when gossipers were interdependent with targets but not when interdependent with receivers. As such, false positive gossip (self-serving when interdependent with targets) increased but false negative gossip (self-serving when interdependent with receivers) did not. In conclusion, the interdependence structure affected gossip's trustworthiness: When gossipers' outcomes were interdependent with targets, gossip was less trustworthy.

Logistics hub location for high-speed rail freight transport with road-rail intermodal transport network.

Project description:In this paper, we propose a new freight mode to describe how the designed HSR freight train serve for express delivery. We introduce the functions of the hubs and design the hybrid hub-and-spoke network of road-rail intermodal transportation from the perspective of planners, which features as single allocation rule and configures varying levels of hubs. The problem is accurately described by a mixed integer programming model with the objective to minimize total construction cost and total operation cost. We develop a hybrid heuristic algorithm based on a greedy strategy to obtain the levels of hubs, customer allocation and cargo routing. Taking the example of HSR freight network consisting of 50 cities in China, numerical experiments are conducted on the basis of forecasting data from the real-life express market to obtain the hub location schemes. The validity of the model and the performance of the algorithm are verified.
