Project description:Liver disease is highly prevalent in Africa, especially in the western African country Burkina Faso, due to the presence of multiple biological and chemical aggressors of the liver. Furthermore, diagnosis and appropriate care for liver disease are uneven and usually insufficient. This drives local communities to turn to folk medicine based on medicinal plants from healers. Small scale, ethnopharmacological studies on reputed hepatoprotective plants have been carried out in defined regions worldwide, but so far, no study has been carried out on a countrywide scale. Therefore, we have explored traditional healers' practices in all thirteen regions of Burkina Faso. We interviewed 575 healers and we compiled a database with 2,006 plant entries. Here, we report results on liver nosology, liver pathologies, medicinal plants used for liver disease, and traditional practices through the lens of Burkinabe healers. Our goal was to give a full inventory of medicinal plants used to treat liver disease and to determine if there was consensus on the use of specific plants for specific symptoms. Analysis of the medicinal plants in use across the whole country provides local communities with a wider evidence base to determine which plants may be more effective in treating liver disease and could provide the scientific community, with a shortlist of plants suitable for chemical and pharmacological investigation to validate the plants' therapeutic role.

Project description:IntroductionMalaria is an important parasitic disease that affects mostly the African continent. Traditional medicine is very important in Mozambique and traditional healers play a key role in the primary health care services, particularly in rural areas. We aim to report the results of an ethnobotanical survey undertaken in Mogovolas district, northern region of Mozambique. We recorded and identified the medicinal plants used by traditional healers for treatment of malaria, as well as the mode of preparation and administration.MethodsThe study was conducted in 14 villages from Mogovolas between June and August 2015. Sixteen traditional healers were interviewed using semi-structured questionnaires. Under their guidance, we collected medicinal plants and prepared herbarium specimens that were sent and kept at Eduardo Mondlane University Herbarium for scientific identification. We searched for information on the in vitro and in vivo studies of the cited plants for antiplasmodial activity.ResultsTraditional healers from Mogovolas district reported the use of 37 plants to treat malaria, belonging to 22 families. The most used species are Ochna kirkii Oliv. (5 citations), Ehretia amoena Klotzsch and Pteleopsis myrtifolia (M.A.Lawson) Engl. & Diels (both with 3 citations). These plants belong to Ochnaceae, Boraginaceae and Combretaceae families, respectively. The herbal remedies are prepared using leaves (22/37), roots (18/37), stem barks (16/37) and stems (3/37). The administration of the herbal remedies was made essentially by oral route and bathing.ConclusionThe ethnobotanical data resulted from this study can be the starting point for further chemical and pharmacological studies aiming to identify medicinal species with antimalarial activity, thus, open the insights for the discovery of new antimalarial substances, as well as better integration of the traditional medicine into the national health systems, particularly in developing countries, as the health system coverage is limited.

Project description:There is no ethnobotanical study conducted specifically on medicinal plants traditionally used to treat cancer in Ethiopia. Yet, traditional herbalists in different parts of the country claim that they have been treating cancer-like symptoms using herbal remedies. The objective of this study was to document medicinal plants traditionally used to treat cancer-like symptoms in eleven districts, Ethiopia. Traditional herbalists were interviewed using semistructured questionnaires, and field visits were also carried out to collect claimed plants for identification purpose. Seventy-four traditional herbalists, who claimed that they knew about and/or had used medicinal plants to treat cancer-like symptoms, were selected using the snowball method and interviewed. Herbalists used their intuition and relied on the chronicity, growth of external mass, and spreading of the disease to other parts of the body, as a means to characterize cancer symptoms. Furthermore, in some of the study districts, herbalists reported that they treat patients who had already been diagnosed in modern healthcare institutions prior to seeking help from them. The inventory of medicinal plants is summarized in a synoptic table, which contains the scientific and vernacular names of the plants, their geographical location, the parts of the plants, and the methods used to prepare the remedies. A total of 53 traditionally used anticancer plants, belonging to 30 families, were identified during the survey. The most frequently reported anticancer plants were Acmella caulirhiza Del (Asteraceae), Clematis simensis Fresen. (Ranunculaceae), Croton macrostachyus Del. (Euphorbiaceae), and Dorstenia barnimiana Schweinf. (Moraceae). Organizing traditional healers, documenting their indigenous knowledge, and scientifically validating it for the development of better cancer therapeutic agents constitute an urgent and important task for policymakers and scientists.

