Partnership Choice and Childbearing in Norway and Spain.

ABSTRACT: Cohabitation has, in a number of countries, become a genuine alternative to marriage. Where this occurs, will we see a convergence in fertility behavior between the two partnership options? We address this question by comparing two societies, Norway and Spain, that contrast sharply not only in the evolution of cohabitation, but also in overall birth rates and public support for families. Using the Generations and Gender Survey for Norway (2007/2008) and the most recent Fertility, Family and Values Survey for Spain (2006), we estimate a three-equation multi-process model for selection into a union and fertility in order to take into account unobserved heterogeneity. For Norway, we find a significant association between selection into either partnership type and fertility, whereas for Spain, a newcomer to cohabitation, we find a significant association between fertility and selection into marriage.

Project description:ObjectivesThe Choice and Partnership Approach (CAPA) was developed to create an accessible, child-centred and family-centred model of child and adolescent mental health service delivery that is adaptable to different settings. We sought to describe the state of evidence regarding the extent, outcomes and contextual considerations of CAPA implementation in community mental health services.DesignScoping review.Data sourcesPublished and grey literature were searched using MEDLINE, Embase, CINAHL, PsycINFO, Scopus and Google to 13 and 20 July 2022, respectively.Eligibility criteriaWe included reports focused on the implementation, outcomes (clinical, programme or system) or a discussion of contextual factors that may impact CAPA implementation in either child and adolescent or adult mental health services.Data extraction and synthesisData were extracted using a codebook that reflected the five domains of the Consolidated Framework for Implementation Research (CFIR) and reviewed for agreement and accuracy. Data were synthesised according to the five CFIR domains.ResultsForty-eight reports describing 36 unique evaluations were included. Evaluations were observational in nature; 10 employed pre-post designs. CAPA implementation, regardless of setting, was largely motivated by long wait times. Characteristics of individuals (eg, staff buy-in or skills) were not reported. Processes of implementation included facilitative leadership, data-informed planning and monitoring and CAPA training. Fidelity to CAPA was infrequently measured (n=9/36) despite available tools. Health system outcomes were most frequently reported (n=28/36); few evaluations (n=7/36) reported clinical outcomes, with only three reporting pre/post CAPA changes.ConclusionsGaps in evidence preclude a systematic review and meta-analysis of CAPA implementation. Measurement of clinical outcomes represents an area for significant improvement in evaluation. Consistent measurement of model fidelity is essential for ensuring the accuracy of outcomes attributed to its implementation. An understanding of the change processes necessary to support implementation would be strengthened by more comprehensive consideration of contextual factors.

Project description:BackgroundPatients and their families play an important role in efforts to improve health service safety.ObjectiveThe objective of this study is to understand the safety partnership preferences of patients and their families.MethodWe used a discrete choice conjoint experiment to model the safety partnership preferences of 1,084 patients or those such as parents acting on their behalf. Participants made choices between hypothetical safety partnerships composed by experimentally varying 15 four-level partnership design attributes.ResultsParticipants preferred an approach to safety based on partnerships between patients and staff rather than a model delegating responsibility for safety to hospital staff. They valued the opportunity to participate in point of service safety partnerships, such as identity and medication double checks, that might afford an immediate risk reduction. Latent class analysis yielded two segments. Actively engaged participants (73.3%) comprised outpatients with higher education, who anticipated more benefits to safety partnerships, were more confident in their ability to contribute, and were more intent on participating. They were more likely to prefer a personal engagement strategy, valued scientific evidence, preferred a more active approach to safety education, and advocated disclosure of errors. The passively engaged segment (26.7%) anticipated fewer benefits, were less confident in their ability to contribute, and were less intent on participating. They were more likely to prefer an engagement strategy based on signage. They preferred that staff explain why they thought patients should help make care safer and decide whether errors were disclosed. Inpatients, those with immigrant backgrounds, and those with less education were more likely to be in this segment.ConclusionHealth services need to communicate information regarding risks, ask about partnership preferences, create opportunities respecting individual differences, and ensure a positive response when patients raise safety concerns.

