Dual-energy CT for automatic organs-at-risk segmentation in brain-tumor patients using a multi-atlas and deep-learning approach.

ABSTRACT: In radiotherapy, computed tomography (CT) datasets are mostly used for radiation treatment planning to achieve a high-conformal tumor coverage while optimally sparing healthy tissue surrounding the tumor, referred to as organs-at-risk (OARs). Based on CT scan and/or magnetic resonance images, OARs have to be manually delineated by clinicians, which is one of the most time-consuming tasks in the clinical workflow. Recent multi-atlas (MA) or deep-learning (DL) based methods aim to improve the clinical routine by an automatic segmentation of OARs on a CT dataset. However, so far no studies investigated the performance of these MA or DL methods on dual-energy CT (DECT) datasets, which have been shown to improve the image quality compared to conventional 120 kVp single-energy CT. In this study, the performance of an in-house developed MA and a DL method (two-step three-dimensional U-net) was quantitatively and qualitatively evaluated on various DECT-derived pseudo-monoenergetic CT datasets ranging from 40 keV to 170 keV. At lower energies, the MA method resulted in more accurate OAR segmentations. Both the qualitative and quantitative metric analysis showed that the DL approach often performed better than the MA method.

Project description:Organ delineation is crucial to diagnosis and therapy, while it is also labor-intensive and observer-dependent. Dual energy CT (DECT) provides additional image contrast than conventional single energy CT (SECT), which may facilitate automatic organ segmentation. This work aims to develop an automatic multi-organ segmentation approach using deep learning for head-and-neck region on DECT. We proposed a mask scoring regional convolutional neural network (R-CNN) where comprehensive features are firstly learnt from two independent pyramid networks and are then combined via deep attention strategy to highlight the informative ones extracted from both two channels of low and high energy CT. To perform multi-organ segmentation and avoid misclassification, a mask scoring subnetwork was integrated into the Mask R-CNN framework to build the correlation between the class of potential detected organ's region-of-interest (ROI) and the shape of that organ's segmentation within that ROI. We evaluated our model on DECT images from 127 head-and-neck cancer patients (66 training, 61 testing) with manual contours of 19 organs as training target and ground truth. For large- and mid-sized organs such as brain and parotid, the proposed method successfully achieved average Dice similarity coefficient (DSC) larger than 0.8. For small-sized organs with very low contrast such as chiasm, cochlea, lens and optic nerves, the DSCs ranged between around 0.5 and 0.8. With the proposed method, using DECT images outperforms using SECT in almost all 19 organs with statistical significance in DSC (p<0.05). Meanwhile, by using the DECT, the proposed method is also significantly superior to a recently developed FCN-based method in most of organs in terms of DSC and the 95th percentile Hausdorff distance. Quantitative results demonstrated the feasibility of the proposed method, the superiority of using DECT to SECT, and the advantage of the proposed R-CNN over FCN on the head-and-neck patient study. The proposed method has the potential to facilitate the current head-and-neck cancer radiation therapy workflow in treatment planning.

Project description:ObjectivesLate contrast enhancement CT (LCE-CT) visualizes the presence of myocardial infarcts. Differentiation of the contrast-enhanced infarct from blood pool is challenging. We developed a novel method using data from first pass CT angiography (CTA) imaging to enable automatic infarct detection.Materials and methodsA canine model of myocardial infarction was produced in 11 animals. Two months later, first pass CTA (90 kVp) and LCE-CT (dual energy 90 kVp/150 kVp tin filtered) were performed. Late gadolinium enhancement MRI was used as reference standard. The CTA and LCE-CT were co-registered using a fully automatic non-rigid method based on curved B-splines. The method allowed for limited elastic deformation and the considerable differences in attenuation between first-pass and delayed image. The blood pool was easily identified on the CTA image by high attenuation. Because CTA and LCE-CT were registered, the blood pool segmentation can be directly transferred to the LCE-CT - thereby solving the key problem of infarct/blood pool differentiation. The remaining segmentation of infarcted vs. noninfarcted myocardium was performed using a threshold. Automatic and MRI-guided expert segmentations of LCE-CT infarcts were compared to each other on volume and area basis (intraclass correlation coefficient, ICC) and on voxel basis (dice similarity coefficient, DSC between automatic and expert CT segmentation). CT infarct volumes were compared with the reference standard MRI.ResultsThe infarcts were mainly subendocardial (81%) and relatively small (median MRI infarct mass 7.4 g). The automatic segmentation showed excellent agreement with expert segmentation on volume and area measurements (ICC = 0.96 and 0.87, respectively). DSC showed moderately good agreement (DSC = 0.47). Compared to MRI there was modest agreement (ICC = 0.62) and excellent correlation (R = 0.9). Manual interaction was less than 1 min per exam.ConclusionWe propose an automatic method for infarct segmentation on LCE-CT using multiphase CT information, which showed excellent agreement with expert readers and favorable correlation with MRI.