Project description:Rural communities in Kenya largely depend on plant resources for their livelihood. The utilization of these resources depends on the availability of plant resources and the level of knowledge of the residents. We conducted an ethnobotanical study in Tharaka-Nithi County in Kenya to determine the knowledge and utilization of various plant species by the local communities. The study was conducted in four major administrative regions from June 2018 to February 2019, involving interview schedules using semi-structured open-ended questionnaires and guided field collections with 48 informants. A total of 214 plant species distributed in 73 families and 169 genera with 616 Use Reports (URs) were documented. Fabaceae was the highest family cited by the informants (31 species) followed by Lamiaceae and Euphorbiaceae (each with 11 species). Trees (49%) and shrubs (32%) were the top life forms of the plants frequently utilized by the local residents. The general plant uses reported were medicinal, food, fodder, construction, fuel, pesticidal, religious, live fencing, and making crafts. Zanthoxylum gilletii, Prunus africana, and Solanum incanum were found to be highly valued by the local communities. Plant utilization as food and medicinal uses against snake-bite related problems had the highest Informant Consensus Factor (ICF). Only 29 (13.6%) of the species reported had their status assessed by the International Union for Conservation of Nature (IUCN). Conservation measures, alongside awareness creation in this region, are highly recommended for the species endemic to the region, highly depended on by the community, and those threatened according to IUCN standards.

Project description:Background: Throughout the history, nature has provided mankind with most of their basic needs, which include food, shelter, medicine, clothes, flavours, scents as well as raw materials. Given that they are an integral part of cultural heritage, medicinal plants have played a significant role in human healthcare systems around the world. Investigating various biological resources for use as medicines requires ethnomedicinal studies. Methods: Data on utilization of ethnomedicinal plants from local healers in Kenya's Mosop Sub-County in Nandi County was documented through open-ended, semi-structured questionnaires. A number of quantitative indices, such as the Use Citation (UC), Informant Consensus Factor (ICF), Use Value (UV), Frequency of Citation (FoC) and Relative Frequency of Citation (RFC) were used to convey the potential medical benefits, vitality and variety of the ethnomedicine. Results: 102 informants provided information on 253 ethnomedicinal plant species, classified into 74 families. There were 249 native plant species identified, along with few exotic species: Senegalia senegal (L.) Britton, Persea americana Mill, Carica papaya L. and Solanum betaceum Cav. Of all recorded species, 32% and 27% were herbs and trees, respectively. Among plant parts, leaves were most frequently utilized (27%) and roots (26%), while decoctions (21%) were the most widely used formulations. The dominant family was Asteraceae, with 28 species, followed by Lamiaceae, with 19 species. The highest ICF value was 0.778 for a number of parasitic and infectious illnesses, including ringworms, athlete's foot rot, tetanus, typhoid, intestinal parasites, abscesses, malaria, and amoebiasis. The study's data validates the region's widespread use of traditional medicinal plant remedies. Conclusion: The current study will lay a foundation of knowledge for future research investigations. The abundance of knowledge regarding ethnomedicinal species and their medicinal applications will stimulate further phytochemical and pharmacological research, which could lead to the discovery of potentially significant pharmaceuticals.

Project description:Productions of various bacterial traits like production of virulence factors (e.g. toxins, enzymes), biofilm formation, luminescence among others, have been known to be controlled by quorum sensing (QS), a process that is dependent on chemical signals or autoinducers (AIs). Bacteria known to rely on such AIs are known to be virulent and tend to be resistant against various antimicrobial agents. Therefore, strategies aimed at the inhibition of QS pathways, are regarded as potential novel therapies in managing bacterial virulence hence reducing their ability to induce infections in humans. In the present study, a portfolio of 25 medicinal plant extracts (ethanol 50% v/v) used in southwestern Kenya were assayed against a transformed E. coli Top 10 reporter QS strain. This biosensor responds to the exogenous addition of 3-oxo-N-hexanoyl homoserine lactone (3OC6HSL) expressing green fluorescent protein (GFP). The large majority of the screened medicinal plants seemed to exhibit toxic effects and almost none of them induced antiquorum sensing (AQS) activity. This could be the consequence of the presence of mixed compounds in the extracts. Elaeodendron buchananii Loes and Acacia gerrardii Benth extracts that seemed to show AQS activity were further proved found to possess mild AQS but with defined antimicrobial activities, and no antibiofilm formation inhibition. As a control, an E. coli pBCA9145_jtk2828::sfGFP strain that produces constitutively GFP was used and confirmed that none of the two extracts quenched the fluorescence of sfGFP. Cytotoxicity assays with mammalian MDCK cells also did indicate that the selected extracts with putative AQS activity, also reduced the cell viability. Therefore, further studies will be needed to separate and re-test the individual compounds especially from the selected two promising plants.

