Determinants of China's development assistance for health at the sub-national level of African countries (2006-2015).

ABSTRACT:

Background

Despite the increasing interest in China's development assistance for health (DAH) in African countries, little is known regarding the distribution and determinants of China's DAH project allocation among the principle subdivisions (provinces & states) within African countries.

Methods

We mapped the distribution of China's DAH projects in 670 principle subdivisions of 50 African countries during 2006-2015 using web-based information. The political, demographic, health and socioeconomic indicators of DAH allocation were analyzed using conditional logistic regression models. The national capital city and political leader's birth place were selected as the main political indicators, and health indicators were selected according to different fields of the DAH projects.

Results

China's DAH projects (mainly China medical teams [CMTs], hospitals and anti-malaria centers) were mostly allocated to the western and eastern coasts of Africa, although CMTs were also dispatched to northern Africa. National capital cities were significantly associated with the allocation of China's DAH projects (P < 0.001). Anti-malaria centers were more likely to be allocated to principle subdivisions with larger populations (OR = 1.35), and CMTs were allocated to subdivisions with high population densities (OR = 79.01). No health-related indicators were identified to affect project allocation except for the facility delivery rate and under-five mortality rate, which were associated with hospital allocation. We also found an association between CMT allocation and the use of artemisinin-based combination therapy in children.

Conclusions

Allocation of China's DAH projects is strongly affected by political and demographic factors. Implementation of China's new DAH projects should target health and socio-economic indicators and impact metrics in scaling up tailored and cost-effective programs in Africa.

Project description:BackgroundAccelerating declines in tuberculosis (TB) incidence is paramount for achieving global goals set for 2030 by the Sustainable Development Goals and the End TB Strategy. The aim of this study was to identify key country-level social determinants of national TB incidence trends.MethodsThis longitudinal ecological study used country-level data extracted from online databases from the period 2005-2015. We used multivariable Poisson regression models allowing for distinct within- and between-country effects to estimate associations between national TB incidence rates and 13 social determinants of health. The analysis was stratified by country income status.ResultsThe study sample included 48 low- and lower-middle-income countries (LLMICs) and 68 high- and upper-middle income countries (HUMICs), with a total of 528 and 748 observations between 2005-2015, respectively. National TB incidence rates declined in 108/116 countries between 2005-2015, with an average drop of 12.95% in LLMICs and 14.09% in HUMICs. Between LLMICs, higher Human Development Index (HDI), social protection spending, TB case detection, and TB treatment success were associated with lower TB incidence. Higher prevalence of HIV/AIDS was associated with higher TB incidence. Within LLMICs, increases in HDI over time were associated with lower TB incidence rates. Between HUMICs, higher HDI, health spending, and diabetes prevalence were associated with lower TB incidence, whereas higher prevalence of HIV/AIDS and alcohol-use were associated with higher TB incidence. Within HUMICs, increases in HIV/AIDS and diabetes prevalence over time were associated with higher TB incidence.ConclusionsIn LLMICs, TB incidence rates remain highest in countries with low human development, social protection spending and TB programme performance, and high rates of HIV/AIDS. Strengthening human development is likely to accelerate declines in TB incidence. In HUMICs, TB incidence rates remain highest in countries with low human development, health spending and diabetes prevalence, and high rates of HIV/AIDS and alcohol use. Here, slowing rising rates of HIV/AIDS and diabetes is likely to accelerate declines in TB incidence.

Project description:BackgroundThe aim of the study was to analyse trends in the relationship between mother's educational level and mortality of children under the year of five in Sub-Saharan Africa, from 1990 to 2015.Data and methodsData used in this study came from different waves of Demographic and Health Surveys (DHS) of Sub-Saharan countries. Logistic regression and Buis's decomposition method were used to explore the effect of mother's educational level on the mortality of children under five years.ResultsAlthough the results of our study in the selected countries show that under-five mortality rates of children born to mothers without formal education are higher than the mortality rates of children of educated mothers, it appears that differences in mortality were reduced over the past two decades. In selected countries for our study, we noticed a significant decline in mortality among children of non-educated mothers compared to the decrease in mortality rates among children of educated mothers during the period of 1990-2010. The results show that the decline in mortality of children under five years was much higher among the children born to mothers who have never received formal education-112 points drop in Malawi, over 80 in Zambia and Zimbabwe, 65 points in Burkina Faso, 56 in Congo, 43 in Namibia, 27 in Guinea, Cameroon, and 22 to 15 in Niger. However, we noted a variation in results among the countries selected for the study-in Burkina Faso (OR = 0.7), in Cameroon (OR = 0.8), in Guinea (OR = 0.8) and Niger (OR = 0.8). It is normally observed that children of mothers with 0-6 years of education are about 20% more likely to survive until their fifth year compared to children of mothers who have not been to school. Conversely, the results did not reveal significant differences between the under-five deaths of children born to non-educated mothers and children of low-level educated mothers in Congo, Malawi and Namibia.ConclusionThe decline in under-five mortality rates, during last two decades, can be partly due to the government policies on women's education. It is evident that women's educational level has resulted in increased maternal awareness about infant health and hygiene, thereby bringing about a decline in the under-five mortality rates. This reduction is due to improved supply of health care programmes and health policies in reducing economic inequalities and increasing access to health care.