Project description:Multi-atlas segmentation has been shown to be highly robust and accurate across an extraordinary range of potential applications. However, it is limited to the segmentation of structures that are anatomically consistent across a large population of potential target subjects (i.e., multi-atlas segmentation is limited to "in-atlas" applications). Herein, the authors propose a technique to determine the likelihood that a multi-atlas segmentation estimate is representative of the problem at hand, and, therefore, identify anomalous regions that are not well represented within the atlases.The authors derive a technique to estimate the out-of-atlas (OOA) likelihood for every voxel in the target image. These estimated likelihoods can be used to determine and localize the probability of an abnormality being present on the target image.Using a collection of manually labeled whole-brain datasets, the authors demonstrate the efficacy of the proposed framework on two distinct applications. First, the authors demonstrate the ability to accurately and robustly detect malignant gliomas in the human brain-an aggressive class of central nervous system neoplasms. Second, the authors demonstrate how this OOA likelihood estimation process can be used within a quality control context for diffusion tensor imaging datasets to detect large-scale imaging artifacts (e.g., aliasing and image shading).The proposed OOA likelihood estimation framework shows great promise for robust and rapid identification of brain abnormalities and imaging artifacts using only weak dependencies on anomaly morphometry and appearance. The authors envision that this approach would allow for application-specific algorithms to focus directly on regions of high OOA likelihood, which would (1) reduce the need for human intervention, and (2) reduce the propensity for false positives. Using the dual perspective, this technique would allow for algorithms to focus on regions of normal anatomy to ascertain image quality and adapt to image appearance characteristics.

Project description:PurposeFast and accurate auto-segmentation on daily images is essential for magnetic resonance imaging (MRI)-guided adaptive radiation therapy (ART). However, the state-of-the-art auto-segmentation based on deep learning still has limited success, particularly for complex structures in the abdomen. This study aimed to develop an automatic contour refinement (ACR) process to quickly correct for unacceptable auto-segmented contours.Methods and materialsAn improved level set-based active contour model (ACM) was implemented for the ACR process and was tested on the deep learning-based auto-segmentation of 80 abdominal MRI sets along with their ground truth contours. The performance of the ACR process was evaluated using 4 contour accuracy metrics: the Dice similarity coefficient (DSC), mean distance to agreement (MDA), surface DSC, and added path length (APL) on the auto-segmented contours of the small bowel, large bowel, combined bowels, pancreas, duodenum, and stomach.ResultsA portion (3%-39%) of the corrected contours became practically acceptable per the American Association of Physicists in Medicine Task Group 132 (TG-132) recommendation (DSC >0.8 and MDA <3 mm). The best correction performance was seen in the combined bowels, where for the contours with major errors (initial DSC <0.5 or MDA >8 mm), the mean DSC increased from 0.34 to 0.59, the mean MDA decreased from 7.02 mm to 5.23 mm, and the APL reduced by almost 20 mm, whereas for the contours with minor errors, the mean DSC increased from 0.72 to 0.79, the mean MDA decreased from 3.35 mm to 3.29 mm, and more than one-third (39%) of the ACR contours became clinically acceptable. The execution time for the ACR process on one subregion was less than 2 seconds using an NVIDIA GTX 1060 GPU.ConclusionsThe ACR process implemented based on the ACM was able to quickly correct for some inaccurate contours produced from MRI-based deep learning auto-segmentation of complex abdominal anatomy. The ACR method may be integrated into the auto-segmentation step to accelerate the process of MRI-guided ART.