Project description:BackgroundMalaria remains a major health problem worldwide especially in sub-Saharan Africa. In Kenya, 80% of the population is at risk of contracting the disease. Pregnant mothers and children under five years are the most affected by this disease. Antimalarial drug resistance poses a major threat in the fight against malaria necessitating continuous search for new antimalarial drugs. Due to inadequate and inaccessible health facilities, majority of people living in rural communities heavily depend on traditional medicine which involves the use of medicinal plants for the management of malaria. Most of these indigenous knowledge is undocumented and risks being lost yet such information could be useful in the search of new antimalarial agents.Aim of studyAn ethnobotanical survey was carried out among the Luhya community of Kakamega East sub-County, a malaria epidemic region, with the aim of documenting the plants used in the management of malaria.Materials and methodsSemi-structured questionnaires were used to collect information from 21 informants who included traditional medicine practitioners and other caregivers who had experience in use of plants in management of malaria. These were drawn from 4 villages located in Kakamega East sub-county, within Kakamega County based on their differences in topography. Information recorded included plant names, parts used, mode of preparation and administration and the sources of plant materials. A literature search was conducted using PubMed and google scholar to identify the reported traditional uses of these plants and studied antiplasmodial activities.ResultsIn this study, 57% of the informants were aged above 50 years and a total of 61% had either no formal education or had only attained primary school education. A total of 42 plant species belonging to 24 families were identified. Most plants used in the management of malaria in this community belonged to Lamiaceae (18%), Leguminosae (9%) and Compositae (9%) plant families. Plants mostly used included Melia azedarach L, Aloe spp, Ajuga integrifolia Buch. Ham, Vernonia amygdalina Del., Rotheca myricoides (Hochst.) Steane and Mabb, Fuerstia africana T.C.E.Fr., Zanthoxylum gilletii (De Wild.) P.G.Waterman and Leucas calostachys Oliv. Rumex steudelii Hochst.ex A. Rich and Phyllanthus sepialis Müll. Arg are reported for the first time in the management of malaria. Although Clerodendrum johnstonii Oliv. (Jeruto et al., 2011) and Physalis peruviana L.(Ramadan et al., 2015) are reported in other studies for management of malaria, no studies have been carried out to demonstrate their antiplasmodial activity. The plant parts mostly used were the leaves (36%) and stem barks (26%). Majority of these plants were prepared as decoctions by boiling and allowed to cool before administration (66%) while infusions accounted for 28% of the preparations. The literature mined supports the use of these plants for the management of malaria since most of them have demonstrated in-vitro and in-vivo antiplasmodial activities.ConclusionMost of the reported plant species in this study have been investigated for antiplasmodial activity and are in agreement with the ethnomedical use. Two (2) plants are reported for the first time in the management of malaria. There is need for documentation and preservation of the rich ethnomedical knowledge within this community given that most of the practitioners are advanced in age and less educated. There is also the danger of over-exploitation of plant species as most of them are obtained from the wild, mainly Kakamega forest. Therefore, there is need for determining the economically and medicinally important plants in this community and planning for their preservation.

Project description:The prevalence of kidney-related diseases has been increasing and has emerged globally as a leading cause of mortality, especially in developing countries where they are considered a neglected public health problem. Renal diseases are commonly progressive and may cause irreversible loss of organ function, eventually necessitating renal replacement therapy. Although different pharmaceuticals are considered for the treatment of these pathologies, the uncertain effectiveness and presence of adverse effects have generated a growing need for the development of novel nephroprotective compounds. Because many medicinal herbs are typically used in Brazilian folk medicine to prevent and cure kidney ailments, ethnomedicine may play a promising and strategic role in identifying and adding new potential molecules to the pharmacological arsenal. This review focuses on the use of plants and secondary metabolites belonging to different classes to treat renal diseases, associating the screened plant extracts with the bioactive components present in each species. Flavonoids and triterpenes are notable metabolites that have therapeutic potential. The putative pharmacological mechanisms related to nephroprotective properties are also discussed in in vitro and in vivo models, when available.

