Anxiety in Rural Chinese Children and Adolescents: Comparisons across Provinces and among Subgroups.

ABSTRACT: China's competitive education system has produced notably high learning outcomes, but they may be costly. One potential cost is high levels of anxiety. China has launched several initiatives aimed at improving student mental health. However, little is known about how effective these programs and policies are. The goal of this paper was to examine anxiety levels among children and adolescents in rural China, and to identify which subpopulations were particularly vulnerable to anxiety. Data were aggregated from 10 different school-level surveys conducted in rural areas of five provinces between 2008 and 2015. In total, 50,361 students were evaluated using the 100-item, nine-subcategory Mental Health Test (a variation of the Children's Manifest Anxiety Scale). Seven percent of students were at risk for overall anxiety. However, over half of students were at risk for at least one subcategory of anxiety. Students at higher risk for anxiety included students from poorer counties and families, female students, secondary school students, and students with lower levels of academic performance. Many students in rural China are at risk for anxiety, and certain student subpopulations are particularly vulnerable. We suggest that China's government review and update student mental health programs and policies.

Project description:In light of the novel coronavirus's (COVID-19's) threat to public health worldwide, we sought to elucidate COVID-19's impacts on the mental health of children and adolescents in China. Through online self-report questionnaires, we aimed to discover the psychological effects of the pandemic and its associated risk factors for developing mental health symptoms in young people. We disseminated a mental health survey through online social media, WeChat, and QQ in the five Chinese provinces with the most confirmed cases of COVID-19 during the late stage of the country-wide lockdown. We used a self-made questionnaire that queried children and adolescents aged 6 to 18 on demographic information, psychological status, and other lifestyle and COVID-related variables. A total of 17,740 children and adolescents with valid survey data participated in the study. 10,022 (56.5%), 11,611 (65.5%), 10,697 (60.3%), 6868 (38.7%), and 6225 (35.1%) participants presented, respectively, more depressive, anxious, compulsive, inattentive, and sleep-related problems compared to before the outbreak of COVID-19. High school students reported a greater change in depression and anxiety than did middle school and primary school students. Despite the fact that very few children (0.1%) or their family members (0.1%) contracted the virus in this study, the psychological impact of the pandemic was clearly profound. Fathers' anxiety appeared to have the strongest influence on a children's psychological symptoms, explaining about 33% of variation in the child's overall symptoms. Other factors only explained less than 2% of the variance in symptoms once parents' anxiety was accounted for. The spread of COVID-19 significantly influenced the psychological state of children and adolescents in participants' view. It is clear that children and adolescents, particularly older adolescents, need mental health support during the pandemic. The risk factors we uncovered suggest that reducing fathers' anxiety is particularly critical to addressing young people's mental health disorders in this time.

Project description:BackgroundSchool bullying victimization is associated with increased risk of anxiety in children and adolescents. However, little is known about the role of resilience in this association. The purpose of this study was to investigate the possible mediation by resilience in this association in a large group of Chinese children and adolescents.MethodsA population-based cross-sectional study adopting two-stage simple random cluster sampling was implemented in Yunnan province, southwestern China. A comprehensive questionnaire was used to collect information from the participants. Among all the participants, 4624 were included in the final analysis. Descriptive statistics were used to present general characteristics of the study participants. Univariate and multivariate logistic regression models were adopted to estimate crude and adjusted associations among bullying victimization, anxiety, and resilience. A path model was used to analyze the hypothesized mediation by resilience in the association between bullying victimization and anxiety.ResultsAnalytical results of multivariate logistic regression models suggested that bullying victimization was significantly associated with anxiety in Chinese children and adolescents; compared with individuals who had not been bullied, victims of bullying were more likely to experience anxiety symptoms, with an adjusted odds ratio (OR) of 3.02 (95% CI 2.46-3.71). Path analysis revealed a prominent mediating effect of resilience in the association between bullying victimization and anxiety, accounting for 31.89% of the total association. Further analysis indicated that, among the five dimensions of resilience, emotional regulation, interpersonal assistance, and family support were significant mediators, accounting for 30.34%, 10.79%, and 8.35% of the total association.ConclusionsOur major findings highlighted the promising role of resilience-based intervention measures in reducing the risk of anxiety associated with school bullying victimization in Chinese children and adolescents.