Project description:Optimal birth spacing (defined as a birth spacing of 24-59 months) is incontrovertibly linked to better health outcomes for both mothers and babies. Using the most recent available Demographic and Health Survey data, we examined the patterns and determinants of short and long birth intervals among women in selected sub-Saharan African (SSA) countries.Reproductive health and sociodemographic data of 98,934 women from 8 SSA countries were analyzed. Unadjusted and adjusted multinomial logistic regression models were used to examine the net relationship between all the independent variables and short and long birth intervals.Overall, the majority of women in all the countries optimally spaced births. However, a significant proportion of women had short birth intervals in Chad (30.2%) and the Democratic Republic of Congo (Congo DRC) (27.1%). Long birth spacing was more common in Eastern and Southern African countries, with Zimbabwe having the highest rate of long term birth interval (27.0%). Women who were aged 35 years and above in Uganda (RRR = 0.72, CI = 0.60-0.87), Tanzania (RRR = 0.62, CI = 0.49-0.77), Zimbabwe (RRR = 0.52, CI = 0.31-0.85), Nigeria (RRR = 0.82, CI = 0.72-0.94) and Togo (RRR = 0.67, CI = 0.46-0.96) had significantly lower odds of having short birth intervals compared to women aged 15-24 years. Older women (above 34 years) had increased odds for long birth intervals in all countries studied (Chad (RRR = 1.44, CI = 1.18-1.76), Congo DRC (RRR = 1.73, CI = 1.33-2.15), Malawi (RRR = 1.54, CI = 1.23-1.94) Zimbabwe (RRR = 1.95, CI = 1.26-3.02), Nigeria (RRR = 1.85 CI = 1.56-2.20), Togo (RRR = 2.12, CI = 1.46-3.07), Uganda (RRR = 1.48, CI = 1.15-1.91), Tanzania RRR = 2.12, CI = 1.53-2.93).The analysis suggested that the determinants of long and birth intervals differ and varies from country to country. The pattern of birth spacing found in this study appears to mirror the contraceptive use and fertility rate in the selected SSA countries. Birth intervals intervention addressing short birth intervals should target younger women in SSA, especially in Chad and Congo DRC, while intervention for long birth spacing should prioritize older, educated and wealthy women.

Project description:ObjectivesBreast cancer is the most prevalent cancer and the second leading cause of cancer-related deaths among women after cervical cancer in much of sub-Saharan Africa. This study aims to examine the prevalence and sociodemographic-socioeconomic factors associated with breast cancer screening among women of reproductive age in sub-Saharan Africa.DesignA weighted population-based cross-sectional study using Demographic and Health Surveys (DHS) data. We used all available data on breast cancer screening from the DHS for four sub-Saharan African countries (Burkina Faso, Ivory Coast, Kenya and Namibia). Breast cancer screening was the outcome of interest for this study. Multivariable Poisson regression was used to identify independent factors associated with breast cancer screening.SettingFour countries participating in the DHS from 2010 to 2014 with data on breast cancer screening.ParticipantsWomen of reproductive age 15-49 years (N=39 646).ResultsThe overall prevalence of breast cancer screening was only 12.9% during the study period, ranging from 5.2% in Ivory Coast to 23.1% in Namibia. Factors associated with breast cancer screening were secondary/higher education with adjusted prevalence ratio (adjusted PR)=2.33 (95% CI: 2.05 to 2.66) compared with no education; older participants, 35-49 years (adjusted PR=1.73, 95% CI : 1.56 to 1.91) compared with younger participants 15-24 years; health insurance coverage (adjusted PR=1.57, 95% CI: 1.47 to 1.68) compared with those with no health insurance and highest socioeconomic status (adjusted PR=1.33, 95% CI : 1.19 to 1.49) compared with lowest socioeconomic status.ConclusionDespite high breast cancer mortality rates in sub-Saharan Africa, the prevalence of breast cancer screening is substantially low and varies gradually across countries and in relation to factors such as education, age, health insurance coverage and household wealth index level. These results highlight the need for increased efforts to improve the uptake of breast cancer screening in sub-Saharan Africa.

Project description:BackgroundEnsuring minimum dietary diversity is crucial for lactating mothers. It allows them to consume a variety of foods from different groups, meeting their nutritional needs and supporting maternal and infant health. Despite the global impact of undernutrition and micronutrient deficiencies, the prevalence and determinants of minimum dietary diversity during lactation remain poorly recognized in developing countries. This research aims to assess minimum dietary diversity and its determinants among lactating mothers in five Sub-Saharan African countries.MethodsData from the most recent Demographic and Health Surveys, which covered five Sub-Saharan African countries between 2021 and 2023, were used to execute secondary data analysis. This study included 19,917 lactating mothers in total. Using a multilevel mixed-effects logistic regression model, the variables associated to the minimum dietary diversity were established. Significant factors associated with the minimum dietary diversity were found at p-values < 0.05. The adjusted odds ratio and 95% CI were used to interpret the result. The model with the highest loglikelihood ratio and lowest deviance was declared to be the best fit.ResultsThe magnitude of minimum dietary diversity among lactating mothers in Burkina Faso, Ghana, Kenya, Mozambique, and Tanzania was 25.66% (95% CI: 24.47, 25.75). Factors such as secondary and above educational level (AOR = 1.38, 95% CI: 1.18, 1.61), employed mothers (AOR = 1.40, 95% CI: 1.26, 1.56), distance which was not a big problem to access health facilities (AOR = 1.35, 95% CI: 1.21, 1.51), health facility delivery (AOR = 1.25, 95% CI: 1.08, 1.45), rich wealth status (AOR = 1.86, 95% CI: 1.60, 2.17), high community ANC utilization (AOR = 1.18, 95% CI: 1.04, 1.35), and reside in Ghana (AOR = 4.21, 95% CI: 3.60, 4.94) had higher odds of minimum dietary diversity.ConclusionsThis study reveals that lactating mothers have low dietary diversity. Both community-level and individual-level factors impact this diversity. Consequently, health ministries in Burkina Faso, Kenya, Ghana, Mozambique, and Tanzania should prioritize women who underutilize antenatal services and those without formal education when designing strategies and policies.