Project description:BACKGROUND:During pregnancy or lactating, adequate nutrition for adolescents becomes critical to reduce risks for both child and maternal-related morbidity and mortality. Power dynamics play a massive role in health outcomes. The main objective of this study was to examine the power dynamics in the families and communities and their impact on the pregnant and lactating adolescent girls' access and utilization of nutrition services in Trans-Mara East Sub-County, Narok County. METHODS:A cross-sectional approach that employed mixed methods with both quantitative and qualitative research was adopted. Probability proportionate to size sampling techniques using cluster and simple random methods were used to practically access pregnant or lactating adolescents. Data was collected using questionnaires, in-depth interview and Focus Group Discussion. Quantitative data was analyzed descriptively using frequencies and inferentially using odds ratio and Z-test. Framework analysis was employed to analyze qualitative data. P ≤ 0.05 was considered statistically significant. RESULTS:In the power dynamics analyses, the intrinsic capability (Intrinsic capabilities are those adolescent driven initiatives that facilitate their access to nutrition services) was more likely to decrease awareness by half (OR = 0.52, 95% CI = 0.4-0.7, P < 0.01) whereas extrinsic dependency was likely to increase utilization by 1.2 times (OR = 1.2, 95% CI = 1.0-1.5, P = 0.055). From the stakeholder power matrix, the health personnel had observable visible power to influence access and utilization of nutrition services. Additional results revealed that adolescents who draw their support from significant others were more likely to utilize nutrition services as compared to those who attempted to make their own efforts to seek these services. Furthermore, health personnel have the most influential powers in ensuring adolescents access services and thus the most important actors in the stakeholder matrix. Other actors requiring focus included parents, political figures and governments while stakeholder engagement have higher potential of increasing access and utilization of services through dialogue. CONCLUSIONS:Community access to nutritional services can be increased through use of multiple avenues to reach adolescents, including school-based, health system-based, community-based approaches and even marriage registries. A heightened engagement in the identified stakeholder network is necessary when planning community conversations, to ensure a multi-stakeholder approaches in meeting the nutrition needs of adolescents.

Project description:BackgroundIt has been established that use and utilization of nutrition services among adolescents are highly linked to availability, access, cost and quality of care. The main objective of this study was to assess the socio-demographic and facility-based factors as proxies to access to perceived quality of nutrition-specific and nutrition-sensitive services among adolescents in Trans-Mara East Sub-County, Narok County.MethodsThe study adopted a cross-sectional approach that employed mixed methods on 291 households. Probability proportionate to size sampling techniques using cluster and simple random methods were used to practically access adolescents who are pregnant or lactating. Data were collected using questionnaires, in-depth interview and Focus Group Discussion. Quantitative data was analyzed descriptively using frequencies and inferentially using odds ratio and Z-test. Framework analysis was employed to analyze qualitative data.ResultsA nutritionist was more likely to increase overall utilization (considered as a proxy index to access quality nutrition-sensitive and -specific services) by 3.18 times (OR = 3.18, 95% CI = 1.50-6.60, P = 0.002) and nurses 2.7 times (OR = 2.70, 95% CI = 1.40-5.30, P = 0.005). Generally, 80.7 and 69.4% attached positive value to environmental and basic personal hygiene, respectively, as being areas of nutrition-sensitive service delivery with a significant number higher than expected frequency of 50% (P < 0.05). An assessment of facility networks isolated only public health center as the key determinant of overall utilization. Public health centers among other health facilities were more likely to increase utilization (OR = 4.52, 95% CI = 1.50-13.50, P = 0.007). Assessment of distance to facility identified both distances as key determinants of overall utilization as those resident < 1 km from the facilities were 2.4 times more likely to utilize the facilities (OR = 2.42, 95% CI = 1.20-4.80, P = 0.012) while those resident 1-5 km were 5.3 times more likely to utilize the services (OR = 5.34, 95% CI = 1.90-15.10, P = 0.002) relative to longer distances. Finally, on methods of conveying messages, those who received messages through Information Education and Communication (IEC) materials were 7.8 times (OR = 7.85, 95% CI = 1.50-40.50, P = 0.014) and through face-to-face were 3.9 times more likely to utilize the services (OR = 3.91, 95% CI = 1.30-11.90, P = 0.016).ConclusionCritical facility-based determinants of utilization of nutrition services include personnel (mainly nutritionist and nurse), distance and IEC materials.