Project description:BackgroundSub-Saharan Africa faces high neonatal and maternal mortality rates due to limited access to skilled healthcare during delivery. This study aims to improve the classification of health facilities and home deliveries using advanced machine learning techniques and to explore factors influencing women's choices of delivery locations in East Africa.MethodThe study focused on 86,009 childbearing women in East Africa. A comparative analysis of 12 advanced machine learning algorithms was conducted, utilizing various data balancing techniques and hyperparameter optimization methods to enhance model performance.ResultThe prevalence of health facility delivery in East Africa was found to be 83.71%. The findings showed that the support vector machine (SVM) algorithm and CatBoost performed best in predicting the place of delivery, in which both of those algorithms scored an accuracy of 95% and an AUC of 0.98 after optimized with Bayesian optimization tuning and insignificant difference between them in all comprehensive analysis of metrics performance. Factors associated with facility-based deliveries were identified using association rule mining, including parental education levels, timing of initial antenatal care (ANC) check-ups, wealth status, marital status, mobile phone ownership, religious affiliation, media accessibility, and birth order.ConclusionThis study underscores the vital role of machine learning algorithms in predicting health facility deliveries. A slight decline in facility deliveries from previous reports highlights the urgent need for targeted interventions to meet Sustainable Development Goals (SDGs), particularly in maternal health. The study recommends promoting facility-based deliveries. These include raising awareness about skilled birth attendance, encouraging early ANC check-up, addressing financial barriers through targeted support programs, implementing culturally sensitive interventions, utilizing media campaigns, and mobile health initiatives. Design specific interventions tailored to the birth order of the child, recognizing that mothers may have different informational needs depending on whether it is their first or subsequent delivery. Furthermore, we recommended researchers to explore a variety of techniques and validate findings using more recent data.

Project description:IntroductionEarly identification and appropriate treatment of child and adolescent mental health disorders can often be hampered by patchwork services with poorly planned or unclear pathways. The Choice and Partnership Approach (CAPA) is an evidence-based transformational model of community (community-based or outpatient) mental health and addictions services for children and adolescents that aims to better match services to needs and to improve timely access to care. CAPA has been variably implemented across jurisdictions but has not been comprehensively evaluated for its impact on system and client outcomes. Our research question is, 'To what degree does CAPA work, for whom and under what circumstances?'. The purpose of this review is twofold: (1) to gain an understanding of the extent and outcomes of the implementation of CAPA in community mental health and addictions services; and (2) to identify the role of context as it influences the implementation of CAPA and resulting client and system outcomes.Methods and analysisWe will conduct a realist-informed scoping review of the literature related to CAPA in either child and adolescent or adult community mental health and addictions services. Relevant studies, reports and documentation will be identified by searching the following online databases: MEDLINE, Embase, CINAHL, PsycINFO, Academic Search Premier, ERIC, Web of Science, Cochrane, Dissertations Abstracts, NCBI Bookshelf, PubMed Central and the Canadian Health Research Collection. The search strategy was developed by a health sciences library scientist and informed by a multidisciplinary team comprising methodological and content knowledge experts. The search will gather evidence from multiple online databases of peer-reviewed literature and grey literature repositories. All articles will be independently assessed for inclusion by pairs of reviewers. The key themes derived from a thematic analysis of extracted data will be presented in a narrative overview.Ethics and disseminationResearch ethics review is not required for this scoping review. The results will be disseminated through meetings with stakeholders (including clients and families, clinicians and decision-makers), conference presentations and peer-reviewed publication. The results of this review will inform an overarching programme of research, policy and quality indicator development to ultimately improve mental health and addictions care and subsequent mental health outcomes for children and adolescents.

Project description:IntroductionImmunization is the most effective strategy for the prevention of invasive meningococcal disease caused by Neisseria meningitidis serogroup B (MenB); however, parents need to weigh the risk-benefit and financial impact of immunizing their children against MenB in the absence of a national immunization program (NIP). This study aimed to explore societal preferences (of parents and pediatricians) regarding the attributes of a MenB vaccine in Spain.MethodsA discrete choice experiment (DCE) based on cross-sectional surveys was carried out to determine preferences. A literature review and scientific committee determined the six attributes related to the MenB vaccine included in the DCE: vaccination age, cost, duration, percentage of protection, adverse events probability, and expert/authority recommendation. Data were analyzed using a mixed logit model. Relative importance (RI) of attributes was calculated and compared between parents and pediatricians.ResultsA total of 278 parents [55.8% female, mean age 40.4 (standard deviation, SD 7.3) years] and 200 pediatricians [73.0% female, mean age 45.8 (SD 12.9) years] answered the DCE. For parents, the highest RI was attributed to vaccine cost, expert/authority recommendation, and percentage of protection (26.4%, 26.1%, and 22.9%, respectively), while for pediatricians the highest RI was assigned to percentage of protection, expert/authority recommendation, and vaccination age (27.2%, 23.7%, and 22.6%, respectively). Significant differences between parents and pediatricians were found in the RI assigned to all attributes (p < 0.001), except for vaccine recommendation.ConclusionIn the decision regarding MenB vaccination, cost was a driver in parental decision-making but had a low RI for pediatricians and, conversely, vaccination age was highly valued by pediatricians but was the attribute with least importance for parents. Despite these differences, expert/authority recommendation and percentage of protection were essential criteria for both groups. These results provide relevant information about MenB vaccination, highlighting the importance of considering societal preferences for NIP inclusion.