Project description:The rational flow and optimal allocation of urban and rural factors is the key to solving the problem of unbalanced and insufficient urban and rural development. This study draws on the theory of ecological symbiosis to examine the mechanism of factor flow and urban-rural symbiosis. It analyzes panel data from 27 Chinese provinces and autonomous regions between 2010 and 2020 to empirically demonstrate the influence of labor, capital, technology, and land mobility on urban-rural symbiosis. The study found that: (1) The relationship between the total factor flow and urban-rural symbiosis is U-shaped nonlinear, and the time when cities feed the development of rural areas has come; (2) The impact of labor factor flow on urban-rural symbiosis exhibits an inverted U-shaped relationship, the impact of capital factor flow displays a positive U-shaped relationship, the impact of land factor flow shows an inverted U-shaped relationship, and the impact of technical factor flow on urban-rural symbiotic development is not significant. (3) The factor flow exerts a region-oriented impact on the level of urban-rural symbiosis. In developed areas, total factor flow does not affect urban-rural symbiosis, but the level of labor flow and the urban-rural symbiosis demonstrates an inverted U-shaped relationship, the flow of technological factors has a U-shaped impact on the urban-rural symbiosis level, the flow of land factors and the urban-rural symbiosis show an inverted U-shaped relationship, and the flow of capital factors has no impact on the urban-rural symbiosis relationship. In underdeveloped areas, the impact of the total factor flow on urban-rural symbiosis shows a significantly positive U-shaped characteristic, the labor mobility level and urban-rural symbiosis show an inverted U-shaped relationship, the flow of capital factors has a U-shaped impact on the urban-rural symbiosis level, and the flow of lands and technology does not have a remarkable impact on urban-rural symbiosis in underdeveloped areas.

Project description:BackgroundIgA vasculitis is the most common form of vasculitis in children. Vitamin D deficiency has been observed to contribute to immune function and the pathogenesis of various immune diseases. However, at present, only a few studies with small sample sizes have shown that IgA vasculitis patients have lower vitamin D levels than healthy children. Thus, we conduct a large study to investigate the significance of serum 25-hydroxyvitamin D3 (25(OH)D) levels of children with IgA vasculitis across different subgroups and healthy children.MethodsIn this retrospective study, 1,063 children were recruited from the Ningbo Women and Children's Hospital between February 2017 and October 2019, including 663 patients hospitalized with IgA vasculitis and 400 healthy examination children who served as the control group at the same time. There wasn't any bias in the season. The healthy group came from children who underwent normal physical examination. The 663 IgA vasculitis patients were then divided into the IgA vasculitis-nephritis and non-IgA vasculitis-nephritis groups, streptococcal-infection and no-streptococcal-infection groups, gastrointestinal-involvement and no-gastrointestinal-involvement groups, and joint-involvement and no-joint-involvement groups. The serum 25(OH)D levels at disease onset were analyzed. All the participants were followed up for 6 months from the date of onset.ResultsThe serum 25(OH)D levels of the IgA vasculitis group (15.47±6.58 ng/mL) were significantly lower than those of the healthy control group (22.48±6.24 ng/mL) (P<0.01). There were no significant differences in terms of age and sex between the IgA vasculitis and healthy control group. Further, among the IgA vasculitis patients serum 25(OH)D levels were reduced in the IgA vasculitis-nephritis (12.99±4.92 ng/mL), streptococcal-infection (14.2±6.06 ng/mL), and gastrointestinal-involvement (14.43±6.33 ng/mL) groups (P=0.00, 0.004, 0.002, respectively). The vitamin D levels with IgA vasculitis were significantly lower in winter and spring than summer and autumn. Conversely, the joint-involvement group did not show a significant reduction in vitamin D levels compared to no joints involved group.ConclusionsIgA vasculitis patients have reduced vitamin D levels, which suggests that vitamin D deficiency may be involved in the development of IgA vasculitis. Vitamin D supplementation may reduce the incidence of IgA vasculitis, and maintaining high vitamin D levels in IgA vasculitis patients may prevent renal damage.
